An echovirus 11 outbreak occurred among neonates in an obstetric clinic in November 2003. Thirteen neonates were transferred to our medical center, and all were found to have echovirus 11 infection. Viral studies were performed for 32 other infants born in the clinic during the same period, including 30 asymptomatic neonates and 2 febrile infants transferred to another hospital. Two of the asymptomatic infants had echovirus 11 isolated from rectal swabs. The first patient transferred to our medical center developed extensive hemorrhage and died 6 days later. Three family members of this infant

Over a five-month period eight breastfed infants required readmission to Wesley Long Community Hospital in Greensboro, North Carolina, within 48 hours of discharge from the newborn nursery. The primary diagnoses upon readmission included hyperbilirubinemia and suspected sepsis. At discharge from the intensive care nursery, mild dehydration secondary to inadequate breastfeeding was deemed a significant factor in precipitating the need for readmission. This increase in readmissions represented a significant change from the past year. An intensive review of each initial hospital stay was conducte

INTRODUCTION: Remote postpartum self-measured blood pressure (SMBP) monitoring programs improve rates of recommended BP ascertainment among individuals with hypertensive disorders of pregnancy (HDP) overall. Healthcare utilization patterns differ among individuals whose infants are admitted to the neonatal intensive care unit (NICU) and those with infants in the standard newborn nursery, but it is unclear whether this difference translates into a reduction of SMBP program engagement.
METHODS: Our SMBP program is offered to all postpartum individuals with HDP at our hospital. Participants were

BACKGROUND: We report four cases of cutaneous infection with Rhizopus microsporus in vulnerable preterm infants in one neonatal nursery.
METHODS: There was no overlap in hospital stay between the first two cases and an extensive programme of environmental sampling was therefore initiated, with special attention being paid to items that had close contact with infants' skin. Samples were cultured by standard techniques, including fungal culture with Sabouraud's medium. Filamentous fungi with non-septate hyphae were presumptively identified as agents of mucormycosis and referred to the Mycology

For decades, the diagnosis of neonatal abstinence syndrome (NAS) was relatively rare in the US; however, the recent epidemic of prescription and nonprescription opioid use across the country has made this once novel diagnosis common. The number of infants diagnosed with NAS in the US has grown nearly 5-fold since 2000. Today, one infant with NAS is born every 25 minutes, accounting for an estimated $1.5 billion in hospital charges. The rapid rise of NAS in our nurseries and neonatal intensive care units demands that improvements in research and care delivery systems keep pace.

Some newborn nurseries located near sea level are utilizing the Denver (high altitude) fetal growth curve to identify and screen infants at high risk for hypoglycemia and/or polycythemia: small for gestational age (SGA) and large for gestational age (LGA) infants. It has been shown that this curve may underestimate the number of SGA infants. Neonates at high risk for these conditions may therefore go undetected and unscreened. To investigate this possibility, we evaluated the risk for hypoglycemia and polycythemia in 354 newborn infants, comparing a high altitude with a sea level fetal growth

Three cases of generalized exfoliative staphylococcal scalded skin syndrome (SSSS) were encountered during a 25-day period in a nursery for premature infants. A single strain of Staphylococcus aureus, phage type 42E/54/75 (group 3), was recovered from each of the three infants and from no others. Two of the three isolates were tested and both produced epidermolytic toxin (ET). Most cases of SSSS have been associated with phage group 2 strains. To our knowledge, this is the first report of a nursery outbreak caused by a phage group 3 staphylococcus. It illustrates that ET production and not pha

The group morbidity with cryptosporidiosis (13 cases) was recorded in infants from a somatic hospital and a nursery school and also among unorganized groups. The factor of transmission was kefir prepared in the nursery milk kitchen. Cryptosporidian oocysts were not found in the personnel of the kitchen. It was supposed that the contamination of milk by oocysts occurred on milk farm. Single oocysts were found in milk sediments on the filter taken from a tank at a milk factory.

Providing safe and effective care requires coordination among the multiple levels of the health care system. These levels comprise the newborn (patient, family and community), nursery or primary care practice (microsystem), hospital or managed care organization (macro-organization) and policy, payment or regulatory issues (environmental context). Contemporary care practices associated with childbirth and early newborn care often reflect disruptions in coordination of these processes and place newborns at risk for poor outcomes. For example, with routine early postpartum discharge, often at les

In a prospective randomized study different regimens for skin and umbilical disinfection in newborn infants were tested: daily whole body soap wash (control group), daily whole body soap wash and umbilical cleansing with (i) benzine solution, or (ii) 0.05% chlorhexidine, and daily whole body wash and umbilical cleansing with a 4% chlorhexidine detergent solution (Hibiscrub). Bacterial cultures were taken from the nose and umbilical area at discharge. Clinical infections were registered in the nursery, and after discharge until 6 weeks of age. Cultures were taken from infected areas. In the con

The follow-up results of intensive care for 68 infants with birth weights less than 801 g treated at Stanford University Hospital were reviewed. The overall survival rate for these infants was 35%, but was 50% for those infants who had been successfully resuscitated in the delivery room and were admitted to the Intensive Care Nursery. Infants under 601 g in weight or less than 25 weeks gestation were more likely to die in the delivery room, but survival among those admitted to the Intensive Care Nursery did not depend on birth weight or gestational age. One-minute and 5-minute Apgar scores les

We reviewed the maternal transfers for one year (1980) to the Emory Perinatal Center, a tertiary care center for North Georgia. Of 445 telephone consultations or patient referrals, 150 women gave birth at the tertiary center. Indications for maternal transfer were predominantly based on the needs or anticipated needs of the fetus. There were a total of 163 births, 10 of them stillborn and 153 live-born. Three infants expired in the delivery room. Sixty infants (40%) required 580 days of intensive care. A total of 1,439 days of care in the intermediate unit was required and 1,077 days of care i

BACKGROUND: Late preterm infants are infants born between 34 and 36 6/7 weeks gestation. Compared to term infants, late preterm infants are at increased risk for breastfeeding difficulties, hypoglycemia, hyperbilirubinemia, and hypothermia due to their relative physiologic and metabolic immaturity.
PROBLEM: Medical record reviews performed at a level III maternal and newborn hospital in central Illinois revealed only 64% of late preterm infants admitted to the newborn nursery received care per the unit late preterm infant policy. The aim of this quality improvement project was to increase nur

Newborn infants are thought to be particularly susceptible to infection with Mycobacterium tuberculosis, and the possibility of exposure to unrecognized clinical tuberculosis in nursery and hospital personnel is ever present. To reduce this possibility, periodic skin testing of hospital personnel is often carried out, and tuberculin-positive personnel are given preventive treatment with isoniazid (INH). Also, environmental controls, such as ultraviolet light and periodic air exchange, are used. The efficacy of these measures has not been fully established. A nursery supervisor with smear- and

BACKGROUND: Postaxial type B polydactyly is characterized by the presence of a rudimentary extra digit on the small finger side attached by a soft tissue stalk. The recommended treatment is ligation in the neonatal nursery. The purpose of this study was to evaluate the result of excision of the supernumerary digit under topical anesthesia.
METHODS: The study group included 11 consecutive infants with 15 supernumerary digits on the hand. To ensure complete excision, the digit was retracted by the surgeon and excised at the base of the skin triangle formed on retraction beyond the proximal end
