Insurance status and health-related quality-of-life disparities after trauma: results from a nationally representative survey in the US.

PURPOSE: To determine whether insurance status modifies the association between injuries and health-related quality of life (HRQOL) in a nationally representative sample of US adults. METHODS: This is a longitudinal, observational study using the pooled 2000-2006 Medical Expenditure Panel Survey (MEPS). A total of 50,225 adults (age ≥ 18) with or without injuries were included in the study. HRQOL was evaluated using the EuroQoL Health Index (EQ-5D), visual analog scale (VAS), and the SF-12 physical component score (PCS) and mental component score (MCS). A categorical variable of injury-insura

A long and winding road: federally qualified health centers, community variation and prospects under reform.

Community health centers have evolved from fringe providers to mainstays of many local health care systems. Those designated as federally qualified health centers (FQHCs), in particular, have largely established themselves as key providers of comprehensive, efficient, high-quality primary care services to low-income people, especially Medicaid and uninsured patients. The Center for Studying Health System Change's (HSC's) site visits to 12 nationally representative metropolitan communities since 1996 document substantial growth in FQHC capacity, based on growing numbers of Medicaid enrollees an

Outcomes among patients with isolated traumatic brain injury before and after Medicaid expansion.

INTRODUCTION: Insurance coverage is a critical determinant of access to care. Uninsured patients suffer poorer outcomes including increased risk of morbidity/mortality. To reduce uninsurance among adults, the Affordable Care Act provisioned states the option to expand Medicaid eligibility. We hypothesized that patients with isolated traumatic brain injury (TBI) had more insurance coverage and better outcomes after Medicaid expansion as compared with before. METHODS: National data on trauma admissions were obtained from the American College of Surgeons Trauma Quality Program Public Use File fo

Association between changes in out-ofpocket expenditures during 2020 with the Covid-19 pandemic in Mexico.

OBJECTIVE: To disentangle the contribution of the Covid-19 pandemic and the abolition of the Seguro Popular (SP) health protection system to changes in out-of-pocket health expenditures (OPE) in Mexico during 2020. MATERIALS AND METHODS: We used national representative surveys between 2014 and 2020. We described OPE in absolute and relative terms and per disposable income. Changes in OPE were estimated using adjusted two-part models. Results were reported nationally, by health insurance and income quintile. RESULTS: OPE increased for all in 2020 with the largest increases for the high- and v

Frailty and Quality of Life in Low-income, Uninsured Men With Prostate Cancer.

OBJECTIVE: To evaluate frailty across multiple domains and examine its association with prostate cancer (PCa)-specific health-related quality of life (HRQoL) in low-income, uninsured men enrolled in the Improving Access, Counseling and Treatment for Californians with Prostate Cancer (IMPACT) Program. MATERIALS AND METHODS: We retrospectively assessed frailty using a deficit accumulation frailty index, derived from the RAND SF-12v2 (12-Item Short Form Survey, version 2), California Health Interview Survey (CHIS), and Total Illness Burden Index for Prostate Cancer (TIBI-CaP). PCa-specific HRQoL

Increased Medicaid eligibility of Affordable Care Act: Evidence of improved outcomes for patients with peripheral artery disease.

Peripheral artery disease (PAD) is a debilitating disease that disproportionately affects people of low socioeconomic status and racial minority individuals. These groups also tend to have lower rates of revascularization and worse outcomes, including higher rates of major amputation. In 2010, the Affordable Care Act (ACA) was signed into law, providing better opportunities for health care access to millions of uninsured Americans, although the implementation of different components started at a later date. Political issues led to uneven adaptation by states of the different ACA components. In

Timely care after an abnormal mammogram among low-income women in a public breast cancer screening program.

BACKGROUND: Since 1990, the National Breast and Cervical Cancer Early Detection Program (BCCEDP) has funded breast cancer screening and diagnostic services for low-income, underinsured women. Case management was implemented in 2001 to address barriers to follow-up after an abnormal mammogram, and free treatment was introduced in 2004. However, the effect of these policies on timeliness of care has not been empirically evaluated. METHODS: Among 2252 BCCEDP participants in Massachusetts during 1998 through 2007, we conducted a time-to-event analysis with prepolicy-postpolicy comparisons to exam

Association between insurance status at diagnosis and overall survival in chronic myeloid leukemia: A population-based study.

BACKGROUND: Chronic myeloid leukemia (CML) can be treated effectively with tyrosine kinase inhibitor therapy directed at BCR-ABL, but access to care, medication cost, and adherence may be barriers to treatment. This study was designed to determine whether the insurance status at diagnosis influences CML patient outcomes. METHODS: The Surveillance, Epidemiology, and End Results database was used to identify 5784 patients, aged 15 years or older, who were diagnosed with CML between 2007 and 2012 and whose insurance status was documented at diagnosis. The primary outcome was 5-year overall survi

Sociodemographic Factors in Pituitary Adenomas.

