Relationship between pleural changes after myocardial revascularization and pulmonary mechanics.

It is known that coronary artery bypass grafting (CABG) results in impairment of postoperative pulmonary function. There is also a high incidence of pleural changes (pleural effusion or pleural thickening) after CABG. We hypothesized that those patients with pleural changes in the postoperative period would have a greater decrease in pulmonary function test (PFTs) results. The present study reports the results of 110 male patients who underwent CABG. The chest films and the PFT results obtained preoperatively and on the sixth postoperative day were reviewed. The relationship between pleural ch

Coronary-artery spasm immediately after myocardial revascularization: recognition and management.

We investigated coronary-artery spasm in six patents who had had unexpected hemodynamic collapse within two hours after cardiopulmonary bypass for myocardial revascularization. All six had profound hypotension and recurrent ST-segment elevation in electrocardiographic Leads II, III, and aVF. All had either normal or noncritical luminal irregularities of dominant right coronary arteries and more than 75 per cent occlusions in the left coronary circulation. Right-coronary-artery spasm, which was reversed after intracoronary nitroglycerin, was demonstrated angiographically in one patient; a paten…

Neurological injury in patients with acute myocardial infarction undergoing operative myocardial revascularization within 48 h.

BACKGROUND: Acute myocardial infarction (AMI) is associated with an increased rate of neurological events (NE). AMI patients, who undergo coronary artery bypass graft (CABG) surgery may be at an even greater risk for peri-operative NE, but detailed data is missing. METHODS: We conducted a retrospective, single-center data analysis of 1628 patients that underwent CABG within 48 h after being diagnosed with AMI. Between 01/2001 and 03/2023, 77 patients (4.7%) suffered from a peri-operative NE. This included 66 (4.0%) thromb-embolic strokes and 11 (0.7%) hypoxic brain damages. We compared the ou

Prophylactic intra-aortic balloon counterpulsation before surgical myocardial revascularization in patients with acute myocardial infarction.

INTRODUCTION: Prophylactic intra-aortic balloon counterpulsation (pIABC) is recommended for high-risk patients undergoing coronary artery bypass grafting (CABG) surgery. Criteria for high-risk patients benefiting from pIABC are unclear. This study aimed to specifically describe the effect of pIABC on outcomes of patients with acute myocardial infarction (AMI) undergoing CABG. METHODS: In 178 of 484 AMI patients (non-ST-segment elevation myocardial infarction [NSTEMI] or ST-segment elevation myocardial infarction [STEMI] ≤5 days before surgery) without cardiogenic shock who underwent CABG betw

Molecular and Cellular Mechanisms of Cardioplegic Protection in Surgical Myocardial Revascularization.

Coronary artery bypass grafting (CABG) remains the gold standard for patients with advanced multivessel coronary artery disease. Optimal myocardial protection versus ischemia during reversible and controlled cardiac arrest is a cornerstone of successful outcomes. Myocardial ischemia represents a state of reduced coronary perfusion with oxygenated blood, insufficient to meet the metabolic demands of the myocardium. Conventional cardioplegic solutions offer controlled and reversible cardiac arrest while actively modulating the molecular and cellular mechanisms that mediate ischemia-reperfusion i

Unresolved Controversies in Revascularization for Cardiogenic Shock with Acute Myocardial Infarction.

Cardiogenic shock occurs in 5–10% of patients with acute myocardial infarction (AMI). Routine early culprit-vessel revascularization and selective microaxial flow pump placement improve survival, but the management of multivessel coronary artery disease (CAD) in patients with cardiogenic shock remains controversial. Randomized trials are needed to address the optimal mode and timing of coronary revascularization with respect to the initiation of mechanical circulatory support. Ventricular unloading prior to multivessel percutaneous coronary intervention and emergency coronary artery bypass gra

Challenge of concomitant thymoma resection and myocardial revascularization: A Case Report.

Myocardial revascularization in patients presenting with an anterior mediastinal mass poses considerable challenges. In this report, we outline two cases involving patients with anterior mediastinal masses who underwent surgical resection alongside concurrent myocardial revascularization. One patient underwent coronary artery bypass graft surgery, while the other was treated by percutaneous coronary intervention with drug-eluting stent placement. Both patients fully recovered from the relative procedures and were discharged within two weeks post-surgery, ultimately diagnosed with thymoma. The …

[Multiple myocardial revascularization on the beating heart: risks, benefits and outlooks].

INTRODUCTION: Avoidance of extracorporeal circulation during beating heart surgery (OP CAB = Off-Pump Coronary Artery Bypass Surgery) for aortocoronary bypass grafting (ACBG) is gaining increasing importance in modern cardiac surgery. With the development of new mechanical stabilization devices, the revascularization of the posterior wall of the heart and the distal right coronary artery became feasible. Especially high-risk patients with multiple risk factors for open heart surgery will profit from this approach, because the negative effects of the extracorporeal circulation are avoided. MET

Cryopreserved veins in myocardial revascularization: possible mechanism for their increased failure.

