Fatal abuse of nitrous oxide in the workplace.

Abuse of nitrous oxide (N2O) while on the job caused at least 11 deaths in 1984 to 1987, as found in investigations by the Occupational Safety and Health Administration and in reports to the Consumer Product Safety Commission. Although nitrous oxide has had a reputation as a "clean and safe" analgesic and anesthetic, the possibility of asphyxiation by inhalation is not widely recognized, and there are no requirements for warning labels on nonmedical sources of N2O. The 11 deaths involved recreational inhalation of N2O by young male employees from tanks or cylinders normally used for legitimate…

Relationships among nitrous oxide exposure, neurological injury and biomarkers.

AIMS: Recreational nitrous oxide (N2O) misuse is an increasing public health problem associated with functional cobalamin (B12) deficiency and severe neurological complications. Despite growing recognition of its harms, the dose-response relationship and clinical value of biomarkers remain unclear. This study aimed to characterize the demographics, patterns of use, outcomes, and biomarker utility in patients hospitalized with N2O toxicity, as well as to examine the relationship between exposure and toxicity. METHODS: This retrospective cohort study included hospitalized individuals with N2O t…

Acute nitrous oxide-induced neuropathy mimicking Guillain-Barré syndrome.

The early clinical features of nitrous oxide (N2 O)-induced neuropathy were mimicking that of Guillain-Barré syndrome (GBS). We aimed to explore clinical and laboratory characteristics of N2 O-induced neuropathy in comparison with GBS. We retrospectively reviewed data of 15 patients with N2 O-induced neuropathy and compared them with 15 GBS patients. The age of the N2 O-induced neuropathy group was significantly younger than that in the GBS group (22 ± 5 vs 45 ± 17). Paresthesia was more common in N2 O-induced neuropathy group (100% vs 53.3%). The proportion of distal upper limbs weakness was …

Current status of nitrous oxide use in pediatric patients.

Nitrous oxide is one of the most commonly used inhalational anesthetic agents used in practice. It is a cost-effective, pleasant, safe, and versatile anesthetic agent with many desirable properties like good quality analgesia, decreased awareness, accelerated induction and recovery from anesthesia, and reduced utilization of other expensive inhalational agents with potential cost savings. The use of nitrous oxide has been questioned by a lot of studies and case reports perceiving its adverse systemic, hematological, immune, and neurologic adverse effects. However, the literature in the recent …

[Extra hospital use of nitrous oxide for recreational purposes].

Extra hospital use of nitrous oxide. FOR RECREATIONAL PURPOSESThe problematic recreational consumption of nitrous oxide is increasing in the adolescent population and its harms are little publicized and / or trivialized. This therefore represents a public health issue. In high doses, nitrous oxide becomes a dissociative anesthetic. It can lead to hypoxia and even asphyxia causing loss of consciousness. A chronic consumption is addictive and is characterized by megaloblastic anemia, dysfunction of the reproductive system and sensitivomotor myeloponeuropathy. After review of the previously cases…

Encephalitis secondary to nitrous oxide and vitamin B12 deficiency.

A 27-year-old woman presented with confusion, agitation and fever. Having initially been treated as an infective encephalitis case her initial and subsequent lumbar punctures revealed cerebrospinal fluid with a worsening pleocytosis and elevated protein. It was initially felt she had been suffering from tuberculous meningitis and started on treatment it later became apparent that she had a severe vitamin B12 deficiency related to recreational nitrous oxide use. She also was noted to have a peripheral neuropathy. After replacing her vitamin B12 and later stopping her tuberculous medication once…

Use of Nitrous Oxide in Dermatology: A Systematic Review.

BACKGROUND: Many dermatologic procedures are painful and can be distressing to patients. OBJECTIVE: To determine whether nitrous oxide has been used in dermatology and whether literature supports its use in terms of providing analgesia and anxiety associated with dermatologic procedures. METHODS: A search of PubMed and Cochrane databases was conducted through July 15, 2016, to identify studies involving nitrous oxide use in dermatology. RESULTS: Eight studies were identified and reviewed. The use of nitrous oxide/oxygen mixture resulted in a significant reduction in pain when used for photo…

Nitrous oxide and spontaneous abortion in female dental assistants.

The relation between anesthetic gas exposure and spontaneous abortion remains unresolved. We examined the effect of nitrous oxide on spontaneous abortion among female dental assistants. Questionnaires were sent to 7,000 dental assistants aged 18-39 years who were registered in California in 1987; 4,856 (69%) responded. Analysis was based on 1,465 respondents whose most recent pregnancy was conceived while working full time. Women were asked how many hours a week they worked with nitrous oxide during this pregnancy and whether the excess gas was scavenged (vented). Relative risk of spontaneous …

Vitamin B12 supplements as protection against nitrous oxide inhalation.

Chronic exposure to ambient nitrous oxide levels found in dental offices may pose unrecognized health risks, among them, increased incidence of spontaneous abortion or infertility among female workers. Vitamin B12 supplements can reverse some of the effects of chronic N2O exposure.

Nitrous oxide antagonizes CNS stimulation by laudanosine in mice.

We have investigated whether nitrous oxide antagonizes or augments the CNS stimulant action of laudanosine in mice by comparing the mean convulsive doses (CD50 (SE] of a control group and those following pretreatment with 65% nitrous oxide in oxygen for 20 and 180 min. Nitrous oxide significantly increased CD50 from 46.8 (1.4) mg kg-1 of control to 57.3 (1.3) mg kg-1 at 20 min and 53.5 (1.7) mg kg-1 at 180 min. The attenuation of the effect of nitrous oxide at 180 min, suggestive of possible partial drug tolerance, was not statistically significant. These findings indicate that nitrous oxide a…

Analgesic nitrous oxide in neuropsychiatry: past, present and future.

We highlight the distinction between analgesic and anesthetic concentrations of nitrous oxide, with special reference to the safety of analgesic nitrous oxide. We present evidence that the gas at analgesic concentrations is an opioid agonist. Its extremely low abuse potential, despite its opioid properties, is discussed with regard to its evanescent action and possible partial agonistic effects, making it the least addictive of all mind-altering addictive substances. The activities of analgesic nitrous oxide make it an almost ideal agent with which to investigate the functions of the opioid sy…

Hypoxaemia after inhalation of 50% nitrous oxide and oxygen.

We have investigated, in volunteers, the effect of inhaling 50% nitrous oxide in oxygen on subsequent oxygenation. Subjects breathed either 50% nitrous oxide in oxygen or 50% nitrogen in oxygen during hyperventilation and normal ventilation. Arterial oxygen saturation and end-expired oxygen concentrations were lower after 50% nitrous oxide in oxygen than after the other mixture with both patterns of ventilation and were lowest after hyperventilation with 50% nitrous oxide in oxygen.
