Trajectories of adherence to ACEI/ARB medications following a motivational interviewing intervention among Medicare Advantage beneficiaries in Texas.

OBJECTIVES: To assess the impact of student telephone motivational interviewing intervention on angiotensin-converting enzyme inhibitors/angiotensin-receptor blockers (ACEI/ARBs) adherence trajectories and identify predictors of each trajectory. METHODS: The intervention group included continuously enrolled Medicare Advantage Plan patients non-adherent to ACEI/ARBs vs the control group (1:2 ratio). The intervention was tailored by pre-intervention trajectories and included an initial and five follow-up calls. Adherence was measured 6 months after initial calls using the proportion of days cov

Healthcare utilization and expenditures associated with hyperkalemia management: a retrospective study of Medicare Advantage patients.

AIMS: This study aimed to estimate the association of patiromer exposure vs. no potassium (K+) binder (NoKb) exposure with healthcare utilization and expenditures among a cohort of Medicare Advantage patients with hyperkalemia (HK). METHODS: Using Optum's Clinformatics Data Mart (study period 2016-2019), the authors assessed propensity score-matched patients (1:1) with a serum K+ concentration ≥5.0 mmol/L and an HK diagnosis that were exposed to patiromer or NoKb on baseline characteristics. The following outcomes were compared: (1) inpatient/emergency department (ED) encounters, (2) inpatien

Zero-Dollar Copayment Impact on Adherence Scores for Centers for Medicare and Medicaid Services Star Ratings Generic Medications.

INTRODUCTION: The Centers for Medicare & Medicaid Services (CMS) Star Ratings system pushes Medicare Advantage health plans to achieve ever greater attainments in key metrics, including adherence to hydroxymethylglutaryl-CoA reductase inhibitor (statins), renin-angiotensin system (RAS) antagonist, and noninsulin antihyperglycemic (DM) medications. The purpose of this observational study was to evaluate the impact of expanding a $0 copayment (copay) benefit from mail order-only to mail order plus retail pharmacies on adherence to statin, RAS, and DM medications. METHODS: Medicare beneficiaries

Health Care Access, Preventive Care, and Affordability Among Rural Adults With Cardiometabolic Conditions: Medicare Advantage Versus Traditional Medicare.

BACKGROUND: Cardiovascular mortality has worsened among rural adults compared with urban counterparts over recent decades. Medicare Advantage (MA) enrollment is increasing in rural areas, yet its impact on access and affordability for high-risk cardiovascular populations remains unclear. This study evaluated differences in health care access, preventive care, and affordability barriers between rural adults with cardiometabolic conditions enrolled in MA versus traditional Medicare. METHODS: A cross-sectional analysis was performed using 2021 to 2022 Medicare Current Beneficiary Survey data. Mu

High-Cost Cancer Drug Use in Medicare Advantage and Traditional Medicare.

IMPORTANCE: Medicare Advantage (MA) plans are designed to incentivize the use of less expensive drugs through capitated payments, formulary control, and preauthorizations for certain drugs. These conditions may reduce spending on high-cost therapies for conditions such as cancer, a condition that is among the most expensive to treat. OBJECTIVE: To determine whether patients insured by MA plans receive less high-cost drugs than those insured by traditional Medicare (TM). DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from the linked Colorado All Payer Claims Database and Color

Cost-Associated Unmet Dental, Vision, And Hearing Needs Among Low-Income Medicare Advantage Beneficiaries.

Medicare Advantage (MA) supplemental benefits offered at no or low premiums are a key value proposition for low-income beneficiaries. Despite nearly $20 billion in rebate payments to MA plans for funding supplemental benefits, their quality or enrollee access is not monitored. Using 2018-19 Medicare Current Beneficiary Survey data linked to MA plan data, we found that regardless of plan benefit generosity, low-income beneficiaries were more likely to report dental, vision, and hearing unmet needs because of cost. Enrollment in plans with higher corresponding-year (that is, the same year as unm

Long-term Risk of Overdose or Mental Health Crisis After Opioid Dose Tapering.

IMPORTANCE: Patients prescribed long-term opioid therapy are increasingly undergoing dose tapering. Recent studies suggest that tapering is associated with short-term risks of substance misuse, overdose, and mental health crisis, although lower opioid dose could reduce risks of adverse events over the longer term. OBJECTIVE: To assess the longer-term risks of overdose or mental health crisis associated with opioid dose tapering. DESIGN, SETTING, AND PARTICIPANTS: This is a cohort study using an exposure-crossover analysis. Data were obtained from the OptumLabs Data Warehouse, which includes

Assessment of Differences in Inpatient Rehabilitation Services for Length of Stay and Health Outcomes Between US Medicare Advantage and Traditional Medicare Beneficiaries.

IMPORTANCE: Enrollment in Medicare Advantage (MA) has been increasing and has reached one-third of total Medicare enrollment. Because of data limitations, direct comparison of inpatient rehabilitation services between MA and traditional Medicare (TM) beneficiaries has been very scarce. Subgroups of elderly individuals admitted to inpatient rehabilitation facilities (IRFs) may experience different care outcomes by insurance types. OBJECTIVE: To measure the differences in length of stay and health outcomes of inpatient rehabilitation services between TM and MA beneficiaries in the US. DESIGN,

Long-Term Services and Supports in Supplemental Benefits in Medicare Advantage Plans.

