Outpatient dacryocystorhinostomy.

We report 105 dacryocystorhinostomy procedures in 87 patients performed on an outpatient basis. Seventy-six patients received local, and 29 received general, anesthesia. The patients left the hospital an average of 2.6 hours after surgery, but 14 were admitted after surgery--9 immediately, and 5 after some delay, primarily for epistaxis. There were no serious complications related to the outpatient nature of the surgery. The success rate was 94%. We conclude that outpatient dacryocystorhinostomy is successful, well accepted by patients, and safe, provided that inpatient care facilities are rea

Outpatient orchiopexy.

Outpatient surgery in children has been recommended as a means to help contain the increasing cost of health care. In addition, the shorter the child's hospitalization the less psychologic trauma. This study examines the feasibility of performing outpatient orchiopexy based on preoperative evaluation, surgical success rate and cost-effectiveness. The records of 459 inpatient and 77 outpatient orchiopexies were reviewed with regard to patient age, medical history (including previous inguinal surgery), preoperative palpability and intraoperative location, anesthetic technique, complications, dur

Skin Infections and Outpatient Burn Management: Outpatient Burn Management.

Burns, whether caused by thermal, chemical, or electrical exposure, are common and often preventable. Burn injuries are most common in children. All patients with burns should undergo primary and secondary assessment, including assessment of airway, breathing, and circulation. Evaluation of the location, size, and depth of burns can help to determine the optimal setting for management. Patients with full-thickness burns, circumferential burns, or burns on the face, hands, feet, genitals, or perineum should be referred to a burn subspecialist. Minor acute pain can be managed with irrigation of

Age-neutrality of the SCID-II: Differential Item Functioning in Younger Outpatients (Aged 20-45) and Older Outpatients (Aged 65+).

OBJECTIVES: Measurements are often developed for the assessment of personality disorders (PDs) in younger adults and seldom evaluated on the applicability in older adults. Remarkably, research has not yet been conducted into age-group appropriateness of the gold standard for the assessment of PDs, known as Structured Clinical Interview for DSM-IV Personality Disorders (SCID-II). Therefore, the current study empirically investigated the age-neutrality of the PDs assessed by the SCID-II. METHODS: Age-neutrality was examined in 84 younger adults (aged 20-45 years) and 68 older adults (aged 65-85

Assessing suitability for short-term cognitive-behavioral therapy in psychiatric outpatients with psychosis: a comparison with depressed and anxious outpatients.

OBJECTIVE: The Suitability for Short-Term Cognitive Therapy (SSCT) rating procedure has predicted outcome in depressed and anxious patients. This study examines its relevance in assessing patients with psychosis. METHOD: Outpatients with psychosis (n=56), depression (n=93), and anxiety (n=264) received cognitive- behavioral therapy in a university hospital teaching unit (mean number of sessions=16, SD=11). Demographic, clinical, and suitability variables were assessed as potential predictors of dropout and success as measured by the Reliable Change Index. RESULTS: Despite lower suitability s…

Tablet Acceptability in Older Outpatients Undergoing Cancer Chemotherapy.

Background/Objectives: Patient acceptability of oral anticancer drugs is a critical factor that influences treatment in older outpatients receiving cancer chemotherapy and plays a central role in enhancing adherence and treatment effectiveness. Identifying older outpatients receiving cancer chemotherapy who exhibit poor tablet acceptability before initiating oral anticancer therapy and offering alternative treatment options are beneficial. Therefore, we investigated the characteristics of patients with poor tablet acceptability by focusing on the tablet size, geriatric assessment, and polyphar…

Health literacy levels in outpatients with liver cirrhosis.

OBJECTIVE: Health literacy (HL) is a concept covering a range of cognitive and social skills that comprises aspects necessary for patients to navigate in the healthcare system. Our study aimed to investigate HL in patients with liver cirrhosis and determine factors associated with low HL. METHODS: Data were collected among outpatients with cirrhosis (n = 108), using three dimensions from the Health Literacy Questionnaire. The selected dimensions were: 'Social support for health' (Social support scale), 'Ability to actively engage with healthcare providers' (Engagement scale), and 'Understand …

Predictive factors of treatment adherence in cancer outpatients.

OBJECTIVE: Identify which variables are predictors of treatment adherence in cancer patients. METHODS: Two hundred twenty cancer outpatients were evaluated by the following instruments: questionnaire on sociodemographic and clinical data, NEO-FFI Personality Inventory Revised (NEO-FFI-R), Multidimensional Health Locus of Control (MHLC) scale, Hospital Anxiety and Depression Scale (HADS), patient's knowledge about cancer disease questionnaire, and Adherence Determinants Questionnaire (ADQ). A logistic regression analysis was applied to verify the predictive power of the variables, and network …

Subjective anger and overt aggression in psychiatric outpatients.

BACKGROUND: The attention given to anger and aggression in psychiatric patients pales in comparison to the attention given to depression and … sorders. CONCLUSIONS: Anger is as common as depressed mood and psychic anxiety amongst psychiatric outpatients, and problems with anger cut across diagnostic categories. Given the high prevalence of problems with anger in psychiatric patients, more research should be directed towards its effective treatment.

Detecting delirium in elderly outpatients with cognitive impairment.

BACKGROUND: Delirium may be more prevalent in elderly outpatients than has long been assumed. However, it may be easily missed due to overlap with dementia. Our aim was to study delirium symptoms and underlying somatic disorders in psycho-geriatric outpatients. METHODS: We performed a case-control study among outpatients that were referred to a psychiatric institution between January 1st and July 1st 2010 for cognitive evaluation. We compared 44 cases with DSM-IV delirium (24 with and 20 without dementia) to 44 controls with dementia only. All participants were aged 70 years or older. We extr…

Pathogens Causing Upper Respiratory Tract Infections in Outpatients.

The aim of the present study was to determine the results of typical and atypical bacteria microbiological tests in patients with symptoms of chronic cough. We investigated 230 outpatients aged from 1 to 83 years (112 female, 72 male, and 46 children) who were free of any respiratory tract infection at the time of study. The material for the investigation consisted of pharyngeal swabs. Two hundred and thirty pharyngeal swabs were examined for Chlamydia pneumoniae antigen and for typical pathogens each. Chlamydia pneumoniae antigen was detected using an indirect immunofluorescence test and clas…
