BACKGROUND: Vitamin B₁₂ deficiency is common among the elderly, and early detection is clinically important. However, clinical signs and symptoms have limited diagnostic accuracy and there is no accepted reference test method.
METHODS: In elderly subjects (n = 700; age range 63-97 years), we investigated the ability of serum cobalamin, holotranscobalamin (holoTC), total homocysteine (tHcy), methylmalonic acid (MMA), serum and erythrocyte folate, and other hematologic variables to discriminate cobalamin deficiency, defined as red blood cell cobalamin <33 pmol/L.
RESULTS: Serum holoTC was the

Mitochondrial dysfunction is increasingly recognized as a driver of sarcopenia pathogenesis, progression, and prognosis. Muscle mass is a fundamental and objective component of sarcopenia. In some studies, relative muscle loss has been used to define sarcopenia. Methylmalonic acid (MMA) is a biomarker of mitochondrial dysfunction and vitamin B12 deficiency. Evidence has shown a negative link between MMA and muscle function, yet population-level evidence on its predictive and prognostic value in low muscle mass and sarcopenia remains scarce. This cohort study analyzes 10,414 U.S. adults from th

BACKGROUND AND OBJECTIVE: Methylmalonic aciduria (MMA) and propionic aciduria (PA) are organic acidurias characterised by the accumulation of toxic metabolites and hyperammonaemia related to secondary N-acetylglutamate deficiency. Carglumic acid, a synthetic analogue of N-acetylglutamate, decreases ammonia levels by restoring the functioning of the urea cycle. However, there are limited data available on the long-term safety and effectiveness of carglumic acid. Here, we present an interim analysis of the ongoing, long-term, prospective, observational PROTECT study (NCT04176523), which is inves

Vitamin B12 is essential for cell function and only accessible in food for mammals. To monitor vitamin B12 deficiency, methylmalonic acid (MMA) is used. Since MMA in serum/plasma is a frequently requested analyte at clinical laboratories the analytical method was improved and validated on a 96 well plate. Using a Tecan robot a working solution of acetonitrile containing MMA-D3 was added to plasma/serum samples. The solution was shaken for 1 min and then centrifuged for 10min. The supernatant was transferred to another plate and evaporated with nitrogen gas. The residual was redissolved with 0.

BackgroundVitamin B12 deficiency can cause anaemia, fatigue, and neurological sequelae. Total B12 assays may not be reflective of tissue B12. Methylmalonic acid (MMA) can assess tissue B12 more accurately. NICE published clinical guidelines on Vitamin B12 deficiency in adults in 2024. They outlined MMA testing in two scenarios: in patients where nitrous oxide use is possible or who have an indeterminate total B12 result (180-350 ng/L) alongside symptoms or signs of B12 deficiency. MMA testing is costly, not readily available, and time consuming, so increased use will have implications.MethodsT

Objectives: For diagnosis of vitamin B12 deficiency, plasma methylmalonic acid (P-MMA) is considered superior to plasma vitamin B12 (P-B12). Reduced kidney function elevates P-MMA, hence, hampering P-MMA as a biomarker. We assessed whether correcting P-MMA for estimated glomerular filtration rate (eGFR) can affect the estimated prevalence of B12 deficiency. Methods: We included 115,245 patients with concomitant measurements of P-MMA, P-B12 and P-Creatinine. B12 deficiency was classified using P-MMA decision limits at >0.75 and >0.43 µmol/L. The non-linear relation between eGFR and P-MMA was es

BACKGROUND: Measurement of vitamin B12 (B12) levels is routinely used to monitor B12 sufficiency. However, its accuracy may not be reliable in patients with short-bowel syndrome (SBS). The presence of elevated methylmalonic acid (MMA) levels can also be used as a marker for B12 deficiency. Therefore, our aim was to analyze serum B12 and MMA levels simultaneously in a cohort of children with SBS to evaluate the accuracy of these markers in the assessment of B12 status.
METHODS: Children ages 1-18 with intestinal failure were eligible for enrollment. MMA and B12 levels were checked simultaneous

Four biomarkers are commonly employed to diagnose B12 deficiency: vitamin B12 (B12), holotranscobalamin (HoloTC), methylmalonic acid (MMA), and homocysteine (Hcy). 4cB12, a combined index of the B12 status, has been suggested to improve the recognition of B12 deficiency. We aimed to evaluate the four different markers for detecting B12 deficiency, as determined by 4cB12. Within a large, mixed patient population, 11,833 samples had concurrent measurements of B12, HoloTC, MMA, and Hcy. 4cB12 was calculated according to the methods described by Fedosov. BACKGROUND: Oxidative stress and mitochondrial dysfunction play an important role in diabetic retinopathy (DR), and serum methylmalonic acid (MMA) levels are related to oxidative stress and mitochondrial dysfunction. We aimed to explore the impact of MMA levels and the composite dietary antioxidant index (CDAI) on DR risk in patients with diabetes.
METHODS: Data on patients with diabetes from the 1999-2004 and 2011-2014 National Health and Nutrition Examination Survey (NHANES) datasets were included in this cross-sectional study. Logistic regression analysis (accounting for NHANES survey weig