IMPORTANCE: Medicare Advantage (MA) plans have had the ability to offer long-term services and supports (LTSS) supplemental benefits (eg, in-home support services, adult day health services, caregiver support, home-based palliative care, and nonopioid pain management) since 2019. However, it is largely unknown how available these benefits currently are among MA plans. OBJECTIVE: To examine the prevalence of LTSS supplemental benefits among MA plans and enrollment within these plans. DESIGN, SETTING, AND PARTICIPANTS: This longitudinal cohort study used publicly available Centers for Medicare

Pharmacist-supported electronic outreach to address medication nonadherence for Medicare Advantage enrollees.

BACKGROUND: Improved medication adherence, represented as an increase in the proportion of days covered (PDC), to chronic medications is associated with better patient outcomes, yet effective strategies to improve adherence are often resource intensive. OBJECTIVE: To quantify the impact of a pharmacist-supported electronic outreach initiative on medication adherence measures and to qualitatively evaluate patient engagement with and response to electronic messaging. METHODS: This retrospective cohort evaluation used mixed methods to assess the impact of a population health quality improvement

Baseline characteristics and secondary medication adherence among Medicare patients diagnosed with transthyretin amyloid cardiomyopathy and/or receiving tafamidis prescriptions: A retrospective analysis of a Medicare cohort.

BACKGROUND: Transthyretin amyloid cardiomyopathy (ATTR-CM) is an underdiagnosed, life-threatening condition that mostly affects older persons. In May 2019, regulatory approval of tafamidis provided the first pharmacologic treatment of ATTR-CM. In the pivotal phase 3 Transthyretin Amyloidosis Cardiomyopathy Clinical Trial (ATTR-ACT), 97.2% of patients were classified as adherent (defined as taking ≥ 80% of scheduled doses). Given its recent approval, there is limited real-world evidence examining patient adherence to tafamidis. OBJECTIVE: To evaluate adherence patterns, demographics, and clinic

A Motivational Interviewing Intervention to Improve Adherence to ACEIs/ARBs among Nonadherent Older Adults with Comorbid Hypertension and Diabetes.

BACKGROUND: Hypertension and diabetes mellitus are independent risk factors for cardiovascular diseases. Due to the cardioprotective nature of angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin II receptor blockers (ARBs), they are recommended for patients with comorbid hypertension and diabetes. However, poor adherence to ACEIs/ARBs among older adults is a major public health concern. This study aimed to assess the effectiveness of a telephonic motivational interviewing (MI) intervention conducted by pharmacy students among a nonadherent older population (≥ 65 years old) with di

Risk Adjustment for Alzheimer Disease and Related Dementias in Medicare Advantage and Health Care Experiences.

IMPORTANCE: Failure to account for the full complexity and costs of high-need populations in the risk-adjusted capitated payment model for Medicare Advantage (MA) plans may create financial disincentives for plans to invest in comprehensive care for affected beneficiaries, potentially exacerbating health disparities. OBJECTIVE: To evaluate the association of reinstatement of Alzheimer disease and related dementias (ADRD) hierarchical condition categories (HCCs) into the MA risk-adjusted payment model in 2020 with access, affordability, and quality of care for beneficiaries with ADRD. DESIGN,

Prevalence of Chronic Medical Conditions Among Medicare Advantage and Traditional Medicare Beneficiaries.

BACKGROUND: The federal government spends billions of dollars per year on payments to Medicare Advantage (MA) plans based, in part, on beneficiaries' risk scores. Despite this, little is known about the true burden of chronic medical conditions among MA beneficiaries compared with those in fee-for-service (FFS) Medicare. OBJECTIVE: To determine whether the prevalence of chronic medical conditions is higher among MA compared with FFS beneficiaries. DESIGN: Cross-sectional. SETTING: Population based. PARTICIPANTS: Adults aged 65 years or older enrolled in MA or FFS Medicare. MEASUREMENTS: U

Medicare in interventional pain management: A critical analysis.

Recent years have been quite eventful for interventional pain physicians with numerous changes in the Medicare payment system with a view for the future and what it holds for interventional pain management for 2006 and beyond. On February 8, 2006, President Bush signed the Deficit Reduction Act of 2005, which cuts the federal budget by 39 billion dollars and Medicare and Medicaid by almost 11 billion dollars over five years. The Act contains a number of important provisions that effect physicians in general and interventional pain physicians in particular. This Act provides one year, 0% conver

Telehealth among Medicare Advantage beneficiaries with and without Alzheimer's Disease and Related Dementias (ADRD), 2018-2024: from pandemic-era surge to post-pandemic decline.

BACKGROUND: Telehealth has become a vital element of healthcare delivery, especially among Medicare Advantage (MA) beneficiaries. Yet its use among individuals with Alzheimer’s Disease and Related Dementias (ADRD) – a population with complex biopsychosocial needs – remains understudied. METHODS: We conducted an observational cohort study of MA beneficiaries aged 65 and older using Optum’s de-identified Clinformatics® Data Mart Database (January 2017–June 2024). A matched sample of enrollees with and without ADRD was constructed. Difference-in-differences and event study analyses were applied t
