Lesser metatarsalgia in the geriatric patient.

This segment provides a classic definition of the clinical condition as it presents in the older patient. This article discusses causative considerations as they relate to aging and management and related aspects involved in the care of the older patient.

Metatarsal osteotomy for relief of metatarsalgia.

If surgery is indicated for metatarsalgia, osteotomy of the metatarsals should be considered. In the years 1967-1977 we have operated on 35 feet. The first 17 we performed a proximal V-shaped osteotomy, the next 18 we performed a distal oblique osteotomy. The results were excellent in 18 cases, good in 13 cases and poor in 4 cases. We recommend the oblique distal osteotomy because the operation is simple, recovery is rapid and symptoms are well relieved.

Metatarsalgia: Assessment Algorithm and Decision Making.

Historically, metatarsalgia was approached as a forefoot condition, most often associated with hallux valgus. Consequently, surgical treatments were limited to that anatomic zone, disregarding more proximal structures. In order to assess this entity properly, it is necessary to consider anatomic and biomechanical factors, as well as general and local conditions of the affected patients. A thorough understanding of the multiple potential causal factors is essential to ensure selection of the optimal treatment.

Resolution of metatarsalgia following oblique osteotomy.

Metatarsalgia of the central ray is a major surgical challenge. Without precise correction, transfer lesions may occur at an adjacent metatarsal or patients may have inadequate pain relief. Current surgical treatment strategies do not facilitate precise positioning in different planes, resulting in disappointing outcomes. To achieve better outcomes we used an oblique sliding osteotomy to facilitate precise correction. We hypothesized the procedure would reduce pain in patients with prominent second and/or third metatarsal heads, with few complications. We retrospectively reviewed 32 consecutiv…

Is primary metatarsalgia associated with a longer 2nd metatarsal? A comparison of metatarsal lengths in individuals with and without metatarsalgia.

BACKGROUND: Metatarsalgia is a generic term used to describe localised forefoot pain around the metatarsals. Primary metatarsalgia is thought to be caused by excessive loading over one or multiple metatarsal heads resulting in localised inflammation and painful callosities. An ideal foot morphotype was described by Maestro et al., which is currently used to plan for corrective osteotomies. The aim of this study is to investigate whether there is a true difference in metatarsal lengths between symptomatic and asymptomatic individuals. METHODS: A retrospective single centre study was carried ou…

Percutaneous Surgery for Metatarsalgia and the Lesser Toes.

The traditional open surgical options for the treatment of metatarsalgia and lesser toe deformities are limited and often result in unintentional stiffness. The use of percutaneous techniques for the treatment of metatarsalgia and lesser toe deformities allows a more versatile and tailor-made approach to the individual deformities. As with all percutaneous techniques, it is vital the surgeon engage in cadaveric training from surgeons experienced in these techniques before introducing them into his/her clinical practice.

The outcome of Morton's neurectomy in the treatment of metatarsalgia.

Morton's neuroma is a common cause of metatarsalgia caused by intermetarsal digital nerve thickening. This study reviews the pathology, presentation, symptoms and signs, and patient satisfaction with surgical treatment. Seventy-eight patients (82 feet) were treated for Morton's metatarsalgia by excision of the interdigital nerve. The patients were followed-up for a mean of 4.6 years (range 0.8-8.1 years) and scored using the Foot Functional Index and the American Orthopedic Foot Ankle Society scoring system. In 74 patients the Foot Functional Index was more than 85 (maximum score 100). Seventy…

[Weil's metatarsal osteotomy in the treatment of metatarsalgia].

The Weil distal shortening osteotomy of the lesser metatarsals has been used by the author since June 1992, particularly in the treatment of metatarsalgia. Its advantages are a direct approach to the metatarsophalangeal (MP) joint and the metatarsal head, providing not only correction of the deformity, but accurate control of the metatarsal shortening, primary healing and early functional recovery thanks to the strong fixation allowed by the twist-off screw. Its disadvantage is the limitation of MP articular range motion, which is temporary in most cases. The role of the Weil osteotomy in meta…

Metatarsalgia: a clinical review of diagnosis and management.

Metatarsalgia is the symptom of pain in the region of the metatarsophalangeal articulations. In a careful analysis of 98 patients who presented with complaints of forefoot pain, 23 distinct diagnoses were made as to the etiology of their pain. These diagnoses were grouped under primary metatarsalgia, secondary metatarsalgia, and pain under the forefoot. Specialized tests, such as stance phase cholesterol-crystal force plate analysis and intraarticular or digital nerve block, are helpful in determining the diagnosis and the likelihood of effective treatment.

The anterior heel for metatarsalgia in the adult foot.

A conservative approach to foot problems is especially useful in patients with diabetes, rheumatoid arthritis, diminished circulatory change … corns, or hammertoe deformities. When the anterior heel is prescribed many foot problems other than metatarsalgia will stop being symptomatic and surgery treatment can be bypassed. A physician must know about the supply of shoes in the community and, if necessary, instigate a better inventory of available shoes. In addition, it is necessary to establish good ra …
