A case of mesenteric panniculitis is presented in which the main signal was an abdominal pulsable mass and severe obstruction of the lymphatic vessels of the mesentery. A review of the literature was done.

A 37-year old man was admitted with colics in the upper right abdomen. Since the laparoscopy revealed a massive thickening of the right colonic flexure a hemicolectomy was performed. The histologic examination yielded the diagnosis of a necrotizing panniculitis of the colon, a rare variant of mesenterial panniculitis usually occurring in the small intestine. Signs and symptoms, diagnostics, and therapeutic approaches of this disorder of unknown cause are discussed.

Two patients with widespread, chronic, relapsing panniculitis resulting in disfiguring lipoatrophy are reported. Histology in both cases showed a mixed septal and lobular panniculitis, with lipophagia. The clinical appearance and histology suggested a diagnosis of lipoatrophic panniculitis. Both cases had features of connective tissue panniculitis, which is likely to be a subtype of this condition. Treatment of lipoatrophic panniculitis can be difficult. Our first patient initially responded well to antimalarial therapy, allowing plastic surgical repair of the defects to be carried out.

Panniculitides encompass a great number of different entities; however, once a vasculitis has been detected histopathologically within the subcutaneous tissue, the differential diagnosis is mainly restricted to polyarteritis (panarteritis) nodosa (PAN), nodular vasculitis (NV), and Bazin's erythema induratum (EI). Infection-induced panniculitis may result from a number of microbes including bacteria, fungi, and parasites. Viruses have also been implicated as a cause. Panniculitis and lipodystrophy are rare disorders of subcutaneous tissue. Recently the incidence of lipodystrophy has been increasing secondary to its appearance in patients with HIV. In this population, the lipodystrophy appears to be a direct consequence of drug therapy. A review of the available literature regarding pathogenesis and treatment options is discussed. The diagnosis of panniculitis has been hampered by problems. The recent literature has concentrated on ways of improving pathologic diagnostic yields, and new aids in diagnosis are presented.

Panniculitis is an inflammation of subcutaneous fat responsible for fairly uniform clinical features including nodules, plaques or swelling located in the subcutaneous tissue, occasionally progressing to atrophy of subcutaneous fat.
