Active surveillance for intermediate risk prostate cancer: the Manitoba Prostate Center experience.

OBJECTIVES: To report long-term oncological outcomes of men with intermediate-risk prostate cancer managed with active surveillance at a single centre. SUBJECTS/PATIENTS AND METHODS: We retrospectively analysed a prospectively maintained Manitoba Prostate Centre active surveillance database. Included were men with disease classed per National Comprehensive Cancer Network risk categories: very low-, low-, favourable intermediate-, and unfavourable intermediate-risk prostate cancer managed with surveillance (2004-2023). Active surveillance involved serial prostate-specific antigen testing, digi…

Fracture probability is predictive of fall-associated hospitalization: the Manitoba BMD Registry.

The current version of FRAX does not consider prior falls as a primary input variable. Limited data suggest that greater FRAX-derived fracture probability is associated with incident falls at least in elderly men. Our aim was to examine the association of FRAX-derived fracture probability with the risk for subsequent fall-associated hospitalizations. We identified individuals aged 40 yr or older at the time of baseline DXA with FRAX-derived major osteoporotic fracture (MOF) probability assessed through the Manitoba BMD Program. We used linkage with population-based data to identify subsequent …

Perspective: Incidence of Clinician-Diagnosed Lyme Disease in Manitoba, Canada 2009-2018.

Introduction: Lyme disease (LD) surveillance yields useful information to monitor the disease trends and spatial distribution. However, due to several factors, the Manitoba Health surveillance system, as with other systems, could be subject to underreporting. Objectives: To estimate the number and incidence of clinician-diagnosed LD over the study period in Manitoba, describe the epidemiology of clinician-diagnosed LD, and compare the findings with Manitoba Health LD surveillance data during the same period to estimate the extent of underreporting. Methods: A retrospective analysis of administ…

Gonorrhea cluster detection in Manitoba, Canada: Spatial, temporal, and spatio-temporal analysis.

In Canada, Gonorrhea infection ranks as the second most prevalent sexually transmitted infection. In 2018, Manitoba reported an incidence rate three times greater than the national average. This study aims to investigate the spatial, temporal, and spatio-temporal patterns of Gonorrhea infection in Manitoba, using individual-level laboratory-confirmed administrative data provided by Manitoba Health from 2000 to 2016. Age and sex patterns indicate that females are affected by infections at younger ages compared to males. Moreover, there is an increase in repeated infections in 2016, accounting f…

Incidence, Awareness, and Reporting of Sport-Related Concussions in Manitoba High Schools.

BACKGROUND AND OBJECTIVES: Federal and provincial governments in Canada are promoting provincial legislation to prevent and manage sport-related concussions (SRCs). The objective of this research was to determine the incidence of concussions in high school sport, the knowledge of the signs, symptoms, and consequences of SRC, and how likely student athletes are to report a concussion. METHODS: A cross-sectional survey of athletes (N = 225) from multiple sports in five high schools in one Manitoba school division was conducted. RESULTS: Participants in this study were well aware of the signs, …

Manitoba-oculo-tricho-anal (MOTA) syndrome is caused by mutations in FREM1.

BACKGROUND: Manitoba-oculo-tricho-anal (MOTA) syndrome is a rare condition defined by eyelid colobomas, cryptophthalmos and anophthalmia/microphthalmia, an aberrant hairline, a bifid or broad nasal tip, and gastrointestinal anomalies such as omphalocele and anal stenosis. Autosomal recessive inheritance had been assumed because of consanguinity in the Oji-Cre population of Manitoba and reports of affected siblings, but no locus or cytogenetic aberration had previously been described. METHODS AND RESULTS: This study shows that MOTA syndrome is caused by mutations in FREM1, a gene previously mu…

Geographical variations in the epidemiology of bacterial sexually transmitted infections in Manitoba, Canada.

Feasible epidemiological approaches are required to make a better assessment of the stage of an epidemic and to monitor its transition through various phases. Application of the Lorenz curve and Gini coefficient to summarise the inequality in STD incidence rates between jurisdictions in Manitoba, Canada, was found to provide useful insights into the concentration of these epidemics over time and thus their transition through epidemic phases. Further exploration of the statistical properties of these and other indices of inequality and their potential application to STD epidemiology is warrante…

Changing trends in β-hemolytic streptococcal bacteremia in Manitoba, Canada: 2007-2012.

OBJECTIVES: European surveillance studies have reported an increasing incidence of β-hemolytic group G streptococcal bacteremia, but no studies have evaluated trends in β-hemolytic streptococcal bacteremia in North America. METHODS: We reviewed bacteremic episodes and positive throat swab cultures from two tertiary care centers in Manitoba, Canada, from January 2007 to December 2012. RESULTS: During the study period, 19 864 bacteremic episodes, and 9948 positive throat swabs were identified. There were 1025 (5.16%) bacteremic episodes attributable to β-hemolytic streptococci: 425 (2.03%), 33…

Trabecular Bone Score (TBS) Predicts Fracture in Ankylosing Spondylitis: The Manitoba BMD Registry.

