WHAT IS KNOWN AND OBJECTIVE: Mebendazole (MBZ) is a broad-spectrum antihelminthic agent of the benzimidazole type. Although MBZ has been reported to cause hepatic injury, case reports of severe hepatic injury are very rare. The therapeutic efficacy of Mebendazole (Vermox) was studied in 13 patients: 8 with liver and 5 with pulmonary echinococcosis. The drug was given in a dose of 50-70 mg/kg body mass. We report the case of a 47-year-old female patient, suffering from multiple hydatid cysts of the liver, in whom hepatitis developed after mebendazole treatment. Clinical manifestations of hypersensitivity were absent. A correlation was found between serum mebendazole concentrations and the degree of cytolysis; this is compatible with a direct hepatotoxic effect of mebendazole.

In clinical drug trials, mebendazole, given in various daily doses for up to 14 days to a maximum total of 16.8 g, was found to be ineffective against both adult Onchocerca volvulus and the microfilariae. Forty patients were treated with either flubendazole or mebendazole, 100 mg twice a day for three days, in a double-blind, prospective, randomized study. Mebendazole, a new broad-spectrum oral anthelmintic for nematodes, was given to patients harboring whipworm in a hospital for the mentally retarded. A three-day regimen (100 mg twice a day) dewormed 75% of the subjects, a five-day regimen cured 95%. The remaining patients excreted strikingly lesser amounts of the parasites' eggs after therapy. No untoward effects were noted clinically. It is the drug of choice in mixed worm infestations.
