A Simple Pattern of Movement is not Able to Inhibit Experimental Pain in FM Patients and Controls: an sLORETA Study.

Motor cortex activation seems to induce an analgesic effect on pain that would be different between patients with fibromyalgia (FM) and control subjects. This study was conducted to analyze the changes of the laser-evoked potentials (LEPs) induced during a finger tapping task in the FM patients and the controls employing a multi-dipolar analysis according to Standardized low resolution brain electromagnetic tomography (sLORETA) method. The LEPs from 38 FM patients and 21 controls were analyzed. The LEPs were recorded while subjects performed a slow and a fast finger tapping task. We confirmed …

Neurophysiologic assessment of small fibre damage in chemotherapy-induced peripheral neuropathy.

OBJECTIVE: In patients with chemotherapy-induced peripheral neuropathy (CIPN), demonstration of small fibre (SF) damage is important to understand chronic late effects. METHODS: Thirty patients having complaints compatible with possible CIPN following treatment with oxaliplatin or docetaxel were compared with 27 healthy subjects. All subjects were evaluated with quantitative sensory testing (QST) assessing SF function and laser evoked potentials (LEP). In addition, SF-damage was assessed using cutaneous silent periods evoked with electrical (El-CSP) and laser (Ls-CSP) stimuli. RESULTS: For L

Changes in cochlear oxygenation, microcirculation and auditory function during prolonged general hypoxia.

Changes in cochlear microcirculation and oxygenation and auditory function were examined in anesthetized guinea pigs during prolonged hypoxic ventilation (8% oxygen in nitrogen) for 1 h. Cochlear blood flow and perilymphatic oxygen partial pressure were measured using laser Doppler flowmetry and oxygen-sensitive microelectrodes. Auditory function was examined by recording cochlear microphonics, compound action potentials and auditory evoked brainstem response. Blood pressure and heart rate were monitored. During systemic hypoxia, the perilymphatic PO2 dropped on average to about 70% of the ini

Inward currents in primary nociceptive neurons of the rat and pain sensations in humans elicited by infrared diode laser pulses.

Radiant heat is often used to study nociception in vivo. We now used infrared radiation generated by a diode laser stimulator (wavelength 98 … ive neurons that underwent the same heat stimuli. The same diode laser elicited pain sensations and laser-evoked potentials in human subjects. No significant differences were seen between the pain thresholds in hairy and in glabrous skin, probably due to the deep penetration of this laser radiation. The mean pain threshold for stimuli > or =200 ms in humans was 2.5 +/- 0 …

Pain in cervical dystonia: Evidence of abnormal inhibitory control.

INTRODUCTION: Several observations would suggest that dystonic pain is not simply muscular in origin. While ascending nociceptive pathways are normal in cervical dystonia, it is unknown whether descending inhibitory pain pathways are also normal. METHODS: We applied a conditioned pain modulation protocol and concomitantly recorded laser evoked potentials in patients with cervical dystonia (n = 15), blepharospasm (n = 15) and healthy volunteers (n = 15). RESULTS: During the application of a heterotopic noxious conditioning stimulation, patients with cervical dystonia, but not with blepharospa

Postnatal changes in glutamatergic inputs of jaw-closing motoneuron dendrites.

Dendrites of masseter (jaw-closing) motoneurons (MMNs) are well developed and ramify extensively throughout the trigeminal motor nucleus and often extend into the adjacent reticular formation. It is possible that the dendrites have active properties, which are altered with the development of the orofacial musculoskeletal system. Thus, we examined the changes in somatic voltage responses evoked by photostimulation of the MMN dendrites by laser photolysis of caged glutamate from postnatal day (P) 2-5 and 9-12 rats. We photostimulated 39 spots that were arranged around each recorded neuron in a c

Warmth and nociceptive evoked potentials in cold-induced sweating syndrome type 1.

INTRODUCTION: Cold-induced sweating syndrome type 1 (CISS1), is a rare, severe, autosomal recessive disease. It is characterized by morphological alterations and profuse sweating when ambient temperature is <22 °C. Although some individuals with CISS1 have decreased pain perception, no study has been conducted to evaluate thermal and pain sensations in these patients. The aim of this study was to assess the function of the nociceptive Aδ-fibers and warmth C-fibers by using CO2 laser-evoked potentials (LEPs) in patients affected by CISS1. METHODS: Four patients were studied. Laser pulses were …

Peripheral and central nervous system correlates in fibromyalgia.

BACKGROUND: Fibromyalgia (FM) is a syndrome characterized by altered pain processing at central and peripheral level, whose pathophysiologic mechanisms remain obscure. We aimed at exploring the structural changes of peripheral nociceptor measured by skin biopsy, the functional changes of central nociceptive pathway assessed by laser-evoked potentials (LEP), and their correlation with clinical features and comorbidities. METHODS: In all, 81 patients diagnosed with FM underwent skin biopsies with quantification of intraepidermal nerve fibre density (IENFD) at the thigh and distal leg, and LEP r…

Murine Autoimmune Optic Neuritis Is Not Phenotypically Altered by the Retinal Degeneration 8 Mutation.

