Effect of oral high frequency ventilation by jet or oscillator on minute ventilation in normal subjects.

Normal subjects were asked to breathe through an open ended tube while high frequency oscillations were superimposed on tidal breathing via a side arm, either an eight inch (20 cm) loudspeaker or a jet ventilator being used. Both systems were comfortable and well tolerated. Spontaneous minute ventilation fell by 19-46% at frequencies up to 33 Hz without a rise in transcutaneous PCO2. Maximum ventilatory savings occurred at 1.6 Hz with the jet ventilator (p less than 0.01) and at a frequency corresponding to respiratory system resonance with the loudspeaker. This suggests that during oral high

The effect of lung volume on regional gas transport during high frequency oscillations.

Boluses of 133Xenon entrained in oscillating flow during high frequency oscillation (HFO) were used to study the effects of lung volume on the distribution of gas to topographical lung regions. The distribution of the tracer was measured with scintillation detectors positioned 5, 15 and 25 cm below each lung apex of subjects seated in a plethysmograph, while they maintained a patent glottis and constant breathhold. An index of HFO gas transport was obtained by measuring the 'wash-in' time for Xenon count rates to attain 0.25-0.75 of their final magnitude within each region. The distribution of

Evaluation of a hot-wire respiratory flowmeter for clinical applicability.

A hot-wire flowmeter was evaluated for clinical applicability. 1) Calibration with a syringe could be done if emptying duration was 1-15 s. 2) Outputs linearly decreased with decreasing concentration of N2 in O2. Outputs with CO2 in O2 and N2O in O2 represented nonlinear convex relationships with varying concentrations of CO2 and N2O. Correction for each gas mixture to be measured is required. 3) Outputs linearly decreased with decreasing barometric pressure. 4) Stability assessed with a piston respirator was excellent (within +/- 2% of syringe volume) after 15 min warmup time. However, daily

Automated estimation of total lung volume using chest radiographs and deep learning.

BACKGROUND: Total lung volume is an important quantitative biomarker and is used for the assessment of restrictive lung diseases. PURPOSE: In this study, we investigate the performance of several deep-learning approaches for automated measurement of total lung volume from chest radiographs. METHODS: About 7621 posteroanterior and lateral view chest radiographs (CXR) were collected from patients with chest CT available. Similarly, 928 CXR studies were chosen from patients with pulmonary function test (PFT) results. The reference total lung volume was calculated from lung segmentation on CT or

Effect of lung volume on the respiratory action of the canine pectoral muscles.

Lung volume influences the mechanical action of the primary inspiratory and expiratory muscles by affecting their precontraction length, alignment with the rib cage, and mechanical coupling to agonistic and antagonistic muscles. We have previously shown that the canine pectoral muscles exert an expiratory action on the rib cage when the forelimbs are at the torso's side and an inspiratory action when the forelimbs are held elevated. To determine the effect of lung volume on intrathoracic pressure changes produced by the canine pectoral muscles, we performed isolated bilateral supramaximal elec

Influence of lung volume in sleep apnoea.

The influence of a constant increase in functional residual capacity on apnoea characteristics was studied in patients with the sleep apnoea syndrome. Pulmonary inflation was achieved by applying a continuous negative extrathoracic pressure into a Poncho type respirator. Nine patients slept in the Poncho for two consecutive nights, negative extrathoracic pressure being applied during the second night. There was no difference in the total sleep time, its composition within the different sleep stages, the apnoea and apnoea-hypopnoea indices, or the sleep time spent in apnoea between the two nigh

Echocardiographic aspects of pulmonary arterial hypertension in chronic lung disease.

An echocardiographic M-mode study has been performed on 58 patients with pulmonary arterial hypertension at rest or on exercise, with chronic lung disease, either obstructive (41 cases of COLD) or restrictive (17 cases of interstitial lung disease, ILD) in nature. The average Ppa was practically the same (i.e. 27.3 +/- 11.7 mmHg and 26.0 +/- 8.0 mmHg respectively) in the two groups of patients, even though the COLD patients were older than the ILD patients (58.7 +/- 9.3 vs 38 +/- 11 years). The echo detection of the pulmonary valve was successful in only 35% of COLD patients and in 86% of ILD

Effects of carotid body denervation on respiratory pattern of awake cats.

