Telavancin for the treatment of nosocomial pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA).

Telavancin is a bactericidal lipoglycopeptide antibiotic that is structurally related to vancomycin. It demonstrates in vitro activity against a variety of Gram-positive pathogens including, but not limited to, methicillin-resistant Staphylococcus aureus (MRSA). Telavancin is currently FDA-approved for the treatment of complicated skin and skin-structure infections. Recently, two randomized clinical trials demonstrated the efficacy and safety of telavancin compared to vancomycin for the treatment of nosocomial pneumonia. Overall, telavancin has a favorable safety profile. However, mild gastroi

Stewardship Challenges, PK/PD Evidence, and Prescribing Appropriateness for Recently Approved Antibiotics in Pediatric Practice.

The rise of multidrug-resistant (MDR) pathogens in the pediatric population represents a significant global health challenge, compounded by … tamase inhibitor combinations (e.g., ceftazidime/avibactam, meropenem/vaborbactam), and long-acting lipoglycopeptides. We analyze their approval trials, pediatric-specific PK/PD profiles, and the balance between on-label use and evidence-based off-label prescriptions. Furthermore, we emphasize the role of antimicrobial stewardship through the "3 D's" rule and the e …

Optimizing Treatment of Infective Endocarditis in Patients Who Inject Drugs.

The treatment of infective endocarditis (IE) in patients who inject drugs (PWID) is complex and challenging. These patients face stigma in t … inpatient hospital setting, including outpatient parenteral antibiotic therapy (OPAT), long-acting lipoglycopeptides, and oral antibiotic therapy for the treatment of endocarditis. Optimal treatment of IE in PWID involves support from a multidisciplinary healthcare team, management of substance use disorder, and effective antibiotic therapy.

Pharmacokinetics and safety of a single dose of telavancin in pediatric subjects 2-17 years of age.

Antimicrobial resistance increases infection morbidity in both adults and children, necessitating the development of new therapeutic options. Telavancin, an antibiotic approved in the United States for certain bacterial infections in adults, has not been examined in pediatric patients. The objectives of this study were to evaluate the short-term safety and pharmacokinetics (PK) of a single intravenous infusion of telavancin in pediatric patients. Single-dose safety and PK of 10 mg/kg telavancin was investigated in pediatric subjects >12 months to ≤17 years of age with known or suspected bacter

The future approach for the management of acute bacterial skin and skin structure infections.

PURPOSE OF REVIEW: To discuss the new available options for the treatment of acute bacterial skin and skin structure infections (ABSSSIs) and how to implement in the clinical practice innovative approaches for their management. RECENT FINDINGS: The availability of long-acting antibiotics, including dalbavancin and oritavancin, changed the approach to patients with ABSSSI. Direct discharge from the emergency department and early discharge from the hospital should be considered in patients with ABSSSI. Despite limited data about different bactericidal properties, the choice between dalbavancin

Fever for days: A challenging case of dalbavancin-induced fever.

Dalbavancin is a novel long acting lipoglycopeptide antibiotic with a favorable safety profile approved for treating Acute Bacterial Skin and Skin Structure Infections (ABSSSI) caused by Gram-positive organisms. Given its long half-life, a two-dose regimen can provide effective systemic therapy for up to six weeks, making it an appealing option to avoid prolonged intravenous antibiotic therapy. Herein, we report a case of a 27-year-old male who developed dalbavancin-induced fever while treating Methicillin-sensitive Staphylococcus aureus (MSSA) bacteremia. Despite being inconsistently reported

Telavancin-associated acute kidney injury
.

Telavancin is a lipoglycopeptide semi-synthetic derivative of vancomycin used for select infections caused by Gram-positive bacteria including Staphylococcus aureus. Human clinical trials suggest that telavancin is potentially nephrotoxic, however there is no histopathologic description of acute kidney injury (AKI) in humans. An animal model has recently characterized the histologic changes associated with telavancin-induced AKI as proximal tubular injury with numerous phagolysosomes. We present a case of a 47-year-old man with methicillin-resistant Staphylococcus aureus bacteremia treated wit

Telavancin (VIBATIV) for the treatment of complicated skin and skin structure infections.

Methicillin-resistant Staphylococcus aureus has emerged as a major causative pathogen in complicated skin and skin structure infections (cSSSIs). Unfortunately, treatment failure with vancomycin has been increasingly reported. Over the past decade, several alternative antimicrobial agents have been studied and approved for the treatment of cSSSIs. One such agent is the lipoglycopeptide telavancin, which was approved by the US FDA 2009. Given its dual mechanism of action, telavancin is characterized by a highly bactericidal activity and low potential for resistance selection. In addition, in cl
