Trigeminal neuralgia.

Trigeminal neuralgia is a sudden, severe, brief, recurrent, stabbing pain in the distribution of the trigeminal nerve. It is a rare condition whose etiology remains unknown. Diagnosis is by careful history and there are few investigations that are of value in the management of this condition. Management is initially medical with anticonvulsant drugs being the primary drugs. If the drugs are no longer effective or tolerated then surgical management needs to be considered. Surgery can be at a variety of different levels along the pathway of the trigeminal nerve. Each procedure has its advantages…

Trigeminal neuralgia.

Severe trigeminal neuralgia (TN) is probably the most excruciating pain experienced in the orofacial region. Apart from the primary idiopathic form (ITN), this lancinating pain may be secondary to a range of underlying organic diseases (STN). Diagnosis requires differentiation of these and elimination of other causes of orofacial pain.

Trigeminal neuralgia.

Although the full and precise etiology of TN still awaits further elucidation, this extremely painful disorder can be treated successfully by a variety of techniques, pharmacologic and surgical, with minimal side effects. There is no excuse in the 1990's for patients with TN not to be diagnosed promptly and treated expeditiously and successfully.

[Perineal neuralgia].

Ninety cases of chronic perineal pain of neurological origin are reported. Alcock's canal syndrome, consecutive to damage of the pudendal nerve in the ischiorectal fossa, is the most frequent of these neuralgias. It is characterized by burning pain or paraesthesia increased in sitting position and relieved by standing up. The specific treatment is CT-guided infiltrations of the pudendal nerve. Other neurological causes are spinal cord lesions (notably tumours of the conus medullaris), sacral meningoradiculitis (perineal herpes zoster), plexitis and pudendal nerve neuritis. In some cases the re…

Glossopharyngeal neuralgia.

A review is presented of the clinical features of glossopharyngeal neuralgia, as analysed from 304 reported instances. The sex-ratio of the condition is equal, the peak-age at onset is between 40 and 60 yrs, and left-side involvement predominates in females. The clinical features are detailed in 18 specific items. In a review of the aetiology it is argued that the so-called "idiopathic" type of IXth nerve neuralgia (which, upon careful analysis, may still rest upon either an obscure tumor or infection or elongated styloid process) should be primarily attributed to vascular elongation and loopi…