Pituitary adenomas have been increasingly detected in recent years, especially in the older population. Black patients have a higher incidence than other racial groups. In patients with functioning tumors, presentation and comorbidities are influenced by age and sex, whereas the impact of ethnoracial background is unclear. Active surveillance recommendation and surgery refusal disproportionally affect Black and older patients. The likelihood of surgery at high-volume centers is lower for patients of Black or Hispanic background, uninsured or with lower socioeconomic status. Multicentric studie

Sociodemographic characteristics and cervical cancer survival in different regions of the United States: a National Cancer Database study.

OBJECTIVE: To determine how sociodemographic factors impact cervical cancer survival in different geographic locations in the USA. METHODS: A retrospective cohort of patients with cervical cancer from January 1, 2004 to December 31, 2015 in the National Cancer Database (NCDB) was identified. Tumor characteristics as well as race, income, insurance type, and treating facility types were compared among nine geographic regions. χ2 tests and Cox regression were used to compare differences between regions; p values <0.05 were considered significant. RESULTS: A total of 48 787 patients were includ

Disparities in Time to Surgeon Evaluation Among Patients with Primary Hyperparathyroidism.

BACKGROUND: A majority of patients with primary hyperparathyroidism are not referred for surgical evaluation. We hypothesized that disparities in the rate of surgeon evaluation by language, race and ethnicity, and insurance contribute to this deficit. METHODS: We queried our institutional electronic health record registry for patients with first-incident hypercalcemia between 2010 and 2018 and subsequent biochemical diagnosis of primary hyperparathyroidism. We used the Kaplan-Meier method and Cox proportional hazards modeling to investigate estimated time to surgeon evaluation by language, ra

Sociodemographic Determinants in Breast Cancer Screening among Uninsured Women of West Texas.

Background and Objectives: Early detection through appropriate screening is key to curing breast cancer. The Access to Breast Care for West Texas (ABC4WT) program offers no-cost mammography to underserved women in West Texas. The U.S. Preventative Task Force (USPSTF) guidelines are breast cancer screening guidelines which suggest screening for all women at the age of 50 years. The focus of this study was to identify sociodemographic barriers and determinants for breast cancer screenings, as well as screening outcomes, in low income, uninsured, or under-insured communities in West Texas. Postpartum readmission and uninsurance at readmission for Medicaid vs privately insured births.

BACKGROUND: Routine postpartum care is essential for managing pregnancy complications, like hypertension and diabetes, or screening for new conditions, such as depression. Insurance coverage can impact women's ability to access these postpartum services, with women who change or lose insurance postpartum having a lower likelihood of receiving the recommended postpartum care. Pregnancy-related Medicaid eligibility is only federally guaranteed for a minimum of 60 days after birth, creating the potential for publicly insured women to lose their insurance and their access to postpartum care. Provision of Temporary Access to Inpatient Hemodialysis to Uninsured Patients Initiating Hemodialysis.

IMPORTANCE: Uninsured patients who initiate hemodialysis (HD) typically cannot be placed in outpatient HD centers until their insurance applications are processed. Allowing these patients temporary access to an inpatient HD unit as outpatients could shorten their hospital length of stay (LOS). OBJECTIVE: To evaluate the association of hospital LOS with a first-in-state model in which an inpatient HD unit received regulatory program flex approval to provide temporary, transitional outpatient access to uninsured patients. DESIGN, SETTING, AND PARTICIPANTS: This quasi-experimental, pre-post qua

Full subsidy and direct delivery of essential chronic disease medications for low-income populations: A pilot randomized controlled trial.

INTRODUCTION: Care transition interventions focused on medication therapy management have not proven effective among low-income populations. This pilot study assesses a novel intervention approach that directly addresses social determinants of health (SDoH) reported as major barriers to medication adherence post-hospitalization by low-income patients. METHODS: This pilot pragmatic randomized controlled trial enrolled hospitalized adult Medicaid and uninsured patients with ambulatory care sensitive conditions using ≥2 chronic disease medications from a large safety-net hospital in the Mid-Sout

Adverse Selection in Community Based Health Insurance among Informal Workers in Bangladesh: An EQ-5D Assessment.

Community-based Health Insurance (CBHI) schemes are recommended for providing financial risk protection to low-income informal workers in Bangladesh. We assessed the problem of adverse selection in a pilot CBHI scheme in this context. In total, 1292 (646 insured and 646 uninsured) respondents were surveyed using the Bengali version of the EuroQuol-5 dimensions (EQ-5D) questionnaire for assessing their health status. The EQ-5D scores were estimated using available regional tariffs. Multiple logistic regression was applied for predicting the association between health status and CBHI scheme enro