Vitamin B12 (cobalamin) and folate deficiencies are preventable causes of neurological and hematological disorders, particularly in vulnerable populations. Functional biomarkers such as total homocysteine (tHcy) and methylmalonic acid (MMA) are more sensitive indicators of metabolic impairment than circulating vitamin B12 concentration. However, their simultaneous quantification remains analytically challenging due to chemical diversity and the presence of structurally related endogenous compounds. Here, we present a robust, high-throughput liquid chromatography-tandem mass spectrometry (LC-MS

Methylcitric acid (MCA) analysis has been mainly utilized for the diagnosis of propionate disorders or as a second-tier test in newborn screening, but its utility for patients monitoring still needs to be established. We explored the potential contribution of MCA in the long-term management of organic acidurias. We prospectively evaluated plasma MCA and its relationship with disease biomarkers, clinical status, and disease burden in 22 patients, 13 with propionic acidemia (PA) and nine with methylmalonic acidemia (MMA) on standard treatment and/or after transplantation. Samples were collected

Hyperammonemia is a common finding in children with methylmalonic acidemia and propionic acidemia, but its contribution to the development of the neurological symptoms in the affected patients is poorly known. Considering that methylmalonic acid (MMA) and propionic acid (PA) predominantly accumulate in these disorders, we investigated the effects of hyperammonemia induced by urease treatment in 30-day-old rats receiving an intracerebroventricular (ICV) injection of MMA or PA on important parameters of redox homeostasis in cerebral cortex and striatum. We evaluated glutathione (GSH) concentrati

OBJECTIVES: Alterations in the enzymes involved in homocysteine (Hcy) metabolism or vitamin deficiency could play a role in coronary artery disease (CAD) development. This study investigated the influence of MTHFR and MTR gene polymorphisms, plasma folate and MMA on Hcy concentrations and CAD development. MMA and folate concentrations were also investigated according to the polymorphisms.
METHODS: Two hundred and eighty-three unrelated Caucasian individuals undergoing coronary angiography (175 with CAD and 108 non-CAD) were assessed in a case-control study. Plasma Hcy and MMA were measured by

The cobalamin metabolism in chronic myelogenous leukemia (CML) was evaluated in 18 newly diagnosed and untreated patients by formiminoglutamic acid (FiGlu) and methyl malonic acid excretion (MMA) tests. A deoxyuridine (dU) suppression test of bone marrow cells was compared in patients with acute myelogenous leukemia (N = 5), myelodysplastic disease (N = 3), untreated pernicious anemia (N = 16), folate deficiency (N = 7), and a hospital reference group without signs of cobalamin or folate deficiency (N = 22). All had normal MMA excretion but 3 of 15 patients had increased FiGlu excretion. In vi

INTRODUCTION: Propionic aciduria (PA) and methylmalonic aciduria (MMA) are rare inherited metabolic disorders marked by recurrent metabolic decompensation and hyperammonaemia, resulting in neurological morbidity, chronic complications, and substantial healthcare burden. Although carglumic acid (Carbaglu®) is established for acute hyperammonaemic crises, evidence for its long-term use is limited. This study reports the 18-month interim analysis of PROTECT, a large prospective observational study evaluating chronic Carbaglu® therapy in PA and MMA.

AIM: Vitamin B-12 deficiency is common among older adults and can lead to irreversible neurological damage and severe complications if not treated early. Despite its clinical significance, effective management remains challenging due to the absence of simple and sensitive biomarkers for early detection and for monitoring treatment response.
PURPOSE: This pilot study evaluated the feasibility and potential of the urinary methylmalonic acid (uMMA)/creatinine ratio as a biomarker of vitamin B-12 deficiency in older adults, while exploring clinical responses following supplementation.

Chronic kidney disease (CKD) is associated with cognitive impairment in older adults. Mitochondrial dysfunction (MD) is a key contributor to aging-related diseases, yet its potential role in mediating the relationship between renal function and cognitive function (CF) remains unclear. Therefore, we aimed to examine this association in older adults and to evaluate the mediating effect of MD. Our study analyzed data from the 2011-2014 National Health and Nutrition Examination Survey, including participants aged ≥60. CF was assessed using the Animal Fluency Test (AFT), the Consortium to Establish