INTRODUCTION: Ankylosing spondylitis (AS) is a chronic inflammatory disease of the spine characterized among other features by spinal boney proliferation, back pain, loss of flexibility, and increased fracture risk. Overlying bone limits the utility of bone mineral density (BMD) by dual X-ray absorptiometry (DXA) in the spine. Trabecular bone score (TBS) is a bone texture measurement derived from the spine DXA image that indicates bone quality and fracture risk independent of BMD. METHODOLOGY: Using the Manitoba Bone Density Program database, patients with diagnosis codes for ankylosing spond…

Validation of administrative case ascertainment algorithms for chronic childhood arthritis in Manitoba, Canada.

We validated case ascertainment algorithms for juvenile idiopathic arthritis (JIA) in the provincial health administrative databases of Manitoba, Canada. A population-based pediatric rheumatology clinical database from April 1st 1980 to March 31st 2012 was used to test case definitions in individuals diagnosed at ≤15 years of age. The case definitions varied the number of diagnosis codes (1, 2, or 3), time frame (1, 2 or 3 years), time between diagnoses (ever, >1 day, or ≥8 weeks), and physician specialty. Positive predictive value (PPV), sensitivity, and specificity with 95% confidence interv…

Effectiveness of the quadrivalent human papillomavirus vaccine against cervical dysplasia in Manitoba, Canada.

PURPOSE: Effectiveness of the quadrivalent human papillomavirus (QHPV) vaccine against cervical dysplasia has not been estimated using population-based individual level data. We assessed the vaccine effectiveness (VE) of the QHPV vaccine against cervical dysplasia using data collected routinely in Manitoba. METHODS: Females ≥ 15 years old who received the QHPV vaccine in Manitoba between September 2006 and April 2010 privately (n = 3,541) were matched on age to up to three nonvaccinated females (n = 9,594). We used Cox regression models to estimate the hazard ratios for three outcomes: atypic…

Epidemiology of juvenile rheumatoid arthritis in Manitoba, Canada, 1975-92: cycles in incidence.

OBJECTIVE: To determine the yearly incidence of juvenile rheumatoid arthritis (JRA) and to seek correlations between this and cyclic infections occurring in the province of Manitoba, Canada, during the same period. METHODS: An estimate of the incidence of JRA in Manitoba was determined from a disease registry of the Pediatric Rheumatology Clinic, Children's Hospital, Winnipeg. The numbers of confirmed Mycoplasma pneumoniae and viral respiratory infections were determined from annual reports of Cadham Provincial Laboratory. Both facilities provide centralized services for the province. RESULT…

Developmental outcomes of preschool children requiring craniosynostosis surgery in Manitoba: a cohort study.

Craniosynostosis is a congenital abnormality resulting in the premature fusion of one or more cranial sutures and usually requires surgical correction in the first year of life. While the majority of craniosynostosis cases represent isolated, nonsyndromic defects, approximately 25% are linked to a genetic diagnosis. Craniosynostosis has been associated with developmental delay. Therefore, historically in Manitoba, all children with surgically managed craniosynostosis were referred to the Child Development Clinic (CDC) for developmental assessment. There are currently prolonged wait times for n…

Examining Early Childhood Health Outcomes of Children Born Late Preterm in Urban Manitoba.

Objective The late preterm population [34-36 weeks gestational age (GA)] is known to incur increased morbidity in the infancy stage compared to the population born at term (39-41 weeks GA). This study aimed to examine the health of these children during their early childhood years, with specific attention to the role of socioeconomic status. Methods A retrospective cohort study was conducted using data from the Manitoba Centre for Health Policy, including all live-born children born at 34-36 and 39-41 weeks GA in urban Manitoba between 2000 and 2005 (n = 28,100). Multivariable logistic regress…

Factors influencing H1N1 vaccine behavior among Manitoba Metis in Canada: a qualitative study.

BACKGROUND: During the first wave of the H1N1 influenza pandemic in 2009, Aboriginal populations in Canada experienced disproportionate rates of infection, particularly in the province of Manitoba. To protect those thought to be most at-risk, health authorities in Manitoba listed all Aboriginal people, including Metis, among those able to receive priority access to the novel vaccine when it first became available. Currently, no studies exist that have investigated the attitudes, influences, and vaccine behaviors among Aboriginal communities in Canada. This paper is the first to systematically …

Cloacal exstrophy and related abdominal wall defects in Manitoba: incidence and demographic factors.

A retrospective descriptive study of omphalocele and exstrophic defects of the abdominal wall was carried out in Manitoba to determine the prevalence of these defects, changes in birth incidence with time and their etiologic heterogeneity. The incidence of isolated omphalocele was similar to that reported in other series but the overall incidence of other types of abdominal wall defects was high. Cloacal exstrophy, bladder exstrophy and amnion disruption defects contributed significantly to the number of cases ascertained and emphasized the heterogeneity of abdominal wall defects. There were f…
