New morphological and functional breast imaging techniques, progress in neoadjuvant treatments, and the development of new surgical techniques that include oncoplastic surgery, have made it possible to perform breast-conserving surgery in patients with large multifocal or multicentric breast tumours, where previously mastectomy would have been recommended. This allows for better aesthetic results, adequate oncological treatment, and improved quality of life. As a result, oncoplastic surgery has increased in local breast cancer treatment with radiologists playing a significant role in its plann

INTRODUCTION: Breast-conserving surgery (BCS) is often the first-line treatment for early-stage invasive breast cancer. Many studies claim that survival is non-inferior to radical treatments in selected cases. Although BCS preserves some breast tissue and often the nipple-areola complex, asymmetry and distortion of the breast contour may occur and reconstruction is sometimes required. The authors' aim is to conduct a review of main breast reconstruction options with local flaps after partial mastectomy.

PURPOSE: Breast radiologists examine the entire breast in full-size images, while breast pathologists examine small tissue samples at high magnification. The diagnostic information from these complementary imaging approaches can be difficult to integrate for a more clinically relevant evaluation of malignancies spanning several centimetres. We have explored the advantages and disadvantages of imaging guided larger section pathology techniques compared with the standard 2 × 2.5 cm. small section technique.
METHODS: We compared the ability of conventional small section histopathology with large

Minimally invasive anatomical sublobar resections have gained relevance in recent years mainly due to advances in imaging techniques, screening programs and the increase in second neoplasms. Accurate identification of the segmental or subsegmental bronchus is vital to guarantee optimal results in segmentectomies and subsegmentectomies. Given the complexity and the possibility of anatomical variations, several authors have published different methods to identify the target bronchus. However, these methods have certain limitations. This article describes a new rapid and effective technique, with

OBJECTIVE: Intraoperative specimen mammography (ISM) is a diffuse technique that allows surgeons to check specimens immediately after lumpectomy. Although the specimen is slightly compressed, the radiological image can be distorted by tissue overlap, and this may affect the evaluation of tumour borders, resulting in extension of the lumpectomy. As ISM may be less precise due to inadequate compression, a vacuum effect was applied to the specimen to increase the precision of margin detection.

The treatment of breast cancer is an evolving field. Different modalities are continuously being developed to maximize patient survival while minimizing the treatment's morbidity. Currently, the two main options for the management of primary breast cancer are total mastectomy and partial mastectomy with radiation. Although partial mastectomies (lumpectomy or quadrantectomy) conserve the nipple and areola complex and native breast tissue, asymmetry and distortion of the breast can still occur. Many methods of reconstruction have been described. The early and long-term effects of radiation also

We report the case of a 77-year-old man with a very rare breast tumor: hemangioma. The clinical examination found a large (6 cm diameter) lump and mammography revealed a well-defined high-density lesion with lobulated contour. Simple mastectomy was performed. The histology confirmed the diagnosis of benign hemangioma. There has been no local recurrence after 9 years of follow-up. There is very little literature on this type of tumor in men. Generally hemangiomas are large and ultrasound and magnetic resonance imaging (MRI) are useful for the diagnosis. The important differential diagnosis with

BACKGROUND: The incidence of occult breast cancer among patients undergoing reduction mammoplasty or risk-reducing mastectomies ranges from 1% to approximately 10%, respectively. Identification of incidental cancer often mandates subsequent mastectomy due to ambiguous margins. This study aimed to determine the incidence of contralateral malignancy among patients undergoing oncoplastic breast-conserving surgery (OBCS) with concurrent symmetry procedures.
METHODS: The authors reviewed their prospectively maintained institutional database of patients with unilateral breast cancer who underwent O

INTRODUCTION: Phyllodes tumors of breast are rare and usually benign. These are histologically fibro-epithelial tumors similar to fibroadenomas. Histological confirmation on the operative specimen is required to establish the diagnosis and histological pronostic of phyllode tumors.
MATERIALS AND METHODS: We reviewed 8 cases of phyllodes tumors and the literature to report the circumstances of occurrence of these tumors, and their specific clinical diagnosis, therapeutic, prognostic features.
RESULTS: 62.5% of patients were nulliparous. The mean age at diagnosis was 33.4 years. Mean tumor siz

In reference to a series of 36 in situ breast carcinomas, the current therapeutic possibilities are considered: 8 lobular carcinomas in situ (LCIS) and 28 ductal carcinomas in situ (DCIS) were diagnosed between January 1985 and July 1988. These are infraclinical forms in 80% of the cases, representing 20% of the breast carcinomas treated during the same period of time. The natural history of carcinomas in situ is presented; LCIS rarely evolve toward an invasive carcinoma and they should not be considered as risk factors. DCIS evolve, after biopsy, toward an infiltrating ductal carcinoma in 26