Eight awake cats have been studied before and after carotid denervation during air and oxygen breathing, and during hypercapnia. Analysis of the variables that characterize the spirogram shows that carotid denervation consistently results in a decrease of the mean inspiratory flow (VT/TI), causing a decrease in tidal volume (VT) and ventilation with a relative alveolar hypercapnia. In carotid-denervated animals, inhalation of oxygen results in an increase in ventilation due to an augmentation of VT/TI and VT and a relative hypocapnia. TI does not significantly change in the different condition

Changes in rib cage shape during quiet breathing, hyperventilation and single inspirations.

We measured the relative changes in upper and lower rib cage volume (delta RCU and delta RCL, respectively) using an induction coil plethysmograph (Respitrace) in seven normal seated subjects during relaxed passive expiration from total lung capacity, during quiet breathing, rapid breathing at 60 breaths/min (RB), during exercise-induced hyperventilation (EH) and during single fast inspirations. The plot of the RCU vs RCL was slightly curvilinear during relaxation from total lung capacity in all subjects. However, the inferred changes in rib cage shape were similar during RB and EH to those ob

Physiological underpinnings of exertional dyspnoea in mild fibrosing interstitial lung disease.

The functional disturbances driving "out-of-proportion" dyspnoea in patients with fibrosing interstitial lung disease (f-ILD) showing only mild restrictive abnormalities remain poorly understood. Eighteen patients (10 with idiopathic pulmonary fibrosis) showing preserved spirometry and mildly reduced total lung capacity (≥70% predicted) and 18 controls underwent an incremental cardiopulmonary exercise test with measurements of operating lung volumes and Borg dyspnoea scores. Patients' lower exercise tolerance was associated with higher ventilation (V̇E)/carbon dioxide (V̇CO2) compared with con

Airways disease: evolution, pathology, and recognition.

Obstruction to airflow in the lung was studied in 150 patients who presented with coin lesions necessitating surgical resection of the lung. Pulmonary function tests were performed before surgery, and the structure of the lung or lobe was analysed by means of morphometric techniques. The evolution of obstructive airways disease appeared to be based on a progressive chronic inflammatory reaction in the respiratory and membranous bronchioles. This led to progressive airway obstruction and was associated with destruction of the lung surface. The early pulmonary vascular problems appeared to be re

Sensitivity of bronchial responsiveness measurements in young infants.

OBJECTIVES: There is limited evidence on the preferred methods for evaluating lung function in infancy. The objective of this study was to compare sensitivity and repeatability of indexes of lung function in young infants during induced airway obstruction. METHODS: The study population consisted of 402 infants (median age, 6 weeks). Forced flow-volume measurements were obtained by the raised volume rapid thoracoabdominal compression technique and were compared with indexes of tidal breathing, measurements of transcutaneous oxygen (Ptco(2)), and auscultation during methacholine challenge testi

Continuous positive airway pressure reduces work of breathing and dyspnea during weaning from mechanical ventilation in severe chronic obstructive pulmonary disease.

Dynamic hyperinflation and the development of intrinsic positive end-expiratory pressure (PEEPi) are commonly observed in patients with severe chronic obstructive pulmonary disease (COPD) and acute respiratory failure. The presence of intrinsic PEEP acts as an inspiratory threshold load, and contributes significantly to the observed increase in work and oxygen cost of breathing. The present study examined the effects of continuous positive airway pressure (CPAP) (at 5, 10, and 15 cm H2O) and its ability to reduce the mechanical load imposed by PEEPi on breathing pattern, work of breathing, and

Capnodynamic monitoring of lung volume and blood flow in response to increased positive end-expiratory pressure in moderate to severe COVID-19 pneumonia: an observational study.

BACKGROUND: The optimal level of positive end-expiratory pressure (PEEP) during mechanical ventilation for COVID-19 pneumonia remains debated and should ideally be guided by responses in both lung volume and perfusion. Capnodynamic monitoring allows both end-expiratory lung volume ([Formula: see text]) and effective pulmonary blood flow (EPBF) to be determined at the bedside with ongoing ventilation. METHODS: Patients with COVID-19-related moderate to severe respiratory failure underwent capnodynamic monitoring of [Formula: see text] and EPBF during a step increase in PEEP by 50% above the ba

Optimizing high-frequency-oscillation ventilation using acoustic parameters of the newborn lung: a feasibility study.

Ventilation using high-frequency oscillation (HFO) has become a standard care for the ventilatory management of critically ill newborns. In recent years, there has been growing recognition that maintenance of an optimal lung volume during high-frequency oscillation plays an important role in minimizing ventilator-induced lung injury. The primary variable affecting lung volume is the mean airway pressure (MAP). To effectively maintain lung recruitment and optimal gas exchange without overstretching (or collapsing) the lung, MAP should be set between two well defined points in the pressure-volum
