Anesthetic and airway management during laser treatment for retinopathy of prematurity: a survey of US ophthalmologists and neonatologists.

Ophthalmologists and neonatologists were surveyed to assess current practices for anesthetic and airway management techniques used during laser treatment for retinopathy of prematurity. Of the 351 practitioners who completed the survey, most perform fewer than 10 treatments annually. Intravenous sedation, rather than general anesthesia, is used by 60% of respondents. Routine intubation is used by 58%, whereas 36% monitor infants at their current level of respiratory support.

Neonatologist-performed point-of-care abdominal ultrasound: What have we learned so far?

This review describes the sonographic appearances of the neonatal bowel in Necrotising enterocolitis. It compares these findings to those seen in midgut-Volvulus, obstructive intestinal conditions such as milk-curd obstruction, and slow gut motility in preterm infants on continuous positive airway pressure (CPAP)-CPAP belly syndrome. Point-of-care bowel ultrasound is also helpful in ruling out severe and active intestinal conditions, reassuring clinicians when the diagnosis is unclear in a non-specific clinical presentation where NEC cannot be excluded. As NEC is a severe disease, it is often

Diuretics for very low birth weight infants in the first 28 days: a survey of the U.S. neonatologists.

OBJECTIVE: The objective of this study was to describe factors influencing diuretic use by neonatologists caring for very low birth weight neonates. STUDY DESIGN: We surveyed 400 U.S. neonatologists. Respondents made therapeutic decisions in clinical scenarios involving very low birth weight infants at 7, 14 and 28 days of age. RESULT: Response rate was 39%. Diuretic therapy was chosen in 31% of scenario decisions, with pro re nata dosing selected early and regular dosing more common at later ages. Diuretic use was strongly associated with method of respiratory support, and was chosen less o…

[Retinopathy of prematurity screening and follow-up with Retcam120: expertise of a team of neonatologists concerning 145 patients].

OBJECTIVES: The high incidence of retinopathy in very premature infants requires strict evaluation and follow-up in neonatal intensive care. The strict organization required in each center, under the responsibility of ophthalmologists, is sometimes puzzling. Therefore, we tested the hypothesis that the introduction of the Retcam allows the neonatologist under the control of ophthalmologist to diagnose retinopathy of prematurity then preventing sequelae, by comparison of pictures interpretations between neonatologists and ophthalmologists. METHODS: The Retcam gives a 120 degrees picture of the…

Neonatologists can impede or support parents' participation in decision-making during medical rounds in neonatal intensive care units.

AIM: We explored the dynamics of neonatologist-parent communication and decision-making during medical rounds in a level three neonatal intensive care unit. METHODS: This was a qualitative study, with an ethnographic approach, that was conducted at Turku University Hospital, Finland, from 2013 to 2014. We recruited eight mothers and seven couples, their 11 singletons and four sets of twins and two neonatologists and observed and video recorded 15 medical rounds. The infants were born at 23 + 5 to 40 + 1 weeks, and the parents were aged 24-47. The neonatologists and parents were interviewed se…

Neonatal Training across Union of European Medical Specialists Member Countries: A Survey from the Next Generation of Neonatologists.

UNLABELLED: <p>Introduction: Taking care of critically ill newborn infants is challenging, even more so in the current era of greater use of non-invasive support, leaving limited exposure to learn critical skills. Supporting a growing workforce of neonatologists requires training of doctors with formal curriculums and assessment of skills ensuring delivery of high-quality care. Our aim was to gain an understanding of neonatal trainee experiences regarding current neonatal training across Union of European Medical Specialists (UEMS) member countries. The main focus of this survey was on pulmono…

The definition of life: a survey of obstetricians and neonatologists in New York City hospitals regarding extremely premature births.

Among obstetricians and neonatologists in administrative roles in New York City hospitals, a survey was initiated to compare the physicians' definitions of live birth and fetal death, the gestational age at which they consider infants viable, and the resuscitation practices of the neonatologists. The target survey population was 34 neonatologists, and 39 obstetricians representing 41 of the City's 43 maternity hospitals. A telephone survey was used to gather qualitative data from the physicians regarding their definitions of live birth, fetal death, and viability, and their practices regarding…

A survey of intravenous sodium bicarbonate in neonatal asphyxia among European neonatologists: gaps between scientific evidence and clinical practice.

