Osteomyelitis is an unusual manifestation of infection with Nocardia spp, and most cases result from direct extension from a primary lung or cutaneous lesion. The authors report what they believe to be the first case of Nocardia vertebral osteomyelitis occurring postoperatively, and review the literature on the 19 previously reported cases of Nocardia osteomyelitis due to hematogenous dissemination or direct inoculation, excluding those occurring from direct extension of a mycetoma or primary lung infection.

Nocardia brasiliensis skin infections were diagnosed in three patients (two with mycetoma and one with lymphocutaneous syndrome) in our hospital during the last 6 years. The mycetomas were detected in otherwise healthy Yemenite immigrants who had acquired their infection while in Yemen. The patient with the lymphocutaneous syndrome--a man with well-controlled diabetes mellitus--acquired his illness in Israel. Nocardia gypsoides, a species characterised by its ability to produce calcium sulphate granules, has been isolated for the first time from a corneal ulcer. In vivo this pseudomycete also forms gypsum. Every effort should be made to identify Actinomycetales to species level. The names Leptothrix and Streptothrix should be abandoned.

Disseminated Nocardia caviae infection occurs only rarely. To date, two cases have been recorded in the literature. We describe a third case of systemic N. caviae infection in a compromised host and compare it with the first two cases. Postmortem, microbiologic and antibiotic sensitivity data are included.

We report a 40-year-old man with advanced Hodgkin disease who received combination chemotherapy and developed disseminated Nocardia brasiliensis infection. We discuss opportunistic infections in the compromised host, with particular reference to nocardial disease and certain biologic characteristics of that organism.

The authors report a case of pulmonary nocardiosis due to Nocardia nova occurring in a smoker who had a renal transplant. A pseudo-tumour was found on a standard X-ray and computerised tomography did not suggest the diagnosis. Needle aspiration under computerised tomographic control enabled a diagnosis to be made.

A recent study of Nocardia asteroides revealed that 95% of clinical strains had one of five antibiotic resistance patterns. We found the pattern of resistance to cefotaxime and cefamandole in 19% of 200 clinical N. asteroides isolates. Isolates with this drug resistance pattern were from numerous geographic sources and were associated with significant clinical disease (56% of patients had disseminated infections). We describe here the isolation and characterization of the bacteriophage, NTR1 from activated sludge. This phage is lytic for Nocardia transvalensis, Nocardia brasiliensis and Nocardia farcinica. NTR1 phage has a genome sequence of 65,275 bp in length, and its closest match is to the Skermania piniformis phage SPI1 sharing over 36% of its genome. Two actinomycete strains isolated from two patients with lung nocardiosis between 1995 and 1997 in Japan were assigned to novel species of the genus Nocardia based on morphological and chemical criteria. Two bacterial isolates from the sputa of a patient with a pulmonary infection were subjected to a polyphasic taxonomic study. Chemotaxonomic investigations revealed the presence of cell-wall chemotype IV and mycolic acids consistent with the profile for the genus Nocardia.
