[Neuromuscular monitoring in a patient with hemiparesis. Resistance of the paralysed musculature to non-depolarising muscle relaxants].

The control of the neuromuscular blockade is part of the standard monitoring in general anaesthesia. However, the degree of blocking is affected by different disorders. We describe the neuromuscular monitoring in four patients suffering from central hemiplegia by stimulation of the ulnar nerve simultaneously on the paretic and the normal side. After application of non-depolarising muscle relaxants (Atracurium, Mivacurium, Rocuronium, Vecuronium) a resistance of the paretic extremity against the relaxant used was shown in all cases. A possible explanation for this observation is the spreading o…

Clinical evaluation of the Datex 221 neuromuscular transmission monitor for recording posttetanic electromyographic responses.

The assessment of deep neuromuscular blockade produced by nondepolarizing neuromuscular blocking agents is not possible with the conventional use of the Datex NMT-221 "relaxograph" (Datex NMT-221 monitor, Datex Instrumentarium, Helsinki, Finland), an otherwise useful electromyographic (EMG) monitoring device. A method whereby the relaxograph can be adapted to quantitatively measure posttetanic responses is described here. In anesthetized adult patients, neuromuscular blockade was monitored simultaneously on both hands with two relaxographs. On one hand, EMG responses of hypothenar muscles to t

Intraoperative neurophysiological monitoring changes during surgical correction of pediatric neuromuscular scoliosis: a single-institution report.

PURPOSE: Intraoperative neurophysiological monitoring (IONM) is a tool that can improve safety during spinal surgery. Patients with neuromuscular scoliosis (NMS) have inherent neural axis or primary muscle pathology that may present challenges to neuromonitoring. An understanding of IONM event rates, patterns, and outcomes would better prepare surgeons to counsel patients and families. METHODS: IONM data from pediatric spine deformity correction procedures in patients with neuromuscular scoliosis at a single quaternary care center between 2012 and 2022 were reviewed to determine cases with mo

A retrospective observational study of neuromuscular monitoring practice in 30,430 cases from six Danish hospitals.

Timely application of objective neuromuscular monitoring can avoid residual neuromuscular blockade. We assessed the frequency of objective neuromuscular monitoring with acceleromyography and the last recorded train-of-four ratio in a cohort of Danish patients. We extracted data from all patients receiving general anaesthesia from November 2014 to November 2016 at six hospitals in the Zealand Region of Denmark. Acceleromyography was available in all operating rooms and data were recorded automatically. The primary outcome measure was acceleromyography use in patients receiving neuromuscular blo…

Tele-monitoring in paediatric and young home-ventilated neuromuscular patients: A multicentre case-control trial.

INTRODUCTION: Tele-monitoring (TM) has proved effective in the home management of adult ventilator-dependent neuromuscular disease (NMD) patients. We aimed to evaluate a 2-year longitudinal multicentre TM trial designed for young ventilated NMD patients in terms of feasibility, home management of exacerbations and caregivers' burden. METHODS: The TM trial protocol included patients' weekly scheduled overnight home-recording of SpO2, heart rate and ventilation and their transmission to each TM centre the following morning. Overnight data were reviewed by non-physicians and calls to families ma

Comparison of TOF scan neuromuscular monitoring on adductor pollicis vs. corrugator supercilii and flexor hallucis brevis.

The TOF-Scan is used for neuromuscular monitoring during general anesthesia. Standard placement involves the adductor pollicis muscle, but alternative sites have been proposed for specific clinical scenarios. This study aimed to compare neuromuscular blockade parameters on the palm vs. eyelid, and the palm vs. toe. This study included cohorts A (n = 14; palm vs. eyelid) and B (n = 20; palm vs. toe). Neuromuscular blockade was induced with mivacurium, and TOF-Scan measurements were recorded until recovery. Parameters were analyzed using paired comparisons, correlations, and Bland-Altman charts.…

Case Report: Monitoring neuromuscular fatigue through jump performance over two seasons in a cerebral palsy sprinter.

INTRODUCTION: World Para Athletics classifies athletes with brain injury, cerebral palsy, and motor coordination impairments into Group Class 31 to 38. Para athletes who can run and jump but are affected by impairments such as athetosis, ataxia, and dystonia, which impact all four limbs and the trunk, are categorized as Class T36. Monitoring training load and performance is essential for guiding training programs and preventing injuries in this population. Vertical jumps are commonly used to assess neuromuscular parameters in athletes with cerebral palsy. In this study, we tracked performance

Facial Nerve Monitoring under Different Levels of Neuromuscular Blockade with Cisatracurium Besilate in Parotid Tumour Surgery.