BACKGROUND: Birth asphyxia is characterized by intermittent periods of hypoxia/ischemia leading to metabolic acidosis. The use of intravenous sodium bicarbonate (IVSB) is still a matter of controversy in clinical practice. OBJECTIVE: To assess the use of IVSB in birth asphyxia by attending neonatologists in European hospitals. DESIGN/METHODS: Survey using a questionnaire administered to neonatologists in Europe whose electronic addresses were provided by the national councils of neonatology of each participating country. The questionnaire consisted of a brief theoretical introduction followe…

Recognising Inborn Errors of Metabolism Beyond the Newborn Screening Net: A Practical Guide for the General Paediatrician and Neonatologist.

Inborn errors of metabolism (IEMs) are individually rare but collectively significant contributors to neonatal and paediatric morbidity. While expanded newborn screening, urine metabolic screening and broad molecular testing have transformed diagnostic capability, a substantial proportion of IEMs remain outside the reach of these tools, and clinical suspicion at first contact, typically the general paediatrician or neonatologist, remains essential to timely diagnosis. This review offers a practical overview for the non-specialist clinician: the scope and limits of newborn and urine metabolic s

Predictive Model of Early Spontaneous Ductus Arteriosus Closure Based on Neonatologist Performed Echocardiography in Preterm Infants.

Background: Patent ductus arteriosus (PDA) treatment remains controversial. Modeling on the predictive capacity of early spontaneous PDA closure would help in decision-making. Aim: To design a predictive model of early spontaneous PDA closure. Methods: As part of a trial to assess efficacy and safety of two ibuprofen treatment schemes for PDA, infants below 29 weeks' gestation were scanned between 18 and 72 h of birth, and serially if indicated. PDA treatment was decided based on echocardiography signs of lung overflow or systemic hypoperfusion and clinical criteria. A PDA score that included

Nephrotoxic medications and acute kidney injury risk factors in the neonatal intensive care unit: clinical challenges for neonatologists and nephrologists.

Neonatal acute kidney injury (AKI) is common. Critically ill neonates are at risk for AKI for many reasons including the severity of their u … obactam, and amphotericin. We aim to illustrate the complexity of decision-making involved for both neonatologists and pediatric nephrologists when managing infants with these conditions and advocate for ongoing multidisciplinary collaboration in the development of better AKI surveillance protocols and AKI mitigation strategies to improve care for these vulnerab …

Utilization of Volume-Targeted Ventilation in Neonatal Intensive Care Units in the United Arab Emirates: A Survey of Practicing Neonatologists.

BACKGROUND: Volume-targeted ventilation (VTV) is an evidence-based, lung-protective strategy demonstrated to reduce the incidence of bronchopulmonary dysplasia (BPD) and severe neurologic injury in premature infants. While variations in VTV adoption are well-documented in North America and Europe, data characterizing its utilization across the broader Middle East remains limited. OBJECTIVE: To evaluate the utilization rates of VTV, the selection of tidal volume (VT) targets across various clinical scenarios, and the perceived barriers to VTV implementation among practicing neonatologists in t…

Respiratory distress in late-preterm neonates: from controversy to consensus - a survey of regional advisory boards and neonatologists in France.

OBJECTIVE: Late-preterm neonates present with more complications at birth than full-term neonates. The care of these infants is sometimes controversial. Our study aims at examining the practices of French neonatologists and compares them to the literature. DESIGN: Regional advisory boards discussed a clinical case with four categories of questions: the risk of term-related respiratory distress (RD), the indication for antenatal corticosteroid therapy, neonatal care at delivery and the use of surfactant. An anonymous survey covering the same question categories was also sent to French neonatol…

Characteristics of chest ultrasound performed by neonatologists in acute viral respiratory infection and its relationship with respiratory support and mortality.

BACKGROUND: Respiratory viruses are a frequent cause of infection in neonatology. Point-of-care lung ultrasound (LU) and targeted neonatal echocardiography (TnE) allow pulmonary and hemodynamic assessment at the patient's bedside. METHODS: The aim of this study was to describe the pattern of alterations found by LU and to assess the hemodynamic status in neonates diagnosed with viral infection (reverse transcription polymerase chain reaction) during 2018-2023. LU was performed in 10 regions, and a semi-quantitative scale LU score (LUS) was calculated. TnE was performed if pulmonary hypertensi
