IMA II brace for deambulatory treatment of Legg-Perthes disease.

Orthotic devices usually prescribed for ambulatory treatment of Legg-Perthes disease do not meet the severe kinematic requirements necessary for a comfortable gait. This paper describes the IMA II brace which offers several advantages in comparison with conventional braces. It is shown that improved kinematic characteristics are obtained by the use of six joints placed symmetrically at two levels of the brace.

Evaluating the effectiveness of an active strap for wearable robot: A Mechanical and Physiological Study.

Straps are the most commonly used elements in the wearable robotic field, where it is employed as a physical human robot interface (pHRI) between the wearable robot and wearer's body. The main role of the strap is to transmit power from the robot to the wearer by pressing and attaching onto specific body parts to reduce the relative motion. To maximize the efficiency and performance using the appropriate function, a high stiffness and tension is required in the strap. However, the continuous force applied by the strap on the wearer's body is one of the main factors that reduces the wearing com

Bracing: Upper and Lower Limb Orthoses.

The goals of bracing in polio and postpolio are to optimize joint position and support weak muscles with the goal of reducing falls, reducing deformities, and optimizing energy conservation. Orthoses are primarily used in the lower extremities to optimize gait. Less frequently, upper extremity orthoses are required. Polio survivors are at increased risk of falls and injuries. Appropriate bracing and compliance with the prescribed device can prevent falls and injuries. The best orthotic results are often achieved with patients who have significant deficits but walk regularly, are well motivated

Orthoses for neurological ankles.

Patients with weakness or abnormal posture of their lower leg may benefit greatly from appropriate orthoses. This paper describes the sorts of problems that can be helped in neurological practice and the range of devices commonly used, and also highlights some of the factors influencing selection. With greater understanding of their use, clinicians will feel more confident about referring patients for early orthotic assessment.

Challenges in Offloading Lesser Toe Deformities: A Case Study.

BACKGROUND: Lesser toe deformities such as hammer, mallet, claw, or cross over toe deformities are common in the elderly. They are attributed to mechanical, neuromuscular, congenital, inflammatory arthritis, or other disorders. They become clinically relevant when they produce pressure and friction at the distal ends of the digits leading to preulcerative callus or ulcer formation. These deformities can be difficult to offload with custom orthotics, accommodative footwear, and off-the-shelf devices such as crest pads and toe crutches. CASE: Mr. D was an 82-year-old man with bilateral lesser t

Numerical modelling of an orthopedic brace with increased functional characteristics for the treatment of idiopathic scoliosis.

BACKGROUND: Orthotic braces play a key role in the correction of spinal deformities. The effectiveness of these devices depends on the design and distribution of corrective forces transmitted through the corset shell. OBJECTIVE: The present study aimed to reduce the weight of the orthosis and improve its functionality while maintaining its corrective function. METHODS: The distribution of corrective forces transmitted by the orthosis was evaluated using the finite element method (FEM). Areas of the orthosis, which had minimal impact on the overall stiffness, were identified and material from

Adolescent idiopathic scoliosis: is the search for aetiology constrained by the orthosis?

While the aetiology of adolescent idiopathic scoliosis is unknown, it is widely believed that the deformity can be controlled by mechanical forces applied by a corset and acting, via skin and soft tissues, to exert pressure on vertebral growth-plates (Hueter-Volkmann Law). The reported efficacy of bracing in reducing the incidence of small degrees of progression reinforces this concept. The supposed mode of action conditioned the model of pathomechanism and precludes other hypotheses. Since the critical Cobb angle changes are usually less than the probable measurement error, another criterion

[When and why functional orthotics?].

Human walking. It's so simple that we almost forget that it works. It's only when the locomotion give pain that we take care about our feet, ankle, knee, hip or back. First of all, the podiatrist must perform a biomechanical examination of the foot and lower extremities. Angular measurements are taken of all the lower joints to determine the range of motion and, therefore, the level of any structural or functional deformities. The podiatrist will also do a walking and/or running gait analysis of the patient to determine how their foot and lower extremities functions. Understanding the biomecha
