Use of magnetic resonance imaging identify suspensory desmitis and adhesions between exostoses of the second metacarpal bone and the suspensory ligament in four horses.

Four horses were examined because of chronic forelimb lameness. In all horses, the cause of the lameness was localized to the metacarpus by means of physical examination and diagnostic anesthesia, and radiography of the affected limb revealed a small exostosis of the second metacarpal bone. Magnetic resonance imaging revealed suspensory desmitis in the region of this exostosis in all 4 horses. In addition, an abnormal area of low signal intensity, suggestive of an adhesion, was seen between the exostosis and the suspensory ligament. In all horses, an adhesion between the suspensory ligament an…

Thumb duplication.

Thumb duplication is a complex congenital anomaly. Treatment requires a detailed understanding of the pathophysiology as well as an appreciation of all the components involved. The goals of surgery are to restore the normal anatomic relationships of the residual thumb. Successful reconstruction requires evaluation and correction of the bony axis; articular congruity; size and angulation; intrinsic and extrinsic musculotendinous abnormalities; and ligament, skin, and soft-tissue elements. The reconstructed thumb may not match the normal thumb because of the hypoplasia that is inherently associa

Avascular necrosis of the metacarpal head: Dieterich's disease.

A 68-year-old woman presented with an osteolytic image on the metacarpal head of the ring finger, first attributed to inflammatory or septic arthritis. She had experienced increasing pain three months before presentation, requiring orthopaedic treatment. At the time of clinical assessment, there was some limitation in the range of movement, accompanied by local pain and chronic swelling. Inflammatory arthritis was not confirmed on laboratory testing or scintigraphy. A diagnosis of Dieterich's disease was established based on the evolution of the condition, with chronic limitation of movement a

Osteonecrosis of the base of the second metacarpal: a case report.

Osteonecrosis of the metacarpal is rare. Previously reported cases have only involved the metacarpal heads, with the long finger being the most commonly affected. We present a patient with avascular necrosis involving the base of the second metacarpal.

Treatment of finger dysplasia with metacarpal base defect.

The hypoplastic finger with metacarpal base defect is a rare anomaly. Metacarpal lengthening is not a reasonable treatment option because of severe hypoplasia of the metacarpal. Complete amputation with closure of the interdigital space is effective but most parents want to save any digit of their child. We prefer to release the syndactyly and save the finger, with its shortened three phalanges, during childhood. Our parents and patients have been satisfied with the short digit. At a later age the metacarpal of the hypoplastic finger can be fused to one or the other adjacent metacarpals. Alter

Metacarpophalangeal Joint Denervation in the Treatment of Dieterich Disease: A Case Report.

Avascular necrosis of the metacarpal head, known as Dieterich disease, is rare. The underlying pathogenesis of the disease is not clearly understood, and there are few cases reported in the literature. Nonoperative treatment with rest and nonsteroidal anti-inflammatory drugs is often successful, but surgical management is sometimes indicated. The case outlined here describes a novel application of the known technique of metacarpophalangeal joint denervation to relieve pain while maintaining joint mechanics and grip strength.

Cleft hands with six metacarpals.

PURPOSE: The purpose of this study was to identify children with cleft hands whose radiographs showed metacarpal polydactyly with 6 metacarpals to support the hypothesis that cleft hand develops not as a result of a longitudinal failure of formation but through a process of central polydactyly and osseous syndactyly. METHODS: We screened the hand radiographs of all children with a transverse or longitudinal deficiency of the upper limb and identified 8 children with cleft hands containing 6 metacarpals. RESULTS: Six cleft hands had a missing middle finger and consisted of a thumb and index f

Relative Echogenicity of Tendons and Ligaments of the Palmar Metacarpal Region in Foals from Birth to 4 Months of Age: A Longitudinal Study.

The objective of this study was to evaluate relative echogenicity of superficial and deep digital flexor tendons, the accessory ligament of the deep digital flexor tendon and interosseous muscle of the metacarpal region in foals ages 1 week to 4 months; and assess the association between echogenicity and sex or side/laterality. Seven Standardbred trotter foals were examined. Right and left metacarpal regions (palmar surface) were ultrasonographically investigated, and four regions of interest were assessed. A significant increase in echogenicity was seen in superficial and deep digital flexor

[The "candlestick" technique for the correction of certain types of congenital metacarpal synotosis].

Metacarpal synostosis is a rare congenital hand malformation requiring only occasionally a surgical correction. However in case of divergent epiphyses there is a progressive accentuation of the deformity. In the "Y" type of symmetrical synostosis, the authors propose a trapezoidal osteotomy with upside down relocation allowing realignement of the epiphyses without distant bone donor site.

One-stage elongation of the third or fourth brachymetacarpia through the palmar approach.

The technique of 1-stage elongation of a short third or fourth metacarpal through the palmar approach was devised to avoid the drawbacks of the conventional procedures through the dorsal approach with or without using an external fixator. This technique was used in the treatment of 3 cases and the results and the technique are described.