Источник пометил публикацию как отозванную

BACKGROUND: Anaesthesia can alter neuronal excitability and vascular reactivity and ultimately lead to neurovascular coupling. Precise control of the skeletal muscle relaxant doses is the key in reducing anaesthetic damage. METHODS: A total of 102 patients with the normal functioning preoperative facial nerve who required parotid tumour resection were included in this study. Facial nerve monitoring was conducted intraoperatively. The surgeon stimulated the facial nerve at different myorelaxation intervals at TOF% (T4/T1) and T1% (T1/T0) and recorded the responses and the amplitude of electrom

Comparison of Phonomyography Prototype With Train-of-Four Watch SX for Neuromuscular Monitoring: A Prospective Observational Study.

BACKGROUND: Quantitative neuromuscular monitoring has been advocated to deal with residual neuromuscular block. Phonomyography (PMG) is a technology that captures the acoustic signals yielded by muscle contraction, which is easy to use, anti-interference, and has multiple alternative monitoring sites. Our previous study has developed a PMG prototype for neuromuscular monitoring, and its feasibility was preliminarily verified, but further study on its clinical reliability with different neuromuscular blocking agents (NMBs) is needed. METHODS: This single-center, prospective, observational stud…

Is quantitative neuromuscular monitoring mandatory after administration of the recommended dose of sugammadex? A prospective observational study.

BACKGROUND: Some anaesthetists wonder whether confirming a train-of-four ratio (TOFR) ≥0.9 is necessary when using sugammadex to antagonise neuromuscular blockade (NMB). In this study, we aimed to determine whether using sugammadex at the recommended dose under neuromuscular monitoring (NMM) would ensure complete recovery, even without further NMM. METHODS: This prospective observational study included 51 patients who underwent robot-assisted surgery under general anaesthesia between March and May 2023. At the end of surgery, sugammadex was administered (2 mg/kg for a train-of-four (TOF) coun…

Validation of home portable monitoring for the diagnosis of sleep-disordered breathing in adolescents and adults with neuromuscular disorders.

STUDY OBJECTIVES: Sleep-disordered breathing (SDB) is common in patients with neuromuscular disorders (NMD), developing before chronic hypercapnia appears. Polysomnography (PSG) is the diagnostic gold standard but is often impractical and poorly accessible for individuals with NMD. We sought to determine the diagnostic accuracy, feasibility, and patient preference of home sleep apnea testing (HSAT) compared with PSG for the detection of SDB in NMD. METHODS: Participants with NMD at risk for SDB aged ≥ 13 years underwent HSAT followed by overnight PSG with concomitant laboratory sleep apnea te

Wearable Wireless EMG Sensors for Monitoring Post-Error Neuromuscular Responses During a Sport-Specific Inhibitory Control Task.

Post-error slowing (PES) is commonly considered a behavioral marker of post-error adaptation. However, adaptive processes may also emerge through subtle modifications of motor preparation, particularly in combat sports such as taekwondo (TKD), where maintaining rapid motor execution is essential. This study examined post-error neuromuscular adjustments during a TKD-specific kicking task by comparing standard Go and post-error Go trials for changes in muscle onset latency, peak electromyographic amplitude, and co-contraction indices. Twenty-eight TKD athletes (14 novice and 14 advanced) perform

Neuromuscular monitoring during modified rapid sequence induction: A comparison of TOF-Cuff® and TOF-Scan®.

BACKGROUND: Acceleromyometry is the clinical standard for quantitative neuromuscular monitoring, mostly using the stimulation pattern train-of-four (TOF). TOF-Cuff®, a recently introduced neuromuscular monitor with stimulating electrodes integrated within a blood pressure cuff, assesses the muscular response in the upper arm. METHODS: The time from administration of a neuromuscular blocking agent to TOF-ratio 0% during modified rapid sequence induction was compared between TOF-Cuff® and acceleromyometry (TOF-Scan®). Included were 26 adults with body mass index <35 kg/m2. TOF-Scan® and TOF-Cuf…

Intraoperative electrophysiologic monitoring of ocular motor nerves under conditions of partial neuromuscular blockade during skull base surgery.

The feasibility and usefulness of intraoperative electromyographic monitoring of the oculomotor nerve (CN III), trochlear nerve (CN IV), and abducens nerve (CN IV) were evaluated under conditions of partial neuromuscular blockade in 21 patients undergoing skill base surgery. Intracranial electrical stimulation of each nerve was performed, and compound muscle action potentials (CMAPs) were reconded from the inferior or superior rectus muscle, the superior oblique muscle, and the lateral rectus muscle for monitoring of CN III, IV, and VI, respectively. Partial neuromuscular blockade was achieved

NeuroVisio SyncPatch: A Skin-Conformal Multimodal EMG-Motion Sensing Platform for Quantitative Neuromuscular Rehabilitation and Performance Monitoring.

Effective musculoskeletal rehabilitation requires simultaneous assessment of muscle activation and movement kinematics, yet no current system enables this at home. Laboratory motion capture systems provide accurate kinematics but require controlled environments, while portable inertial sensors enable home use at the cost of drift and incomplete joint angle resolution. Critically, neither approach provides direct physiological insight into the muscle activation driving functional movement. Here, we present NeuroVisio SyncPatch, a wearable multimodal platform that unifies conformal surface elect