PURPOSE: To investigate whether the presence of the retinal degeneration 8 (rd8) mutation in C57BL/6 mice alters the phenotype of autoimmune optic neuritis (AON). METHODS: C57BL/6J and C57BL/6N mice were genotyped for the rd8 mutation and fundus analyses and examination of retinal layer morphology were performed in vivo by scanning laser ophthalmoscopy and optical coherence tomography. Visual function was assessed by recording electroretinographs, and visual evoked potentials and retinae and optic nerves were assessed histopathologically. Retinal ganglion cell numbers were determined by retro

Functional Characterization of At-Level Hypersensitivity in Patients With Spinal Cord Injury.

UNLABELLED: At-level and above-level hypersensitivity was assessed in patients with chronic complete thoracic spinal cord injury (SCI). Patients were classified using somatosensory mapping (brush, cold, pinprick) and assigned into 2 groups (ie, patients with at-level hypersensitivity [SCIHs, n = 8] and without at-level hypersensitivity [SCINHs, n = 7]). Gender and age-matched healthy subjects served as controls. Quantitative sensory testing (QST), electrically- and histamine-induced pain and itch, laser Doppler imaging, and laser-evoked potentials (LEP) were recorded at-level and above-level i…

Laser microbeam axotomy and conduction block show that electrical transmission at a central synapse is distributed at multiple contacts.

Touch (T) sensory neurons in the leech innervate defined regions of skin and synapse on other neurons, including other T cells, within the ganglionic neuropil. The cells' receptive fields in the periphery are comprised of a central region, innervated by thick axons, and adjoining regions (minor fields) innervated by thinner axons. Secondary branches, known to be sites of synapses, emerge from the thinner and thicker axons. Pairs of T cells appear to make up to 200 separate contacts distributed within the neuropil. When the T cell is hyperpolarized, as occurs during natural stimulation of the c

Involvement of thin afferents in carpal tunnel syndrome: evaluated quantitatively by argon laser stimulation.

The thin afferent nerves were tested quantitatively by determining the thresholds of warmth and pricking pain to argon laser stimulation and by measuring the brain potentials related to pricking pain. In 27 patients with electrophysiologically verified carpal tunnel syndrome these parameters were measured from fingers 3 and 5 on both hands. All patients had had sensory symptoms ranging from 3 months to 25 years. Both the thresholds were elevated (P less than 0.05) at finger 3 compared to measurements from finger 5, and compared to finger 3 in a group of 39 controls. Four patients with symptoms

Diabetic neuropathy: A focus on small fibres.

Diabetic peripheral neuropathy (DPN) is diagnosed too late, which contrasts with our approach for diabetic retinopathy and nephropathy, where incipient disease is detected early enabling timely treatment. The 10-g monofilament and a foot exam are the commonly used methods for screening diabetic neuropathy, but this primarily identifies moderate to severe diabetic neuropathy. Small fibres are damaged early and are associated with the development of painful diabetic neuropathy, foot ulceration, and Charcot foot. Tests of small fibre damage include thermal thresholds, microneurography, evoked pot

ERG and VER findings after laser photocoagulation of the retina.

The present investigation analyzes the results of the ERG and VER studies after argon-laser photocoagulation of the papillomacular region and after panretinal argon-laser photocoagulation. The author stresses the possibility of photocoagulation of the papillomacular region without risk of damaging the papillomacular fibers as well as the observation that panretinal photocoagulation performed under identical parameters does not affect electric activity (and consequently the anatomical integrity of the retina) to the same extent in different eyes. The ERG could thus be used for an objective asse

Response of primary auditory neurons to stimulation with infrared light in vitro.

OBJECTIVE: Infrared light can be used to modulate the activity of neuronal cells through thermally-evoked capacitive currents and thermosensitive ion channel modulation. The infrared power threshold for action potentials has previously been found to be far lower in the in vivo cochlea when compared with other neuronal targets, implicating spiral ganglion neurons (SGNs) as a potential target for infrared auditory prostheses. However, conflicting experimental evidence suggests that this low threshold may arise from an intermediary mechanism other than direct SGN stimulation, potentially involvin

Brain activity affects dynamic but not static autoregulation.

A close correlation between neuronal activity and cerebral blood flow was documented (activation-flow coupling). The matter of an activity-related effect on autoregulation remains elusive. We modulated cortical activity by GABAergic inhibition in 10 pre-anestesized rats and compared effects on activation-flow coupling and autoregulation. Contralateral forepaw stimulation was used to assess activation-flow coupling. Somatosensory potentials as well as local flow velocity responses were recorded with surface electrodes and laser-Doppler flowmetry. Performing intermittent carotid compression we c

Pain perception in patients with chronic disorders of consciousness: What can limbic system tell us?

OBJECTIVE: Although it is believed that patients with Unresponsive Wakefulness Syndrome (UWS) do not feel pain, recent neuroimaging and neurophysiologic studies have demonstrated some residual traces of nociceptive processing. METHODS: To confirm this growing evidence, we evaluated 21 patients suffering from chronic disorders of consciousness (DOC) (both UWS, n=11, and Minimally Conscious State - MCS -, n=10), using an Event-Related Potential (ERP) Low-Resolution Brain Electromagnetic Tomography (LORETA) approach, based on nociceptive repeated laser stimulation (RLS). We delivered laser stimu