BACKGROUND: For individuals with breast cancer, oncoplastic breast surgery (OBS), is considered an innovative technique of breast conservative surgery (BCS), has emerged as the recommended substitute for mastectomy. BCS and mastectomy have been shown to have comparable locoregional control and overall survival in randomized trials. In order to ensure oncological safety and improved aesthetic results, Extreme Oncoplasty, a development in the field of oncoplasty, has given hope for patient to achieve ultimate conservation before mastectomy. BACKGROUND: Large randomized studies show equivalent survival after breast-conserving surgery (BCS) + adjuvant radiotherapy (RT) and mastectomy. In contrast, more recent observational studies suggest BCS + RT to be superior, but it is questionable whether patients in these treatment arms are comparable. Here, overall survival (OS) and breast cancer-specific survival (BCSS) after BCS + RT and mastectomy are compared within a randomized trial comparing intravenous and inhalation anaesthesia during breast cancer surgery.
METHODS: The patient cohort was recruited from the randomized CAN-study. Pa

BACKGROUND: The incidence of multiple ipsilateral breast cancers (MIBC), which includes multifocal and multicentric tumors, is increasing. Despite breast conserving surgery (BCS) being the mainstay of treatment in early breast cancer, it is considered controversial for MIBC due to oncological safety concerns. The aim was to compare oncological outcomes of BCS compared with mastectomy for patients with MIBC.
METHODS: A systematic literature search of relevant databases for comparative studies was conducted. A meta-analysis using the Mantel-Haenszel method was performed to calculate risk ratios

BACKGROUND: Evolution in breast oncology has increased the use of breast-conserving therapy and expanded the role of locoregional autologous reconstruction. While bilateral reduction is common for patients with large, ptotic breasts, those with small to moderate symmetric breasts often prefer unilateral volume restoration. This study demonstrates a free-style approach to flap selection and design for partial and total breast reconstruction, emphasizing intraoperative decision-making and preservation of muscle and nerve function.
METHODS: A retrospective review was performed on patients underg

BACKGROUND: Patients undergoing breast surgery are at risk of severe postoperative pain. Several opioid-sparing strategies exist to alleviate this condition. Regional anesthesia has long been a part of perioperative pain management for these patients.
AIM: This randomized study examined the benefits of interpectoral and pectoserratus plane block (IPP/PSP), also known as pectoralis nerve plain block, compared with advanced local anesthetic infiltration.
METHODS: We analyzed 57 patients undergoing partial mastectomy with sentinel node dissection. They received either an ultrasound-guided IPP/P

Breast cancers have broad histological subtypes with varied molecular expression, which determines the management. Although ductal and lobular breast carcinomas constitute the significant 90% of these tumours, other subtypes constitute about 10% of breast cancers. Adenoid cystic carcinoma (AdCC) is a salivary gland-type tumour described in breast, constituting less than 1% of all breast carcinomas. These tumours have a favourable prognosis and are surgically managed by either lumpectomy or mastectomy. Solid-basaloid and AdCC with high-grade transformation are the sporadic subtypes of AdCC, in

PURPOSE: To describe and analyze major local complications after intraoperative radiotherapy (IORT) with low-energy x‑rays during breast-conserving surgery (BCS) in early breast cancer.
METHODS: Ten women out of 408 who were treated with IORT between 2002 and 2017 and subsequently developed a severe local complication requiring surgical intervention were retrospectively identified and analyzed. Demographic, clinical, and surgical parameters as well as complication characteristics and treatment methods were evaluated.
RESULTS: At initial presentation, eight patients (80%) showed redness, six

PURPOSE: Breast conserving surgery (BCS) has become the preferred surgical option for the management of patients with nonmetastatic breast cancer in high-income countries. However, little is known about the distribution and determinants of BCS in low-and middle-income countries, especially those with high HIV prevalence.
METHODS: We compared demographic and clinical characteristics of female patients who received BCS and those who received total mastectomy (TM) for nonmetastatic invasive carcinoma of the breast in Soweto, South Africa, 2009-2011. We also developed a multivariable logistic reg

BACKGROUND: Breast conserving surgery (BCS) is increasingly offered to suitable patients diagnosed with early stage breast cancer. Occasionally the pathological margins on specimens following BCS are positive. The objective of this study is to assess the proportion of patients within our unit who required completion mastectomy after BCS and to determine if predictive factors could be identified to assist the breast surgeon identifying those patients at risk of positive margins following BCS.
METHODS: All patients diagnosed with breast cancer between 2001 and 2005 were reviewed. Patients under
