Association of glycaemic control with post-radiation nasopharyngeal necrosis in diabetic patients with nasopharyngeal carcinoma.

BACKGROUND: Post-radiation nasopharyngeal necrosis (PRNN) is a rare but life-threatening late complication in patients with nasopharyngeal carcinoma (NPC). Diabetes mellitus (DM) has been reported as a risk factor for PRNN; however, the impact of long-term glycaemic control on PRNN risk and severity remains unclear. PATIENTS AND METHODS: We conducted a retrospective cohort study of diabetic patients with newly diagnosed NPC treated with intensity-modulated radiotherapy (IMRT) between 2019 and 2022. Long-term glycaemic control was assessed using hemoglobin A1c (HbA1c) levels obtained within 3

Predictive value of nasopharyngeal microbiota for necrosis after re-irradiation in recurrent nasopharyngeal carcinoma.

BACKGROUND: Post-radiation nasopharyngeal necrosis (PRNN) is a severe complication following re-irradiation in patients with recurrent nasopharyngeal carcinoma (NPC). This study aimed to explore the association between nasopharyngeal microbiota and PRNN in patients with recurrent NPC undergoing re-irradiation and to evaluate the predictive value of the microbiota for PRNN. METHODS: This retrospective study collected data from 113 patients with recurrent NPC who underwent re-irradiation at the Sun Yat-sen University Cancer Center (SYSUCC) between January 2020 and November 2022. Patients were d

Diagnosis of SARS-CoV-2 in children: accuracy of nasopharyngeal swab compared to nasopharyngeal aspirate.

The tests currently used for the identification of SARS-CoV-2 include specimens taken from the upper and lower respiratory tract. Although recommendations from the World Health Organization prioritise the usage of a nasopharyngeal swab (NS), nasopharyngeal aspirates (NPA) are thought to be superior in identifying SARS-CoV-2 in children. To our knowledge, however, no paediatric study has been published on the subject. The aim of this study is to evaluate the diagnostic performances of NS referred to NPA for SARS-CoV-2 in children. We calculated the sensitivity and specificity of the NS referred

STIL is upregulated in nasopharyngeal carcinoma tissues and promotes nasopharyngeal carcinoma proliferation, migration and invasion.

Nasopharyngeal carcinoma (NPC) is the most common primary malignancy that originates from the nasopharynx. Some regulatory networks involved in nasopharyngeal carcinoma have been reported, but the relevant genes have not been fully identified. We have used mRNA microarray to identify differential expression genes between NPC tissues and adjacent normal tissues. Then high-content shRNA screening was carried out to screen the genes that may control proliferation in nasopharyngeal carcinoma. Cell proliferation was monitored by MTT assays and Celigo image cytometry in vitro and subcutaneous transp

Clinical findings and imaging features of 67 nasopharyngeal carcinoma patients with postradiation nasopharyngeal necrosis.

Postradiation nasopharyngeal necrosis is an important late effect of radiotherapy that affects prognosis in patients with nasopharyngeal carcinoma. In the present study, we reviewed the clinical and imaging features of 67 patients with pathologically diagnosed postradiation nasopharyngeal necrosis who were treated at Sun Yat-sen University Cancer Center between June 2006 and January 2010. Their clinical manifestations, endoscopic findings, and imaging features were analyzed. Early nasopharyngeal necrosis was limited to a local site in the nasopharyngeal region, and the tissue defect was not ob

Total nasopharyngeal stenosis: treatment with laser excision, nasopharyngeal obturators, and topical mitomycin-c.

OBJECTIVE: Severe nasopharyngeal stenosis after uvulopalatopharyngoplasty (UPPP) is a dreaded complication. Very little has been written about successful treatment because attempts at correction frequently fail. Severe stenosis of the nasopharynx drastically worsens speech, swallowing, and obstructive sleep apnea (OSA). We present our successful experience with severe stenosis focusing on a simple functional repair using topical mitomycin-c as well as differing daytime and nighttime palatal obturators. METHODS: Three adults with total or near-total nasopharyngeal stenosis secondary to UPPP we

Epstein-Barr virus in oropharyngeal and nasopharyngeal secretions of patients with nasopharyngeal carcinoma and control subjects.

The frequency of oropharyngeal excreters of the Epstein-Barr virus among patients with nasopharyngeal carcinoma in Hong Kong was compared with those of healthy adults in Hong Kong and California. 6 (3%) of 177 patients, 11 (12%) of 92 Hong Kong residents, and 20 (15%) of 132 Californians were excreters. The virus was detected in the nasopharyngeal secretion of only 1 of 67 patients and in 2 of 73 healthy adults. No convincing evidence for neutralizing antibody in the throat wash and nasopharyngeal secretions of the patients could be obtained. Epstein-Barr viral gene sequencing could not be det

Prognostic indicators and microbial landscape of post-radiation nasopharyngeal necrosis in nasopharyngeal carcinoma patients.

