Thoracoscopic resection of a giant mediastinal parathyroid cyst.

Parathyroid cysts are a rare situation, unusually in the mediastinum. The preoperative diagnosis could be more difficult in some atypical topographies and imaging characteristics in particular in case of huge mediastinal cyst. In the following years traditionally, in case of intrathoracic parathyroid cysts, sternotomy or thoracotomy have been the preferred approaches. We report a case of an older patient with a huge mediastinal parathyroid cyst removed successfully using videothoracoscopy.

Mediastinal bronchogenic cyst compressing left recurrent laryngeal nerve.

A 15-year-old boy developed recurrent laryngeal nerve compression by a bronchogenic cyst, causing recurrent choking. Bronchogenic cyst compressing the recurrent laryngeal nerve is unusual. The patient had a normal voice and occasional choking with periods of normalcy in between. A left posterolateral thoracotomy was performed and cyst was excised. Histopathology revealed bronchogenic cyst.

A thoracoscopically resected case of mediastinal parathyroid cyst.

A 67-year-old male visited his physician because of a 2-month history of cough and sputum. An abnormal shadow at the left upper mediastinum on chest x-ray film was detected, and the patient was referred to our department for further examination. Chest x-ray film revealed a round shadow at the left upper posterior mediastinum. Computed tomography(CT)revealed a uniform iso density mass about 4 cm in diameter, with a well-defined border. After the intravenous contrast administration, a slight peripheral enhancement was seen around the mass. On magnetic resonance imaging, the mass was hypointense

Recurrent respiratory obstruction from a mediastinal bronchogenic cyst.

A large mediastinal bronchogenic cyst presented acutely with paroxysmal atrial fibrillation and severe airflow obstruction. The patient had experienced identical symptoms on two other occasions over the previous 24 years. These had been previously misdiagnosed as due to a mediastinal lymphoma. Percutaneous extrapleural aspiration successfully decompressed the cyst with substantial improvement in lung function. Recurrent swelling of the cyst occurred that could not be relieved surgically. After repeat aspiration percutaneous instillation of bleomycin and alcohol has been used to prevent further

Cervico-mediastinal thymic cyst with vocal cord paralysis.

An unusual case of congenital cervico-mediastinal thymic cyst with vocal cord paralysis is presented together with a brief history and classification of these cysts. A discussion of thymic cyst symptomatology and the surgical approach to these lesions is included. Simple thymic cysts should be considered in the differential diagnosis of cervico-mediastinal masses, particularly in children and young adults.

Traumatic para-mediastinal air cyst. A case report.

The roentgenographic features of a paramediastinal air cyst following chest trauma are reported. After an initial period of enlargement the lesion completely resolved in one month. Experience with this patient and a review of similar cases reported in the literature suggests that lesions of this type may be observed for spontaneous resolution.

Giant mature mediastinal teratoma mimicking a pericardial cyst.

BACKGROUND: Mature mediastinal teratomas are uncommon benign germ cell tumors. When the cystic component predominates and typical features such as fat or calcification are absent, diagnosis can be misleading. A 17-year-old male had a brief syncopal episode two months prior. At presentation, his primary complaint was mild chest tightness. Chest computed tomography showed a 13.6 × 10.3 × 7.9 cm homogeneous cystic mass in the anterior mediastinum, closely attached to the pericardium. Attenuation measured 17 ± 15 Hounsfield units on non-contrast computed tomography, consistent with fluid density a

Pleural effusion revealing a ruptured mediastinal mesothelial cyst.

Mesothelial cysts are uncommon benign cysts of the mediastinum. Rarely, they are discovered after complications or unusual presentations. This report describes a rare case of pleural effusion revealing a ruptured mediastinal mesothelial cyst in a 28-year-old man. The diagnosis of this complicated mesothelial cyst relied on intraoperative and anatomopathological findings. He underwent videothoracoscopy with resection of the cyst. Outcomes were favourable.

Epidermoid Thymic Cyst - A Very Rare Mediastinal Mass.

OBJECTIVES: Thymic cysts represent 1-3% of all tumors in the anterior mediastinum, but their knowledge is important during the differential diagnosis of a mediastinal mass. METHODS: We describe the case of a 52 years old woman, totally asymptomatic, with an incidental diagnosis of a mediastinal mass at thoracic computed tomography evaluation. RESULTS: Thoracic magnetic resonance showed an heterogeneous mass with approximately 3,6 cm, at the anterior mediastinum. For diagnostic clarification, surgical excision was performed. The histopathological exam revealed an epidermoid thymic cyst. CONC

Video-assisted mediastinoscopic resection of mediastinal cyst (lymphangioma).

Mediastinal cysts account for 20% of all mediastinal masses. Symptomatic cysts need treatment, and surgical excision is the treatment of choice. One approach to resect them is video-assisted mediastinoscopy. An 80-year-old man with cough and dyspnea at rest was referred for evaluation. Chest computed tomography revealed a mediastinal cyst. Because of his symptoms, he was a candidate for surgery, and the cyst was resected by video-assisted mediastinoscopy through a cervical incision. Video-assisted mediastinoscopy is a safe and effective approach for complete resection of mediastinal cysts.

[Mediastinal hydatid cyst. Reflections on a case study].

Hydatid cyst is a zoonotic cycle parasitic infection caused by Echinococcus granulosus tapeworm, whose larvae infect man as an intermediate host. It is one of the most serious lung diseases caused by helminths. It is rare to find it in the mediastinum; < 1%. The primitive, or seemingly primitive, location is due to the parasite in the cellular tissue of the mediastinum. The authors report the case of a sixteen year old male, admitted to the E.R. for left posterior chest pain which had spread to the upper left limb.

Occipital encephalocele associated with mediastinal enteric duplication cyst.

Enteric duplication is a rare developmental anomaly. This anomaly can be seen anywhere along the gastrointestinal tract, and less than 2% of the cases extend to the mediastinum. Developmental neuroectodermal and cutaneous ectodermal abnormalities, especially vertebral midline fusion abnormalities, are frequently associated with enteric duplications. We report a case of a newborn with occipital encephalocele, also having a mediastinal enteric duplication cyst. The clinical course of this case is described in detail to emphasize the occurrence of life-threatening complications of these associate

[Anterior mediastinal cyst disclosed by protracted thoracic pain].

The authors report a case of anterior mediastinal cyst discovered incidentally in a 71-year-old man presenting with prolonged chest pain and a history of chronic renal failure secondary to polycystic kidney disease. The diagnosis of these cysts, which are benign congenital tumours usually asymptomatic and with a favourable course, was established by thoracic CT. In difficult cases, magnetic resonance imaging allows precise analysis of pericardial masses. Clinicians should think of this possibility in the presence of an opacity of the cardiophrenic angle associated with chest pain.
