Retrograde labeling of the rat facial nerve with carbocyanine dyes to enhance intraoperative identification.

OBJECTIVES: Removal of head and neck neoplasms, especially those of the parotid gland and those of the internal auditory canal and cerebellopontine angle, often requires microdissection of the facial nerve. Displacement or splaying of the nerve can make it difficult to identify facial nerve fibers and/or distinguish them from surrounding tissues. Here we tested a method of labeling the facial nerve with fluorescent lipophilic dyes as a method of providing intraoperative visual confirmation of nerve fibers. METHODS: The facial nerves of adult rats were retrogradely labeled with 1,1'-dioctadecy

ILLUSTRATIVE CASE OF CAPILLARY HEMANGIOMA OF THE OPTIC NERVE.

Capillary hemangiomas (CH) are benign proliferative vascular neoplasms commonly present in skin and soft tissues but rarely found intracranially. We describe a case of a 14-year-old male patient with histologically proven CH of the optic nerve, who underwent surgical resection of the lesion due to progressive visual loss. Imaging studies revealed a cystic-solid formation in the suprasellar region lateralized to the right and located above the pituitary gland. This case shows that CH may originate from the optic nerve, leading to its gradual compression and causing optic neuropathy. While the c

Orbital spindle cell neoplasm with neural differentiation: diagnostic challenges in the absence of molecular confirmation.

Orbital spindle cell tumors with neural differentiation represent an uncommon and diagnostically challenging entity in clinical practice. We report the case of an 82-year-old man with a history of treated rectal adenocarcinoma, who presented with progressive orbital pain and right-sided proptosis. Computed tomography revealed a soft tissue mass located at the orbital apex, without evidence of bone erosion. Orbital exenteration was performed, and histopathological examination demonstrated an infiltrative high-grade spindle cell neoplasm with a fascicular growth pattern, moderate pleomorphism, a

Label-Free Facial Nerve Identification During Parotidectomy Using Nerve Autofluorescence: An Initial Clinical Series.

Facial nerve (FN) dysfunction remains a clinically important complication of parotidectomy, even when recovery is ultimately complete. Traditional nerve identification relies on anatomic dissection, selective stimulation, and, in many cases, intraoperative monitoring (IONM). These approaches may be limited when anatomy is distorted. This study evaluated the feasibility, safety, and utility of autofluorescence-guided FN identification during parotidectomy. A retrospective case series was conducted in 18 patients who underwent superficial or total parotidectomy for parotid gland tumors. All proc

Myofibroblasts Induce Neuroplasticity to Promote Pancreatic Inflammation and Cancer Progression.

UNLABELLED: Pancreatic ductal adenocarcinoma (PDAC) co-opts the peripheral nervous system through nerve hypertrophy, axonogenesis, and perineural invasion, and these processes correlate with patient morbidity and mortality. Prior work has shown that autonomic nerves directly modulate neoplastic cells in PDAC, but whether cancer-associated fibroblasts (CAF) participate in neural remodeling is unknown. Using thick tissue sections, we identified dense neo-innervation near myofibroblastic CAFs (myCAF) in preinvasive pancreatic intraepithelial neoplasms. Mechanistically, TGFβ produced during inflam

Soft-tissue perineurioma in a 20-year-old patient with neurofibromatosis type 1 (NF1): report of a case and review of the literature.

BACKGROUND: Perineurioma is a rare benign soft-tissue tumor composed of cells showing differentiation toward the perineurial cells of the nerve sheath. Although mutations in the neurofibromatosis 2 (NF2) gene have been documented in this tumor, there is no known association between perineuriomas and type 1 or 2 NF. METHODS: This is the first report of a case of soft-tissue perineurioma occurring in a patient with NF1. RESULTS: Histopathologic examination revealed a 2.0-cm well-circumscribed, spindle-cell neoplasm with slender, elongated, bipolar, wavy cytoplasmic processes and wavy, elongate

A clinically and genomically annotated nerve sheath tumor biospecimen repository.

Nerve sheath tumors occur as a heterogeneous group of neoplasms in patients with neurofibromatosis type 1 (NF1). The malignant form represents the most common cause of death in people with NF1, and even when benign, these tumors can result in significant disfigurement, neurologic dysfunction, and a range of profound symptoms. Lack of human tissue across the peripheral nerve tumors common in NF1 has been a major limitation in the development of new therapies. To address this unmet need, we have created an annotated collection of patient tumor samples, patient-derived cell lines, and patient-der

Clinical management of a patient with advanced mucosal malignant melanoma in the sinonasal area.

We describe a case of mucosal malignant melanoma in the sinonasal area of a 65-year-old woman. She presented with a history of nasal obstruction and epistaxis with subsequent tenderness, facial anesthesia involving cranial nerve V2, red eye, proptosis, diplopia, and conjunctival chemosis. Computed tomography detected a nonspecific solid mass that had involved the left maxillary sinus and surrounding tissues, with extension into the nasal cavity and invasion of the orbital floor and eye muscles. Histopathologic examination of the neoplasm revealed that it was a malignant melanoma. We performed

A rare benign parotid gland tumor: angiomyolipoma.

Angiomyolipomas are benign neoplasms composed of smooth muscle, vasculature, and mature adipose tissue, which most commonly occur in the kidney and located in the head and neck region. A very rare neoplasm, there are only 3 cases of angiomyolipoma in the parotid gland that have been reported to date. Here, we report a case of a 38-year-old man who had a slow-growing mass in the parotid gland for the past 7 years. The results of a physical examination revealed a rubbery mass that was 2.5 cm in diameter in the below superficial lobe of the left parotid gland. A computed tomographic scan showed a

Spontaneous ganglioneuroma possibly originating from the trigeminal ganglion in a B6C3F1 mouse.

