BACKGROUND: Human metapneumovirus (HMPV) is a leading cause of acute respiratory illness (ARI) in children. Population-based incidence rates and comprehensive clinical characterizations of disease have not been established.
METHODS: We conducted population-based prospective surveillance for 2 years in 2 US counties of HMPV infection among children <5 years old who were hospitalized with ARI or fever. Nasal and throat specimens obtained with swabs were tested for HMPV by real-time reverse-transcription polymerase chain reaction and genotyped.
RESULTS: Forty-two (3.8%) of 1104 children tested …

BACKGROUND: Human metapneumovirus (hMPV) is an established pathogen of the respiratory tract of children and adults. hMPV is related to other paramyxoviruses known to cause encephalitis. Reports suggest that hMPV may cause disease of the central nervous system (CNS).
METHODS: Two groups of patients were studied. The first group consisted of children between birth and 18 years from whom nasal scrapings were obtained between January 2004 and October 2005. hMPV RNA amplification by PCR was done and records were reviewed for clinical and demographic data. The second group consisted of patients wi…

Human metapneumovirus (hMPV) is a major respiratory pathogen for which no specific antiviral therapy is approved. In this study, we evaluated the anti-hMPV activity of the oral nucleoside analog VV116. In vitro, VV116 strongly inhibited hMPV replication in LLC-MK2 cells, with a half-maximal effective concentration (EC50) of 0.79 μM and a high selectivity index (SI) > 159. In hMPV-infected mice, oral administration of VV116 significantly reduced viral loads in lung and nasal turbinate tissues in a dose-dependent manner and ameliorated virus-induced lung pathology. Our findings provide the first…

Human metapneumovirus (hMPV) is a globally prevalent respiratory pathogen, primarily affecting young children, the elderly, and immunocompromised individuals. Typical airway-centric radiological findings like other viral infections are commonly reported. However, atypical findings in computed tomography (CT) have yet to be reported in literature. We present a case of hMPV infection in an immunocompetent adult with rare findings of cystic lesions in CT thorax. A 67-year-old female with underlying valvular heart disease and chronic heart failure presented to our centre with respiratory symptoms …

Human metapneumovirus (hMPV) is a significant global pathogen that causes acute respiratory tract infections, especially in infants, young children, the elderly, and immunocompromised individuals. Despite its increasing prevalence, there are currently no vaccines or effective treatments available for hMPV. The pathogenesis of hMPV infection is a complex process involving a multitude of host factors and viral receptors. These interactions determine the virus ability to enter host cells, replicate, and evade the immune response. This review is the first to provide a comprehensive overview of the…

Human Metapneumovirus (HMPV) remains one of the most common viral infections causing acute respiratory tract infections, especially in young children, elderly, and immunocompromised populations. Clinical symptoms can range from mild respiratory symptoms to severe bronchiolitis and pneumonia. The production of mucus is a common feature during HMPV infection, but its contribution to HMPV-induced pathogenesis and immune response is largely unknown. Mucins are a major component of mucus and they could have an impact on how the host responds to infections. Using an in vitro system and a mouse model…

BACKGROUND: Human metapneumovirus has recently been recognized as an important cause of severe respiratory viral infections and of viral infections in patients admitted to intensive care units. Little is known about the course of this infection in pregnancy.
CASE: A late-preterm primigravid woman was admitted to the intensive care unit for acute respiratory distress syndrome and subsequently diagnosed with human metapneumovirus. Because of worsening maternal respiratory status, she was intubated and a primary cesarean delivery was performed. The patient's respiratory status continued to decli…

Introduction. Human metapneumovirus (HMPV) is a paramyxovirus from the same subfamily as respiratory syncytial virus (RSV) and causes similar acute lower respiratory tract infection. Albuterol in the setting of acute RSV infection is controversial and has not yet been studied in HMPV. We sought to determine the frequency of albuterol use in HMPV infection and the association between albuterol administration and patient outcomes. Methods. We conducted a retrospective cohort study identifying all patients hospitalized in a tertiary care children's hospital with laboratory-confirmed HMPV infectio…

Human metapneumovirus (HMPV) is a major cause of respiratory infections, particularly among vulnerable populations, yet effective therapeutics remain unavailable. Monoclonal antibodies (mAbs) offer a promising approach for treatment and prevention. We describe the discovery and characterization of 4F11, a highly potent neutralizing mAb with in vitro and in vivo efficacy against HMPV. Using cryo-electron microscopy, we define a unique mechanism of binding employed by 4F11. 4F11 targets an epitope located at the apex of the prefusion F protein (site Ø) with a 1:1 stoichiometry of Fab to trimer, …

INTRODUCTION: Human metapneumovirus (HMPV) remains a leading cause of severe paediatric and geriatric respiratory morbidity, manifesting in pathologies from bronchiolitis to pneumonia. Despite its clinical burden, no licensed vaccines or targeted antivirals exist. This study utilizes an advanced computational pipeline to identify novel therapeutic leads targeting two high-consequence viral proteins: the Fusion (F) protein, essential for viral-cell entry, and the M2-1 matrix protein, a critical processivity factor for viral replication.

Human metapneumovirus (hMPV) represents a leading cause of both upper and lower respiratory tract infections among children and adults globally. To investigate the prevalence and evolution of hMPV in the Jiaxing area of China between 2023 and 2025, we screened 3600 pharyngeal swab specimens by real-time PCR, identified 101 positives, and obtained genomic sequences of 48 viral isolates using high-throughput sequencing. Using the sequencing data, we reconstructed a phylogenetic tree and examined amino acid substitutions. The epidemiological analysis revealed an overall hMPV positivity rate of 2.…

Human metapneumovirus (HMPV) is a well-identified paramyxovirus that has emerged as a significant global health threat, particularly following recent outbreaks in 2024-2025. It preferentially infects the respiratory epithelium and affects infants, the elderly, and immunocompromised populations. The clinical manifestations of the HMPV range from mild upper respiratory symptoms to severe diffuse bronchopneumonia. As of late 2024 and early 2025, HMPV has been responsible for 6.2% of positive respiratory illness tests and 5.4% of respiratory-associated hospitalizations in China, surpassing COVID-1…
