Effect of a global longitudinal pathway on medical students' attitudes toward the medically indigent.

BACKGROUND: The increase in people with insufficient health insurance has largely been fueled by a record influx of immigrants, who often live in medically underserved areas of inner cities. An increase in the desire of medical students to practice in underserved areas is needed to counter low physician-to-population ratios in these areas. PURPOSE: To assess the effect of a Pathway on Serving Multicultural and Underserved Populations, which includes domestic and international experiences with recent immigrant groups, on the attitudes of students toward the indigent. METHODS: Students from th…

Frontier And Rural Medicine: A 10-Year Review.

South Dakota's largely rural and frontier population faces significant barriers to healthcare access, including a high prevalence of Health Professional Shortage Areas (HPSAs). To address these gaps, the University of South Dakota Sanford School of Medicine (USD SSOM) launched the Frontier and Rural Medicine (FARM) program in 2014. FARM provides longitudinal rural clinical training and community immersion to medical students, aiming to increase the number of physicians entering primary care and practicing in South Dakota. Residency match outcomes (2016-2025) and practice locations of FARM grad

Pain in underserved community-dwelling Chinese American cancer patients: demographic and medical correlates.

BACKGROUND: Little is known about cancer pain in Chinese Americans. The objective of this study was to describe the epidemiology of pain in this population. This information is needed to identify and address unmet clinical needs for culturally relevant interventions targeting pain and its consequences. METHODS: A consecutive sample of underserved ethnic Chinese patients in a large community-based oncology practice was screened for persistent or frequent pain. Those patients with pain completed translated instruments assessing demographics, linguistic acculturation, disease-related characteris

Co-Designing Digital Health Intervention for Monitoring Medication and Consultation Among Transgender People in Underserved Communities: Collaborative Approach.

BACKGROUND: In many parts of the world, men who have sex with men and transgender individuals face criminalization and discrimination. As a result, they are less likely to seek medical help, despite experiencing higher rates of HIV/AIDS, mental health issues, and other health problems. Reaching key populations (KPs) with essential testing, care, and treatment services can be challenging, as they often have a higher likelihood of contracting and spreading the virus. They have limited access to antiretroviral (ARV) therapy (ART) services, which means that KPs may continue to serve as reservoirs

[Medical caravan in Morocco: Mobile dermatology].

This publication is based on personal experiences gathered during participation in several "medical caravans". These caravans where under the leadership of either a Moroccan nongovernmental organization (NGO) or a Belgian NGO. Medical caravans are multidisciplinary mobile groups that offer basic medical care in rural areas. The location usually changes every 1-2 days. The physician team comprises more than 10 different specialties including midwives, psychologists, nurses, hearing instrument specialists and podologists. The work was supported by Moroccan caregivers (fluent in French, Arabic an

Evaluation of the North Country on the Job Network: a model of facilitated care for injured workers in rural upstate New York.

We describe the evaluation of a community-based program designed to facilitate access to care and return to work for injured workers in a rural, medically underserved area in upstate New York. Providers are recruited to provide easily accessible care and are oriented to concepts of transitional duty and rapid return to work as medically appropriate; companies are recruited with the agreement to provide transitional work for injured employees. Registered nurses, hired by the local hospital, serve as case coordinators to facilitate care and coordinate communications among all parties. Over 3000 …

Achieving Full Neurological Recovery in Snakebite using Best Supportive Care.

A 29-year-old woman presented to a community hospital in Sierra Leone 2 hours after being bitten by an unknown snake. On arrival, she was agitated though alert, however deteriorated into respiratory arrest. There was no local availability of antivenom. The patient remained in respiratory arrest undergoing best supportive care in a low-resource setting for 2 hours 55 minutes before returning to spontaneous ventilation. She went on to make a full neurological recovery. Though spontaneous recovery following snakebite envenoming is rare, this case showcases that good communication and basic manoeu

Implementing intervention mapping to tailor the evidence-based PREDIMED intervention for medically underserved prostate cancer patients.

BACKGROUND: Prostate cancer is the most common noncutaneous malignancy among men and disproportionately affects those with low socioeconomic status, particularly men from racial and ethnic minority populations. PURPOSE: This study describes the development of a culturally tailored Mediterranean diet intervention for medically underserved Black and Hispanic men with prostate cancer, using the Intervention Mapping Adaptation (IM ADAPT) framework. METHODS: Conducted at a county safety-net hospital in a large urban area, which serves a population with high medical needs and low socioeconomic sta

Using Data Science and a Health Equity Lens to Identify Long-COVID Sequelae Among Medically Underserved Populations.