In a carcinogenicity study, a neuronal tumor in the cranial cavity was observed in a 110-week-old female B6C3F1 mouse. At necropsy, the tumor was seen at the site of the pituitary gland. Histologically, the tumor consisted of well-differentiated ganglion cells, nerve fiber/neuropil-like elements and ganglion-like cells. The tumor was composed mainly of ganglion-like cells, which were arranged in solid sheets interspersed with thin fibrovascular stroma. Nissl substance was detected at the margin in the cytoplasm of well-differentiated ganglion cells, and nerve fibers were identified by the Klue

Outcomes of Head and Neck Neurogenic Tumors.

BACKGROUND: Lateral neck mass management frequently challenges surgeons. Nerve tissue neoplasms are an uncommon cause of such nodules. Neurogenic tumors form a tiny percentage of the head and neck neoplastic lesions. Considering the number of nerves in this area, it is surprising that such neoplasms are not more frequently seen. METHODS: A retrospective study was conducted on all patients who presented to the National Cancer Institute of Cairo, Egypt, with head and neck neurogenic neoplasms. RESULTS: During the last 10 years at the National Cancer Institute of Egypt (2006-2015), 40 cases of

Giant neurilemmoma of the vagus nerve: a case report and review of literature.

Cervical vagal neurilemmomas are rare, usually asymptomatic, slow-growing tumours and defined as a benign, encapsulated neoplasm that arises in the nerve fibre. Magnetic resonant imaging (MRI) plays a central role in diagnosing vagal nerve neoplasm and in fact, provides important pre-operative information useful in planning optimal surgical treatment. A rare case of giant neurilemmoma is presented with a large swelling in the right side of the neck associated with breathlessness and paroxysmal cough. X-ray chest revealed large homogenous opacity in apical area of the right lung extending into

Cervical vagal schwannoma: single case report.

Schwannomas of the cervical vagal nerve are rare neoplasms, usually occurring between the third and the sixth decade of life. They don't demonstrate any sex predilection, and they often present as slow-growing, palpable neck masses, leftright sided, without early neurological symptoms, and they are often confused with enlarged lymphnodes or lipomas. Several differential diagnosis should be considered. Imaging techniques are largely used to define their etiology. If they are considered primary to evaluate their relationship with surrounding structures (i.e. omolateral neck vessels and nerves, o

A giant sarcoma of the parotid gland: a case report and review of the literature.

CASE REPORT: Primary low-grade fibromyxoid sarcoma (LGFMS) is an extremely rare tumour of the salivary glands and arises from undifferentiated pluripotential mesenchymal cells. We report a case of a huge (17 x ll x 10 cm) primary LGFMS of the parotid gland of a 57-year-old woman. Clinically, a 12x8 cm firm, non-tender, left parotid mass was observed, facial nerve function was not impaired and no enlarged lymph nodules were palpated on the neck. A contrast axial CT scan showed intense enhancement of the enlarged left parotid gland. The fine needle aspiration biopsy (FNAB) was compatible with sa

Osteosarcoma of the skull base: case report and review of literature.

Osteosarcoma is the most common primary malignancy of bone in children and adolescents. Osteosarcomas are an aggressive neoplasm composed of spindle cells producing osteoid. They primarily affect the long bones, particularly after radiation or chemotherapy for other neoplasms; however, 6-7% present in the head and neck. Primary head and neck osteosarcomas in children are rare. There are few case reports and limited-sized case series in the literature. A case report presentation of a skull base osteosarcoma in a teenage female. A 14-year-old African American female presented with dysphagia, voi

P14.05 How far can they grow... - Two clinical examples

Abstract Introduction: Cerebral metastases are the most common form of central nervous system (CNS) tumours in adults. However, malignant neoplasm may also involve structures external to the brain, such as tissue surrounding the base of the skull, and then metastasize to the brain either by direct invasion or by spreading by the cranial nerves. CASE1: A 74 year-old man, with a past history of chronic kidney disease due to renal artery thrombosis and hypertension, was admitted in the emergency room (ER) complaining of persisting pain in the superior half of the right hemiface and frontal region

Dermal Nerve Sheath Myxomas Harbor Recurrent DPYSL2::CDX1 Fusions and Expand the Spectrum of CDX1-Rearranged Soft Tissue Tumors.

Dermal nerve sheath myxoma (DNSM) is a rare benign mesenchymal tumor with presumed nerve sheath differentiation with distinctive morphologic and immunophenotypic features; however, its molecular pathogenesis remains poorly understood. We investigated a series of nine morphologically classic DNSMs and performed molecular testing to identify recurrent genetic alterations. The cohort included five female and four male patients (median age: 56 years; range: 23-75 years). Tumors arose predominantly on the extremities and presented as multinodular dermal-based myxoid neoplasms composed of uniform sp

An unusual lesion of cerebellopontine angle as a pathological and diagnostic enigma: a comprehensive case report.

Cerebellopontine angle (CPA) tumours comprise a pathologically varied group of tumours owing to the origin of the tumour and effect on neurovascular structures, leading to a myriad of clinical manifestations. These tumours are benign, sluggish, with a low potency for malignant transformation. Malignant peripheral nerve sheath tumours (MPNST) are a malignant neoplasm of the peripheral nerves. It may develop from a pre-existing neurofibroma or may arise sporadically. Malignant peripheral nerve sheath tumours have a poor overall prognosis owing to their aggressive nature and limited therapeutic o
