Idiopathic bilateral optic neuritis in adult has been reported very rarely. The objective of this report is to present a case of idiopathic bilateral optic neuritis in adult and treatment responses. A nineteen year old female presented with bilateral optic neuritis. It was characterized by decreased visual acuity, painful ocular motility and sluggish pupillary reaction with Relative Afferent Pupillary Defect (RAPD) in left eye, hyperemic and generalized optic disc swelling and central scotoma in Humphrey visual field of both eyes. MRI showed diffuse thickening and irregularly outlined optic ne…

Optic neuritis is an inflammatory condition of the optic nerve characterized by a sudden onset of unilateral visual loss, usually affecting young females. Demyelination associated with multiple sclerosis (MS) is the most common cause in regions where MS is prevalent; while in other places, there are a substantial proportion of cases where infective or autoimmune causes are seen. Optic Neuritis Treatment Trial (ONTT) was the first major study that provided information on the natural history, role of steroids in treatment and risk of development of MS. Subsequently, numerous clinical trials have…

A case of progressive optic neuropathy in a woman with a history of breast cancer is presented. Differential diagnoses including optic neuritis, infiltrative optic neuropathy, carcinomatous meningitis, and toxic optic neuropathies are discussed. Risk factors for metastatic brain lesions are also discussed.

Optic neuritis can be mimicked by other optic neuropathies and by anterior segment, choroidal or retinal diseases. Retinal diseases may cause central visual loss when they involve the macular or peripapillary area. Central serous retinopathy, hereditary retinal diseases, white dots syndromes and autoimmune retinopathies may present a clinical picture similar to that of optic neuritis. Complete neuro-ophthalmological examination must be performed to differentiate optic nerve damage from other ocular structures involvement. As in optic neuritis, optic disc and macula appearance may be completely…

The Optic Neuritis Treatment Trial continues to generate information and controversy on the visual and neurologic outcome and treatment of optic neuritis. At the same time, other researchers explored cerebrospinal fluid parameters in multiple sclerosis, treatment of experimental optic neuritis, corticosteroid treatment of multiple sclerosis, and variations and mimickers of optic neuritis.

Study of 40 children who presented a first episode of idiopathic optic neuritis. A mean follow-up of more than 8 years revealed only 15% of multiple sclerosis. This frequency is much lower than in adults. The visual outcome is also better. Significant factors are: absence of papilloedema, unilaterality, recurrence, HLA B7 and DR2, oligoclonal cerebrospinal fluid, anomalies of the VEPs.

Three patients, all women, ranging in age from 26 to 44 years, had acute retrobulbar neuritis and laboratory evidence of a collagen vascular disorder. These patients were treated with intravenously administered "pulse" methylprednisolone, and visual acuity recovered in three of four involved eyes. Continued oral administration of prednisone and other immunosuppressive drugs was necessary to maintain vision.

Two patients experienced acute unilateral visual loss as a manifestation of early infectious (secondary) neurosyphilis. In both cases, optic disc swelling was preceded by the development of a characteristic rash accompanied by mild signs of meningeal inflammation. Other signs of ocular inflammation, such as uveitis or retinal vasculitis, were absent. Optic neuritis may occur as a manifestation of the meningeal inflammation that can accompany secondary syphilis. In such patients, it is important to confirm involvement of the CNS and to institute appropriate antibiotic therapy.
