BACKGROUND & AIMS: EM523, a nonpeptide motilin agonist, is being developed for clinical use to improve delayed gastric emptying. The aim of this study was to examine the effect of EM523 on endogenous release of gut and pancreatic hormones in conscious dogs.
METHODS: Motility of the gastrointestinal tract was monitored using force transducers. Blood concentrations of gut and pancreatic hormones were measured using a specific radioimmunoassay. We examined the relationship of interdigestive gallbladder emptying with the different phases of the migrating motor complex (MMC) and with plasma levels of cholecystokinin (CCK) and motilin. In 10 volunteers 20 cycles of the MMC were recorded. In 11 cycles phase III occurred in antrum and duodenum (group 1). In nine cycles phase III was observed only in duodenum (group 2). Natural motilin was investigated for its effect on gastrointestinal motor activity in anesthetized dogs. In doses starting from 10 ng/kg as single injection and 100 ng/kg-h as intravenous infusion, motilin induced phasic pressure changes in the lower esophageal sphincter (LES) and in the lumen of the fundus, antrum, and duodenum. Tachyphylaxis was seen in 6 out of 10 dogs. Motilin had no effect on isolated muscle strip preparations of the LES, fundus, antrum, and duodenum in this species.