BACKGROUND: Post-radiation nasopharyngeal necrosis (PRNN) is a serious complication in patients with nasopharyngeal carcinoma (NPC) treated with radiotherapy, and is often accompanied by infections such as sinusitis, otitis media, or central nervous system infections. This study retrospectively examined the clinical features, microbial profiles, treatment approaches, and prognostic factors of patients with PRNN to improve clinical management. METHODS: Patients with NPC diagnosed with PRNN at Taipei Veterans General Hospital between September 2005 and April 2021 were reviewed. The data include

Classifications of Postradiation Nasopharyngeal Necrosis for Guiding Transnasal Endoscopic Nasopharyngeal Skull Base Surgery.

OBJECTIVE: Postradiation nasopharyngeal necrosis (PRNN) is a severe postradiation complication of nasopharyngeal carcinoma. PRNN not only severely impacts the quality of life but also endangers the lives of patients. Definite grades of PRNN and their corresponding standard surgical procedures are lacking. we aimed to establish the classifications of PRNN for transnasal endoscopic skull base surgery (TESS). METHODS: We enrolled 82 postirradiation NPC patients with PRNN, 75 of whom received TESS. PRNN was categorized into four grades: I, necrosis of the nasopharyngeal mucosa and submucosal musc

Evaluation of nasal and nasopharyngeal swab collection for the detection of Epstein-Barr virus in nasopharyngeal carcinoma.

Epstein-Barr virus detection using nasopharyngeal swabs has been suggested as a potential screening test that could improve the specificity of current EBV-based serological assays. However, application requires insertion of the swab deep into the nasopharynx, a procedure not amenable to non-clinic screening. We reasoned that swabbing the more easily accessible nasal cavity might provide an appealing alternative for NPC detection. Patients > 18 years of age diagnosed with histologically confirmed NPC were recruited from the Otolaryngology Department at the National Taiwan University Hospital. E

Treatment Response Trajectories of Epipharyngeal Abrasive Therapy (EAT) in Chronic Nasopharyngitis: Phenotypic Classification Into Early, Delayed, and Non-Responders Based on ΔEAT With Outcome Stratification.

Background Treatment responses to epipharyngeal abrasive therapy (EAT) for chronic nasopharyngitis are heterogeneous. Previous analyses of this cohort examined longitudinal changes in EAT and bleeding scores with stratification by tissue type and baseline severity. However, those analyses mainly evaluated overall score changes and baseline-based stratification rather than the timing of individual improvement. The novelty of the present study lies in its focus on temporal response trajectories, specifically when patients improve after EAT. This study classified patients into early responders, d…

Pediatric Nasopharyngeal Carcinoma (NPC).

PURPOSE OF REVIEW: Pediatric nasopharyngeal carcinoma (NPC) is a rare epithelial malignancy strongly associated with Epstein–Barr virus infection and typically presents at an advanced stage due to its deep anatomical location and nonspecific symptoms. The purpose of this review is to summarize current evidence regarding epidemiology, molecular pathogenesis, diagnostic advances, and contemporary treatment outcomes in pediatric NPC. RECENT FINDINGS: Recent studies confirm that pediatric NPC is predominantly an undifferentiated non-keratinizing carcinoma and is almost universally associated with

Nasopharyngeal melioidosis: a case report.

A 12-year-old boy was admitted after 11 days of fever and 2 days of nasal obstruction as well as swelling of a right cervical lymph node. Nasal endoscopy and computed tomography of the neck showed a nasopharyngeal mass occupying the entire nasopharynx, extending into the nasal cavity, and obliterating the fossa of Rosenmuller. Abdominal ultrasonography revealed a small solitary splenic abscess. Although a nasopharyngeal tumor or malignancy was initially considered, biopsy of the mass showed only suppurative granulomatous inflammation, and bacterial culture from the enlarged cervical lymph node

Nasopharyngeal tuberculosis: A case report.

Nasopharyngeal tuberculosis is a rare disease. Even in areas where tuberculosis is endemic, its incidence is also extremely low. Here, we will report a rare case of nasopharyngeal tuberculosis. A 29-year-old male patient presented with a 2-month history of sore throat, nasal congestion, dysphagia, and low-grade fever. Thickened mucosa in the posterior wall of the nasopharynx was shown from nasopharyngoscopy, covered with a thick, yellowish, purulent secretion that was not easily removed. Computed tomography of the nasopharynx showed thickening of the mucosa in the right and left bilateral wall

Nasopharyngeal melanoma: An unusual entity.

Nasopharyngeal melanoma is a rare condition with an estimated incidence of about 0.3/10 lac per year and has a slight female predominance. It can arise from stem melanocytes and mature melanocytes of the submucosa that have acquired genetic alterations, may be due to tobacco, trauma and oxidative stress. It resembles other common polypoidal lesions; therefore, histopathological examination with immunohistochemistry plays a pivotal role in confirming the diagnosis. Lack of specific clinical features often leads to a delay in diagnosis.
