[Suppurative chronic otitis media: etiology, diagnosis and therapy].

The chronic otitis media is defined as a permanent perforation of the drum membrane, which does not close by itself, and an inflammatory reaction in the mucosa (mucositis) of the middle ear. Two main forms of the chronic otitis media are distinct: the suppurative otitis media and the cholesteatoma. The suppurative otitis media is often accompanied by secretion into the external ear canal (otorrhoe), but "dry ears" are also common. Other frequent, but not obligatory symptoms are hearing impairment, tinnitus, and aural pain or pressure. Although genetically determined microbial and immunological

[An update on the treatment of vestibular schwannoma].

The increase in the diagnosis of ever smaller vestibular schwannomas (VS), the fact that many tumours can be observed with serial MRI, and the development of radiosurgery as an alternative to microsurgery have led the neurotologic surgeon to a new global approach to patients with VS. On the other hand, the spread of internet-based information sources, often with biased or incomplete information, makes counselling patients with VS a challenging task. This study provides an overview of the natural history of these tumours and the main therapeutic options: observation, surgery and radiosurgery, w

Limits in endoscopic ear surgery.

BACKGROUND: In recent decades, endoscopic ear surgery (EES) has been rapidly evolving, expanding its boundaries from the middle ear to the lateral skull base. Nonetheless, the advantages of the endoscopic technique are associated with a number of intrinsic limitations. METHODS AND OBJECTIVE: A narrative review was conducted to investigate the current limits of EES, analyzing the different otologic and skull base surgery procedures. RESULTS: Limitations of EES can be divided into general and procedure-related. General limitations have been extensively described in the literature and are relat

Endoscopic annular chondroperichondrial tympanoplasty, technical description.

PURPOSE: Tympanoplasty is a surgical procedure performed to cure middle ear infections and restore normal middle ear function. It is one of the most common procedures in otological surgery. Since Wullstein described tympanoplasty, the microscope has been a widely used surgical tool in otological surgery. Transcanal endoscopic ear surgery (TEES) has come to the forefront as an alternative option due to the various limitations of microscopic surgery and the development of otological surgical techniques. When reviewing the types and tecniques of grafts used in tympanoplasty in the current literat

Modified bilateral fasciaperichondrial flap for prominent ear correction.

OBJECTIVE: Various techniques have been described in the literature for prominent ear correction. These cartilage-preserving or cartilage-shaping techniques have their own advantages and disadvantages. We aim to achieve aesthetic and stable results with low complication rates using combinations of these methods. Herein, we present our results of prominent ear surgery with a modified bilateral fasciaperichondrial flap in combination with concha-mastoid and concha-scaphal sutures. METHODS: Patients whose surgeries included a modified bilateral fasciaperichondrial flap for prominent ear deformit

Endoscope-assisted ear surgery.

Endoscopes deliver the otologic surgeon's view into the temporal bone to provide rapid access to its intricate recesses. The attic, sinus tympani, facial recess, hypotympanum, internal auditory canal, and other recesses are immediately visualized with angled endoscopes, using minimal exposure without the time-consuming removal of overlying bone. This report presents results of 32 otologic and neurotologic operations, performed since 1993, that incorporated endoscope-guided dissection as a principal part of the procedure to reduce incision size, exposure width, and operative time, or to access

Endolymphatic sac surgery for Ménière's disease: a systematic review and meta-analysis.

OBJECTIVE: To analyze current endolymphatic surgery techniques and quantify their efficacy in controlling vertigo and maintaining hearing in the short and long term. DATA SOURCES: A comprehensive literature search using the PubMed-NCBI database from 1970 to 2013. STUDY SELECTION: Articles on sac decompression and mastoid shunt (with and without silastic) were included. Included studies had to report data using the 1985 or 1995 American Academy of Otolaryngology-Head and Neck Foundation (AAO-HNS) guidelines, describe surgical technique in detail, include a minimum of 10 patients, and have min

Congenital Microtia Treated at Sapporo Medical University Hospital: Clinical Characteristics and Associated Anomalies.

Seventy-three children who underwent plastic surgery for their external ear malformations between December 2010 and May 2013 at the Sapporo Medical University Hospital were evaluated. The predominance of right-sided malformation (62%) in males (71%) was similar to that reported previously, but the number of bilateral cases (9.3%) was lower than that in previous reports. We classified patients' ears using Marx's classification. The pure-tone average (average air conduction thresholds measured at 500, 1,000 and 2,000 Hz) was used as the representative value for the hearing level, which was compa

What I think of sac surgery in 1989.

The senior author has performed shunt procedures routinely for medically refractory cases of Meniere's disease starting in 1964 and ending in 1984. Five hundred and sixty-eight consecutive patients were operated on and followed from 1970 to 1984. Combining the cases of C. G. Jackson, there have been a total of 676 procedures done at the Otology Group. Initially endolymphatic-subarachnoid (ES) shunts were used, then endolymphatic-mastoid shunts (EM), and most recently, the Denver valve has been employed. No significant difference between these procedures was noted in the control of vertigo or s

[The high bulb of the internal jugular vein in otosurgery: a literature review].

Knowledge of the anatomical features of the structures of the temporal bone is important in the practice of an otosurgeon. Nevertheless, information on the high bulb of the internal jugular vein is extremely scarce in the available medical literature. The search and analysis of modern scientific publications from publicly available information resources, including review articles, original research, monographs, observations of clinical cases, and textbooks from domestic and foreign authors, has been carried out. The features of clinical manifestations and differential diagnosis with high stand

[Endoscopic ear surgery -- Discipline establishment and technological innovation].

The application of microscope is a milestone in the history of otosurgery, which makes otologists deal with middle ear lesions more clearly and finely, and helps otologists expand the scope of treatment to the lateral skull base area, which greatly promotes the development of otosurgery. In the past 20 years, with the continuous improvement of endoscopic equipment research and development and the gradual improvement of endoscopic technology, the application of endoscopic technology in China has shown an explosive development, and Chinese otolaryngologists have experienced from the initial atte
