Involvement of reduced ENPP1 function in the pathogenesis of ossification of the posterior longitudinal ligament.

Ossification of the posterior longitudinal ligament (OPLL) is an ectopic ossification disorder with incompletely understood pathophysiology. In a previously reported 50-patient OPLL cohort, ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1) exonic heterozygous variants were identified in 7 patients (14%), and plasma inorganic pyrophosphate (PPi), a biochemical marker reflecting ENPP1 function, was lower in patients with OPLL than in cervical osteoarthritis controls regardless of ENPP1 variant status. However, the clinical significance of ENPP1 variants and reduced PPi in OPLL, as well

A TACAF-based classification system for thoracic posterior longitudinal ligament ossification.

BACKGROUND AND OBJECTIVES: To evaluate the reliability and clinical applicability of a novel classification system for thoracic posterior longitudinal ligament ossification (OPLL) and its utility in guiding surgical approach selection for anterior controllable ante-displacement fusion (TACAF). METHODS: Based on anatomical and clinical characteristics, thoracic OPLL was classified according to: Grades 1-4 (severity), Zones A-B (location), and Arc morphology (kyphotic curvature). Twenty surgeons independently assessed 50 cases to evaluate system reliability. Fleiss kappa coefficients determined

Dyslipidemia as a novel risk for the development of symptomatic ossification of the posterior longitudinal ligament.

BACKGROUND CONTEXT: Obesity and visceral fat have been implicated as potential factors in the pathogenesis of the ossification of the posterior longitudinal ligament (OPLL); the details of the factors involved in OPLL remain unclear. PURPOSE: We aimed to determine the association between dyslipidemia and symptomatic OPLL. STUDY DESIGN: Single institution cross-sectional study. PATIENT SAMPLE: Data were collected from Japanese patients with OPLL (n=92) who underwent whole-spine computed tomography scanning. Control data (n=246) without any spinal ligament ossification were collected from 627

Analysis of intervertebral disc degeneration in patients with ossification of the posterior longitudinal ligament.

BACKGROUND: As degenerative disease with nerve compression, little is known about the relationship between cervical ossification of the posterior longitudinal ligament (OPLL) and intervertebral disc degeneration (IVDD). This study investigates the effect of ossification of ligaments on IVDD in patients with cervical OPLL. We focus on the ossification-related segment in patients diagnosed with OPLL, which is characterized by ossification that crosses intervertebral space or occurs concurrently in the upper and lower vertebrae. METHODS: A total of 92 ossification-related segments from 40 patien

Ubiquitin-dependent proteolysis of CXCL7 leads to posterior longitudinal ligament ossification.

Ossification of the posterior longitudinal ligament (OPLL), a spinal ligament, reduces the range of motion in limbs. No treatment is currently available for OPLL, which is why therapies are urgently needed. OPLL occurs in obesity, is more common in men, and has an onset after 40 years of age. The mechanisms underlying OPLL remain unclear. In this study, we performed a serum proteomic analysis in both OPLL patients and healthy subjects to identify factors potentially involved in the development of OPLL, and found reduced levels of a protein that might underlie the pathology of OPLL. We isolated

Surgical management of symptomatic ossified anterior longitudinal ligament: A case report.

BACKGROUND: Ossified anterior longitudinal ligament (OALL) of the cervical spine can cause dysphagia, dyspnoea, and dysphonia, although these symptoms are rare. CASE DESCRIPTION: A 71-year-old male presented with gradually progressive dysphagia secondary to OALL. He underwent fiber optic endoscopy and lateral video fluoroscopy. The OALL extended from C4 to C7 and contributed to significant compression of the pharynx as demonstrated on plain cervical radiography, magnetic resonance (MR) imaging, and computed tomography (CT). Following microsurgical resection of the OALL, his symptoms improved.

Resection of uncovertebral joints and posterior longitudinal ligament for cervical disc arthroplasty.

The most commonly accepted indications for cervical disc arthroplasty (CDA) are 1- and 2-level cervical disc herniation or spondylosis causing radiculopathy or myelopathy that is refractory to medical management. Unlike anterior cervical discectomy and fusion (ACDF), which eliminates motion, CDA aims to restore the physiological range of motion of the indexed joint. Thus, the effect of indirect decompression gained by the insertion of a sufficiently large interbody graft and incorporation into arthrodesis after ACDF cannot be duplicated for CDA. For patients undergoing CDA, during extreme flex

Comparative proteomic tissue analysis in patients with ossification of the posterior longitudinal ligament.

OBJECTIVE: The ossification of the posterior longitudinal ligament (OPLL) involves the ligament that lines the posterior surface of the spinal vertebral bodies. Hormonal and metabolic factors as well as hereditary factors have been proposed to be involved in pathologic ligamentous OPLL. However, there are currently no definitive serological biomarkers for OPLL that might be used to achieve a more convenient and economic diagnosis. To find an easier and simpler diagnostic method and to identify pathogenic proteins associated with OPLL, we assessed PLL tissues from patients with OPLL for proteom

Long-term outcome of laminectomy for cervical ossification of the posterior longitudinal ligament.

OBJECT: Although laminectomy is an effective surgical technique for the treatment of multilevel cervical stenotic lesions, postoperative kyphosis and neurological deterioration have been frequently reported after laminectomy. Hence, laminectomy without fusion is seldom performed nowadays. However, the clinical impression from the long-term follow-up of patients who had undergone laminectomy does not support that postoperative kyphosis is common in patients with ossification of the posterior longitudinal ligament (OPLL). In this paper, the authors assessed the long-term outcome of laminectomy f

[Involvement of the anterior longitudinal ligament of the spine in the morphogenesis of osteophytes in spondylosis deformans].

Morphology and relief of the mineralizing front of osteophytes associated with lumbar spondylosis have been studied by means of scanning electron microscopy. During osteophyte formation deep-laid fibers of anterior longitudinal ligament of the spine undergo mineralization followed by their resorption and subsequent replacement by bone structures. Osteophyte surface at its apex consists of mineralized fibers of longitudinal ligament of the spine, formed by mineralized cartilage structures in the area adjacent to the intervertebral disk and by resting bone surface with zone of bone formation and
