Nail Unit Melanoma In Situ Treated With Mohs Micrographic Surgery.

BACKGROUND: Mohs micrographic surgery (MMS) is under used in the treatment of nail unit melanoma in situ (MIS), with limited studies in the literature. OBJECTIVE: Report clinical outcomes for nail unit MIS using MMS with melanoma antigen recognized by T cells-1 (MART-1) immunostaining. METHODS: A retrospective observational study at a single academic institution of patients with a diagnosis of nail unit MIS treated with MMS with MART-1 immunostaining from January 1, 2006, to December 30, 2016. The primary outcome measure was the recurrence rate after MMS. RESULTS: Fourteen patients were ide

Characteristics of Lower Extremity Infection Rates Following Mohs Micrographic Surgery.

BACKGROUND: Surgical site infection (SSI) is the most common complication for Mohs micrographic surgery (MMS). Lower extremity surgical sites are at an increased risk for developing SSI. OBJECTIVE: This study aimed to evaluate lower extremity SSI rates post-MMS based on closure type and antibiotic usage. MATERIALS AND METHODS: A retrospective review was performed of all lower extremity MMS cases from 2011 to 2016 at Vanderbilt University Medical Center. Patient history, surgical details, and follow-up appointments were reviewed. RESULTS: Six hundred twenty MMS lower extremity surgeries were

A case of streptococcal surgical site infection following Mohs surgery.

Group A β-hemolytic Streptococcal (GAS) cellulitis is an uncommon surgical site infection that presents with rapid onset of pain and swelling in the first few days after a procedure. Unlike staphylococcal cellulitis, GAS cellulitis lacks purulence and spreading erythema. The absence of these classic signs may delay the diagnosis of GAS cellulitis and lead to severe complications. We present the case of an immunosuppressed 49-year-old patient who developed swelling and severe pain at his incision site two days after undergoing Mohs micrographic surgery on his forehead. He was clinically diagnos

Mohs micrographic surgery for digital melanoma and nonmelanoma skin cancers.

Treatment of digital skin cancers is challenging due to various functional and cosmetic implications. Traditionally, routine treatment includes radical amputation, but digital skin cancers are increasingly being treated with more conservative, tissue-sparing methods such as Mohs micrographic surgery (MMS), which provides excellent tissue conservation and margin control when used to treat melanoma and nonmelanoma skin cancers (NMSCs). In this study, we conducted a retrospective chart review to evaluate clinical outcomes following MMS for treatment of digital melanoma and NMSCs.

Outcomes of Mohs micrographic surgery for periocular squamous cell carcinoma.

INTRODUCTION: Periocular cutaneous squamous cell carcinoma (cSCC) accounts for 5-10% of all eyelid malignancies. Periocular cSCC carries a low mortality rate but can be destructive to local tissues. Due to the unique function and anatomy of the eyelids, Mohs micrographic surgery (MMS) is gold standard for treating cSCC to preserve healthy tissue and reduce rates of local recurrence. In this study, we describe the success and outcomes of MMS and subsequent oculoplastic reconstruction for periocular cSCC in the North East of England. METHODS: Retrospective analysis of 34 patients who underwent

Quantitative Analysis of Frozen Section Histology in Mohs Micrographic Surgery.

BACKGROUND: Mohs micrographic surgery (MMS) relies on complete frozen tissue sections for the unique combination of high cure rates and tissue preservation. OBJECTIVE: To quantify and characterize the tissue sectioning process in MMS. MATERIALS AND METHODS: Two hundred eighty-four tissue blocks were analyzed. The histotechnician measured the depth cut into the tissue block for every section placed on the slide. The surgeon identified complete sections. RESULTS: First-stage complete sections were achieved at 285 μm. The ear and hair-bearing cheek required increased depth of sectioning compar

Microcystic adnexal carcinoma. Ten cases treated by Mohs micrographic surgery.

BACKGROUND: Microcystic adnexal carcinoma is a slow-growing, nondescript, locally aggressive, deeply infiltrating neoplasm histologically characterized by an infiltrative pattern of basaloid or squamous cells, a desmoplastic stromal reaction, keratin-filled cysts, and glandular structures. OBJECTIVE: Microcystic adnexal carcinoma is uncommon and may be mistaken microscopically for other benign and malignant entities. Perineural or intraneural involvement by tumor cells is characteristic and extension into underlying structures including muscle, fat, and bone are frequently encountered. Althou

Cutaneous malignant melanoma treated by Mohs surgery. Review of the treatment results of 179 cases from the Mohs Melanoma Registry.

BACKGROUND: There has been much debate regarding the efficacy of the Mohs chemosurgery fixed-tissue technique vs the fresh-tissue technique in the treatment of cutaneous melanoma. OBJECTIVE: To review the treatment results of the 179 cases of melanoma registered with the Mohs melanoma tumor registry from 1981 to 1991, accumulated from nine referring Mohs surgeons. METHODS: Review of the two treatment techniques using a case presentation format. There were 113 cases treated by the hybrid fixed-tissue technique and 61 cases treated by the fresh-tissue technique. The data compared technique of

Mohs Micrographic Surgery for the Treatment of Cutaneous Melanomas of the Head and Neck.

Surgical excision achieving clear histologic margins remains the mainstay treatment for primary cutaneous melanoma. Tumors of the head and neck, particularly those arising in chronically sun-damaged skin, often demonstrate extensive and asymmetric subclinical extension. Over the decades, this has proven to be a significant problem for tumors arising on the head and neck, as anatomic and functional complexities of these areas have led to suboptimal surgical treatment, yielding unacceptably high rates of local recurrence and persistently positive margins with traditional wide local excision. Pat
