The indication for subcutaneous mastectomy is analyzed. Prospective investigations seem to confirm that different morphological forms of breast dysplasia (grade I, II, III, PRECHTEL [10]) do not implicate differences in risk for development of invasive breast cancer. Only if these forms of breast dysplasia are associated to high risk factors such as family history of breast cancer, should subcutaneous mastectomy be considered as a cancer prophylaxis.

Seventeen women who had had a mastectomy for cancer of the breast underwent reconstructions. Alloplastic implants were used in all. Preservation of the nipple and areola was possible in some of these patients. The normal (or uninvolved) breast sometimes required reduction in size or reshaping, to match as nearly as possible the reconstructed breast. The conditions suitable and unsuitable for mammary reconstruction, after mastectomy for cancer, are discussed.

Subcutaneous mastectomy appears to remain a procedure of promise. This 10-year experience indicated that it is feasible in at least 80 per cent of selected patients to successfully remove the breast parenchyma and subsequently reconstruct the breast without serious complications. It is resonable to assume that as surgical technique and experience improve, the complication rate will diminish. Galactorrhea, caused by hyperprolactinemia can successfully be treated by bromocriptine. Continuous long-term use of this drug is not always desirable or possible because of side-effects. In cases of hyperprolactinemia with severe galactorrhea, where surgical or radiological therapy is not possible and long-term treatment with bromocriptine is not acceptable, subcutaneous mastectomy might be considered as an ultimate solution.

A simple technique is described for removing the ductal tissue from the nipple in a subcutaneous mastectomy. We believe that complete removal of all the ductal tissue is desirable in this operation, and that it will also decrease the incidence of subsequent infection.