Understanding how post-acute COVID-19 syndrome (PACS or long COVID) manifests among underserved populations, who experienced a disproportionate burden of acute COVID-19, can help providers and policymakers better address this ongoing crisis. To identify clinical sequelae of long COVID among underserved populations treated in the primary care safety net, we conducted a causal impact analysis with electronic health records (EHR) to compare symptoms among community health center patients who tested positive (n=4,091) and negative (n=7,118) for acute COVID-19. We found 18 sequelae with statistical

Addressing physician shortages in New Mexico through a combined BA/MD program.

The University of New Mexico School of Medicine and College of Arts and Sciences developed its combined BA/MD degree program, which will increase the medical school class from 75 students to 100 in the fall of 2010, to address the critical issue of physician shortages in underserved New Mexico. The program, which began operation at the undergraduate (i.e., college) level in 2006, expands opportunities in medical education for New Mexico students, especially those from rural and underserved minority communities, and prepares them to practice in underserved areas of New Mexico. In the BA/MD prog

Circuit librarianship: a twentieth anniversary appraisal.

The twentieth anniversary of circuit librarianship is a fitting occasion to reflect on its history and success. The tradition of circuit riders is traced and the hallmarks and achievements of circuit librarian programs are described. Providing information services to remote health professionals has been a longstanding concern of medical librarianship, and it is presently a major focus of the National Library of Medicine. It is suggested that widespread adoption of circuit librarianship would not only satisfy the information needs of unserved health professionals but could enhance the image of

Efficient engagement: opportunities for mobile methadone maintenance in geographically underserved areas.

In 2021 the Drug Enforcement Agency changed regulations to reduce barriers in the registration and operation of mobile methadone clinics for treating opioid use disorder in rural and underserved areas. How these routes might best be operated and how many clients might be served by mobile units are open questions. This work identifies candidate dispensing locations from mobile units in areas that do not have geographic access to a fixed clinic. Optimal routes for mobile methadone are identified by solving a mixed integer program that maximizes the number of clients without access to care that c

Professional identity and rural workforce commitment among regional quota medical students in Japan: a qualitative study.

INTRODUCTION: Many countries use rural admission pathways and bonded scholarship programs to address persistent shortages of physicians in underserved areas. In Japan, the Chiiki-Waku (regional quota) system was established to recruit and train medical students who are expected to contribute to rural and remote health care. However, variations in educational support and limited understanding of students' lived experiences may weaken the effectiveness of such programs in fostering long-term professional commitment. This study aimed to explore the learning experiences and educational needs of Ch

A low-cost decision support network for electrocardiograph transmission from oil rigs in the North Sea.

A telemedicine service consisting of electrocardiogram (ECG) interpretation and advice on the management of chest pain offshore was offered to oil rig installations in the North Sea. A total of 14 subscribing oil rigs were supplied with thrombolytic drugs and rig paramedics were trained in their delivery. Electrocardiographs could be recorded using a standard ECG machine and then digitized using a scanner for transmission as an email attachment. Several oil companies invested in ECG equipment which allowed direct recording of the patient's ECG in electronic form for transmission by email. Upta

Methodological and technical issues related to the diagnosis, screening, prevention, and treatment of pre-eclampsia and eclampsia.

In contrast with advances made in treating or eliminating many other serious disorders, severe morbidity and mortality associated with pre-eclampsia/eclampsia remain among the leading problems that threaten safe motherhood, particularly in developing countries. This article reviews technical issues related to diagnosis, screening, prevention, and treatment of pre-eclampsia and identifies corresponding needs. The authors stress the lack of standardized definitions of pre-eclampsia and eclampsia and discuss problems in blood-pressure measurements and assessment of urinary protein. They summarize

'It's very hard, it's very very difficult ….' Exploring the lived experiences of managing diabetes among young people living with type 1 diabetes in underserved communities in Ghana.

Sub-Saharan Africa (SSA) is undergoing an epidemiological transition which is driving increases in the risk and prevalence of diabetes and other non-communicable diseases (NCDs). In Ghana, while the prevalence of diabetes is generally increasing, type 1 diabetes mellitus (T1DM) is increasing at a higher rate (9.5 %) among young people compared to type 2 diabetes (4.9 %) mellitus (T2DM). T1DM differs from T2DM in that it is not readily preventable, children are often diagnosed late, or diagnoses are missed entirely, and living with T1DM requires a different approach to care. Young people living
