Staphylococcus aureus bacteremia and endocarditis are serious infections that demand prompt clinical attention to ensure good outcomes. Of foremost importance is identifying and managing the source of infection and any associated complications. Evaluation for the presence of cardiac involvement is essential because inadequately managed S. aureus endocarditis is life threatening. Thus, physicians must aggressively negotiate treatment paths, considering whether the S. aureus bacteremia is complicated, whether foreign sources of infection should be removed or replaced, and whether surgical interv

A large and growing proportion of Staphylococcus aureus clinical isolates are methicillin resistant, and are resistant to practically all beta-lactam antibiotics. Methicillin-resistant S. aureus (MRSA) strains harbor mecA, which is carried by a unique mobile genetic element, staphylococcal cassette chromosome mec (SCCmec) integrated into the S. aureus chromosome. The mecA gene encodes a methicillin-insensitive transpeptidase, the production of which confers resistance to otherwise inhibitory concentrations of beta-lactam antibiotics. Several distinct clones have been identified among MRSA that

Three patients with corneal ulcers had gram-positive cocci isolated on initial scrapings. There was no response to topical fortified aminoglycosides or cephalosporins. Cultures revealed methicillin-resistant Staphylococcus aureus that was sensitive only to topical vancomycin eye drops. Because the microbiologic spectrum is similar, vancomycin should be considered instead of a cephalosporin in the treatment of corneal ulcers to cover methicillin-resistant S. aureus and S. epidermidis.

The purpose of this study is to report a case of methicillin-resistant Staphylococcus aureus (MRSA) keratitis in a dog. A 7-year-old intact male American cocker spaniel that had undergone removal of a nictitating gland was referred for severe ulcerative keratitis. Slit-lamp examination showed swelling of the eyelid, mucopurulent discharge, conjunctival injection and chemosis, diffuse corneal edema and opacity, and a deep ulcer in central cornea. Gram staining of discharge from the eye demonstrated Gram-positive cocci. Despite topical ofloxacin, oxytetracycline and polymyxin B ophthalmic soluti

Community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) has emerged over the last decade across the United States and the world, becoming a major pathogen in many types of community-acquired infections. Although most commonly associated with minor skin and soft tissue infections, such as furuncles, CA-MRSA also can cause necrotizing fasciitis, pyomyositis, osteoarticular infections, and community-acquired pneumonia. This article discusses the epidemiology, diagnosis, and management of these infections from the perspective of the emergency physician.

Staphylococcus aureus is a potentially pathogenic bacterium that causes a broad spectrum of diseases. S. aureus can adapt rapidly to the selective pressure of antibiotics, and this has resulted in the emergence and spread of methicillin-resistant S. aureus (MRSA). Resistance to methicillin and other beta-lactam antibiotics is caused by the mecA gene, which is situated on a mobile genetic element, the Staphylococcal Cassette Chromosome mec (SCCmec). To date, five SCCmec types (I-V) have been distinguished, and several variants of these SCCmec types have been described. All SCCmec elements carry

STUDY DESIGN: Retrospective case series.
OBJECTIVE: To determine relevant demographics, clinical presentations, and outcomes of this condition.
SUMMARY OF BACKGROUND DATA: This is the first study looking specifically at methicillin-resistant Staphylococcus Aureus (MRSA) spondylodiscitis.
METHODS: We performed a retrospective review of patients presenting between 2000 and 2005.
RESULTS: Thirteen cases were identified. The mean age was 65 years; 85% were male. All cases presented with back pain, spinal tenderness, and systemic upset. Neurologic deficit was present initially in 39%, and 8% de

Nosocomial infections due to methicillin-resistant Staphylococcus aureus (MRSA) is an important health problem worldwide. This microorganism causes a variety of clinical infections, including osteomyelitis, invasive endocarditis, septic arthritis and septicemia. Antimicrobial resistance is a factor that influences the persistence of MRSA in the hospital environment. The introduction of molecular typing techniques in epidemiological investigations has provided new tools for identifying the microorganism's origin and routes of dissemination. One of the most important conclusions that have result

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is a potentially lethal organism in pediatric patients. MRSA is an uncommon otologic pathogen that requires special diagnostic and therapeutic intervention.
METHODS: Three pediatric patients with community-acquired MRSA otologic infections were identified during 1999.
SETTING: Tertiary care ear institution.
RESULTS: All patients required intravenous antibiotic therapy to achieve resolution of the infections.
CONCLUSIONS: MRSA in children can be community acquired and can cause otitis externa, otitis media with otorrhea, or acut

BACKGROUND: Chorioamnionitis is a leading cause of morbidity and mortality in preterm infants. Only rarely is Staphylococcus aureus implicated. A case of methicillin resistant Staphylococcus aureus causing chorioamnionitis and endometritis is presented.
CASE REPORT: A 39-year-old gravida 2 para 1 female, who previously worked as the unit clerk in the pediatric pulmonary unit of a children's hospital, was initially admitted at 22 weeks with a shortened cervix. The patient refused emergency cerclage. She was released from the hospital and returned at 25 4/7 weeks' estimated gestational age with

The investigation was performed on 923 strains of S. aureus isolated from clinical material obtained from several regions of Poland. Resistance to methicillin was tested by a dilution method on a solid Mueller-Hinton medium supplemented with 2% of NaCl. Strains exhibiting MIC higher than 4 micrograms/ml were determined as resistant. Resistance to other antibiotics (P, Am, CB, CF, CH, MA, Ge, Bs, E, L, Dx) was tested by a disc method. Bacteriophage typing of S. aureus was performed by a method described by Blair and Williams in RTD and RTD x 100, using basic set of phages and additional phages

We reviewed the records of 20 patients with bacteremia caused by methicillin-resistant Staphylococcus aureus (MRSA) and 49 patients with methicillin-susceptible S. aureus (MSSA) bacteremia occurring during a 39-month period. Sixteen of the MRSA bacteremias were clinically significant, as were 42 of the MSSA bacteremias. MRSA bacteremia was almost always nosocomial and occurred mainly in very young patients who were often prematurely born and severely compromised by underlying diseases. In contrast patients with MSSA bacteremia were frequently older children who had no underlying disease and ac

The aim of the study was to investigate methicillin-resistant Staphylococcus aureus (MRSA) and methicillin-resistant Staphylococcus pseudintermedius (MRSP) among employees of a small animal hospital and the hospital environment. In total, 96 swabs from employees and 73 swabs from the clinic environment were investigated. Cation-adjusted-Mueller-Hinton broth (CAMHB) + 6.5% NaCl was used for enrichment before plating on Mueller-Hinton (MH) agar with 2% NaCl and 0.25 mg/L oxacillin. The staphylococcal species was determined using MALDI-TOF MS. The isolates were subjected to mecA-PCR, macrorestric