BACKGROUND: Cryopreserved veins are used as conduits for myocardial revascularization. However, a high failure rate associated with their use has been reported anecdotally. METHODS: To find an explanation for the poor performance of cryopreserved vein grafts, we conducted a retrospective 5-year study on all patients at a single institution in whom cryopreserved vein grafts were used. We further performed in vitro studies measuring cell adhesion, nitric oxide production, and contractile capacity of saphenous vein, internal thoracic artery, and cryopreserved veins. RESULTS; Forty-one patients w…

[Results of myocardial revascularization in patients aged 70 years and over].

The purpose of this study was to bring up-to-date the immediate risks and long-term benefits of coronary surgery in elderly people. The results obtained in a series of 475 patients in their seventies suffering from coronary disease who underwent myocardial revascularization alone or combined with heart valve replacement at the La Pitié Hospital, Paris, between 1984 and 1989 were analyzed. Coronary disease was associated with heart valve disease in two-thirds of the cases. The mean number of bypasses was 2.3 per patient. Aortic valve replacement was performed in 119 cases and mitral valve repla…

[Myocardial infarction in early postoperative complications in operations of myocardial revascularization: incidences, clinical study and mechanisms].

A study of the electrocardiograms and enzyme levels after surgery to revascularise the myocardium in a group of 174 consecutive patients has allowed us to demonstrate 19 cases of early postoperative myocardial infarction (EPOMI) (10.9%). 10 of these cases had no clinical features of EPOMI; 9 had clinical signs (collapse of altered systemic pressures, arrhythmias) or haemodynamic disorders (raised left atrial pressure). The EPOMI turned out to be fatal in 3 cases (19%), and to be complicated by left ventricular failure in 6 cases (32%). Post-operative coronary arteriography showed that 8 out of

[What's new in the 2018 ESC/EACTS guidelines on myocardial revascularization?].

A revised version of the European Society of Cardiology (ESC) and European Association for Cardio-Thoracic Surgery (EACTS) guidelines on myocardial revascularization has recently been published. These guidelines represent the third time that the ESC and EACTS have brought together cardiologists and cardiac surgeons in a joint Task Force to review the available evidence, with the mission of drafting balanced, patient-oriented, clinical practice guidelines on myocardial revascularization. During the last 4 years, a large body of evidence has become available and brought substantial changes to th…

An Integrated Approach to von Willebrand Disease and Surgical Myocardial Revascularization.

Cardiac surgery in patients with preexisting bleeding disorders can be a challenge. Cardiopulmonary bypass can lead to bleeding disorders, above all in patients with coagulopathy. We report the case of a 42-year-old woman, with type I von Willebrand disease, who underwent off-pump coronary artery bypass grafting. Beating heart surgery associated with an adequate replacement of von Willebrand factor and factor VIII levels were chosen to prevent bleeding disorders. Her postoperative course was uneventful and she was discharged home after 5 postoperative days.

The adequacy of myocardial revascularization in patients with multivessel coronary artery disease.

In patients with multi-vessel coronary artery disease (MVCAD) myocardial revascularization may be accomplished either on all diseased lesions--complete myocardial revascularization--or on selectively targeted coronary segments by percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG). Complete revascularization has a potential long-term prognostic benefit, but is more complex and may increase in-hospital events when compared with incomplete revascularization. No conclusive agreement has been yet reached on the "optimal" extent of revascularization, and guidelines ha…

[Use of the internal thoracic artery in repeat myocardial revascularization surgery].

The incidence of redo myocardial revascularization is increasing lately. These procedures are accompanied by the higher operative risk, and the use of internal thoracic artery graft may have additional negative impact on early-rization. Mortality and morbidity in this group did not differ significantly compared to a group where only venous grafts were used. Independent predictors of the unfavorable outcome (for both groups) were the presence of the chronic pulmonary disease, age over 65 and NYHA functional class IV. In the group with internal thoracic graft it was the age over 65, while in the…

A profile of candidates for repeat myocardial revascularization: implications for selection of treatment.

OBJECTIVES: It is not known whether the results of randomized trials comparing coronary artery bypass grafting to percutaneous transluminal coronary angioplasty for initial revascularization apply to repeat revascularization in patients with prior bypass grafts. We studied the differences between the patients with prior bypass grafts referred for surgery or angioplasty to identify the clinical and angiographic characteristics that correlated best with either choice and to find clues that might aid in selecting one treatment over the other. METHODS: Between 1992 and 1994, 870 patients underwen
