BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a systemic condition characterised by ossification of the ligaments around the spinal column due to unknown aetiology. This is often asymptomatic, but it can cause dysphagia in rare cases.
CASE DESCRIPTION: A 64-year-old man was referred to the ENT department with progressive dysphagia symptoms he had had for years. Extensive analysis by the ENT specialist did not reveal any abnormalities of the swallowing motor functions. However, osteophytes of the cervical spine of the patient were visible in the X-ray of the cervical spine. Th…

OBJECTIVE: The aim of this study was to ascertain the proliferative and probably reparative potentials of the periarticular osteophytes by evaluating the sites of expression of c-myc, c-jun and c-fos oncogenes in this neoplastic repair tissue.
MATERIALS AND METHODS: Sections of osteophytes were obtained from knees of patients undergoing total knee arthroplasty for osteoarthritis. Decalcified sections of osteophytes were stained for c-myc and c-jun oncogenes using the avidin HRP technique. Sections of breast carcinoma were used as positive controls. Undecalcified or frozen sections of osteophy…

We studied the appearance and frequency of the medial epiarticular osteophyte in human femoral heads. Specimens were sampled from 24 men and 31 women who underwent total hip arthroplasty. On radiography, medial epiarticular osteophytes were present in two thirds of the cases. However, histological examination revealed endochondral ossification in all. Presumably, the bone formation was preceded by the formation of canals and cavities containing vessels. Due to the ossification around these canals and cavities, the histological picture frequently featured two layers of cartilage with bone in th…

BACKGROUND: Vertebral osteophytes are one of the principal radiographic diagnostic criteria for degenerative change in the lumbar spine. These osteophytes have been previously classified by morphologic features into two groups, the "traction spur" and the "claw spur." It has been stated that the traction spur is a sign of spinal instability.
METHODS: Lumbar spines from 20 cadavers provided 120 vertebrae from T-11 to L-5 and 240 vertebral rims for study. The presence of osteophytes was determined by measurement with digital calipers. The type of osteophyte was then determined visually, using t…

A review of the literature and four additional cases of surgical excision of anterior cervical osteophytes causing dysphagia is presented. Surgical excision of the osteophytes is recommended only after a complete evaluation to rule out other causes of dysphagia and after an adequate period of conservative therapy. The patients must be advised that frequently symptoms will recur to some extent as time progresses. In patients who have findings consistent with diffuse idiopathic skeletal hyperostosis (DISH Syndrome) recurrence of the osteophyte appears to be more common.

BACKGROUND: Cervical spondylosis frequently leads to osteophyte formation to stabilize degenerative spinal segments. While osteophytes presenting with cervical myelopathy typically arise from the vertebral body, a cervical facet joint osteophyte causing spinal cord compression has not been described.
OBSERVATIONS: This report details the case of a 57-year-old right-handed male waiter with progressive myelopathy. Imaging confirmed a 2-cm calcified extradural mass at the C5 level causing significant spinal cord compression. Surgery revealed a giant osteophyte originating from the right C5-6 fac…

Osteophytes are pointed or beaked osseous outgrowths at the margins of articular surfaces that are often associated with degenerative changes of articular cartilage. They are the most common aspect of osteoarthritis and they infrequently cause symptoms by compression of the adjacent anatomic structures, such as nerves, vessels, bronchi, and esophagus. We present here a rare case of a patient with a left atrial deformation by a large osteophyte.

The interdisciplinary combined project ANCYLOSS is investigating in six subprojects molecular factors which play an essential role in the formation of osteophytes. These bony projections are designed to counteract bone degeneration but, however, in certain rheumatic diseases lead to an excessive reaction with uncontrolled bone spur formation. It could be shown that low levels of Dickkopf-1 protein, which is stimulated by TNF-α, promote the formation of bone spurs.

OBJECTIVES: Osteophytes represent areas of new cartilage and bone formation in human and experimentally induced osteoarthritis (OA). The present study addressed the production of nitric oxide (NO), vascular endothelial growth factor (VEGF) and the occurrence of apoptosis during osteophyte formation.
DESIGN: Osteophytes in the knee joint of rabbits that developed OA-like lesions following anterior cruciate ligament transection (ACLT) were analysed by histology and immunohistochemistry for NO production, and the presence of VEGF. TUNEL was used to detect DNA fragmentation.
RESULTS: At the join…

STUDY DESIGN: A case report is described.
OBJECTIVE: To highlight an unusual cause of thoracic myelopathy.
METHODS: Clinical evaluation of 63-year-old male revealed myelopathy. Thoracic cord compression from a solitary projection of a facetal joint osteophyte at the T9-T10 level was documented on magnetic resonance imaging scans and computed tomography scans.
RESULTS: The osteophyte was successfully excised.
CONCLUSIONS: Thoracic cord compression can be caused by various space-occupying lesions, and a high index of suspicion will lead to diagnosis before neurologic deficit is clinically ex…

The purpose of this study was to determine the morphologic characteristics of the glenoid osteophyte and its relationship to rotator cuff tears. Eighty-six cadaveric shoulders were studied. After inspection of cuff pathology, the glenoid was examined with soft x-rays. Histologically, degenerative changes of the articular cartilage and the labrum were assessed. There were 30 shoulders with rotator cuff tears (35%). Two types of glenoid osteophyte were identified: angular and hooked. Both osteophytes were commonly observed in the anterior to inferior portions of the glenoid. The length of hooked…

Osteophytes associated with spondylosis have been implicated as a cause of multiple extraspinal manifestations. Symptoms are more likely to occur with the large osteophytes associated with diffuse idiopathic skeletal hyperostosis. In the thoracic region, osteophytes have been reported infrequently as a cause of extraspinal complications. We report a case in which an anterior thoracic vertebral osteophyte was responsible for chronic obstructive pneumonia due to obstruction of the right main stem bronchus. The patient's condition improved considerably after surgical resection of the compressing …

Cervical spondylosis and ankylosing hyperostosis of the cervical vertebrae are commonly asymptomatic. Dysphagia caused by cervical osteophyte formation is rare. We report a case of spondylotic dysphagia with striking radiographic findings. A massive anterior cervical hyperostosis was resected via the anterior cervical approach with excellent relief of dysphagia.
