Efficiency of computerized clinical decision support systems involving anticoagulants: A flashmob study in Dutch hospital pharmacies.

AIMS: Computerized decision support systems (CDSSs) aim to prevent adverse drug events. However, these systems generate an overload of alerts that are not always clinically relevant. Anticoagulants are frequently involved in these alerts. The aim of this study was to investigate the efficiency of CDSS alerts on anticoagulants in Dutch hospital pharmacies. METHODS: A multicentre, single-day, cross-sectional study was conducted using a flashmob design in Dutch hospital pharmacies, which have CDSSs that operate on both a national medication surveillance database and on self-developed clinical ru

High-Risk Medicines and Technology-Related Prescribing Errors in 2 Pediatric Hospitals.

OBJECTIVE: High-risk medicine (HRM) prescribing errors in pediatrics are a safety concern. Technology-related errors (TREs) occur when the design of computerized provider order entry (CPOE) facilitates errors. We aimed to measure the proportion of HRM errors that were TREs and the HRM TRE rate at 2 pediatric hospitals, describe TRE types, and examine their underlying mechanisms. METHODS: We identified 7014 HRM orders from retrospective medical record review of 18 951 medication orders for 2192 pediatric inpatients at 2 hospitals with the same CPOE in 2020. HRM prescribing errors were classifi

A multicentric intervention based on nudging for the reduction of potentially unnecessary laboratory tests: insights from a Swiss hospital network.

BACKGROUND: Healthcare overuse is a widespread issue, accounting for approximately 10–30% of total healthcare costs. This study examines the impact of a computerized alert system designed to reduce inappropriate laboratory tests across five public hospitals in southern Switzerland. METHODS: A 32-month observational study - between July 2021 and March 2024 - was conducted to assess the effectiveness of the alert system in discouraging unnecessary laboratory testing. Data were collected from five public hospitals where 12.4% of laboratory tests performed were identified as non-compliant with rec

Factors predictive of intravenous fluid administration errors in Australian surgical care wards.

BACKGROUND: Intravenous (IV) fluid administration is an integral component of clinical care. Errors in administration can cause detrimental patient outcomes and increase healthcare costs, although little is known about medication administration errors associated with continuous IV infusions. OBJECTIVES: (1) To ascertain the prevalence of medication administration errors for continuous IV infusions and identify the variables that caused them. (2) To quantify the probability of errors by fitting a logistic regression model to the data. METHODS: A prospective study was conducted on three surgic

A computerized assisted pharmacy automatic stop order procedure.

Automatic stop orders, which still seem to plague many hospitals, can be instituted with the cooperation of the medical staff, legal counsel, nursing service, and administration. The use of a computerized assisted pharmacy system, described here, allows compliance with Joint Commission on Accreditation of Hospitals' regulations and also prevents possible legal complications. By utilizing computer-generated reports, appropriate labels, and a drug order entry system for entering a discontinue/renewal time and date, compliance with established policies is possible.

Evaluation of stat orders in a teaching hospital: a chart review.

BACKGROUND: Physicians frequently prescribe 'stat' orders that need to be actioned immediately or within a limited time frame. This process can be time consuming and expensive. Stat medications are reserved for the highest priority orders and life-threatening situations that need to be dealt with immediately or within a limited time frame. OBJECTIVES: The goals of this study were to evaluate whether stat medications in a teaching hospital were ordered appropriately and to assess the rationale for the stat order. METHODS: The study was carried out between July and August 2009 in Masih Daneshv

Development of a portable information system: connecting palmtop computers with medical records systems and clinical reference resources.

The portability of palmtop computers makes them an ideal platform to maintain communication between busy physicians and medical information systems. In our academic FHC (Family Health Center) we have developed software that runs on a palmtop computer allowing access to information in the HIS (Hospital Information System) and our FHC's AAMRS (Automated Ambulatory Medical Record System). The resident physicians who staff the hospital and the FHC are frequently at home or otherwise off-site where terminal access is not available. Using a Hewlett-Packard 95LX palmtop computer as the base platform,

SOA approach for integration of departmental systems.

In this paper a unified method for integration of departmental systems into the main systems of a healthcare organization is described. The approach is based on combining Business Process Modeling (BPM) and Service Oriented Architecture (SOA) methods and technologies. A top-down approach is used for modeling the care process and supporting care services which in turn are decomposed down to such a level of granularity that they can be described and implemented as web services described with Web Services Description Language (WSDL) documents. Then a bottom-up approach is used for wrapping the ex

A database perspective of the transition from single-use (ancillary-based) systems to integrated models supporting clinical care and research in a MUMPS-based system.

The Composite Health Care System (CHCS), a MUMPS-based hospital information system (HIS), has evolved from the Decentralized Hospital Computer Program (DHCP) installed within VA Hospitals. The authors explore the evolution of an ancillary-based system toward an integrated model with a look at its current state and possible future. The history and relationships between orders of different types tie specific patient-related data into a logical and temporal model. Diagrams demonstrate how the database structure has evolved to support clinical needs for integration. It is suggested that a fully in

The Importance of Electronic Health Record Access For Accurately Completing Drug Regimen Reviews.

OBJECTIVE: To challenge the standard of practice by evaluating the identification of medication discrepancies found depending on type of access to an electronic health record (EHR). In other words, is there a difference in the number of discrepancies between a pharmacist with only access to the postacute long-term care (PALTC) EHR (ie, single-access pharmacist [SAP]) compared with a pharmacist with access to both the PALTC and hospital EHRs (ie, dual-access pharmacist DAP) In October 2018, the Improving Medicare PostAcute Care Transformation (IMPACT) Act mandated admission drug review (DRR) up

Optimising pharmacy processes in medication delivery service through digitalisation and automation.

The rapid expansion of medication delivery service (MDS) during COVID-19 created many new roles in the pharmacy which are manpower intensive and not sustainable as pharmacy resumes counter collection services. To keep MDS operations sustainable, we identified the need to streamline and automate processes which are manual and repetitive. We seek to determine if the Plan, Do, Study, Act (PDSA) cycles would effectively reduce the man-hours required for the MDS over two phases in 1 year. Phase 1 involved digitalisation of order taking and automation of the data entry process. Phase 2 involved auto

[Identity vigilance and implementation of transfusion software in hospitals].

In hospitals, patients' identity mismatch is the cause of serious incidents during transfusion or other healthcare processes. With a wider use of healthcare software, new dysfunctions and risks may occur in case of ID mismatch between different software and for transfusion between blood bank and hospital. Four critical stages are to be considered when using software management of hospital care: initial patients' identification, modifications of identification data during hospital stay and then synchronization of all different software, proper use of patient's identification means by hospital s

Automated dispensing cabinets and nurse-related medication errors in inpatient settings: A systematic review.

BACKGROUND: Automated Dispensing Cabinets (ADCs) are computerized medication storage and dispensing systems increasingly used in inpatient hospital settings to support safe medication administration. As nurses represent the final point of medication delivery to patients, understanding the impact of ADCs on nurse-related medication errors is critical. This systematic review aimed to assess the association between ADC implementation and medication errors involving nurses in inpatient hospital settings. METHODS: A systematic search of PubMed, MEDLINE, Scopus, and Google Scholar identified 16 eli
