General paresis of insane: A rarity or reality?

There is a widely held clinical opinion that syphilis has disappeared. Contrary to this, there are sporadic cases being reported across the country. The classical cases of neurosyphilis have given way to the milder asymptomatic forms, making them even more difficult to be diagnosed. Conventional presentation of neurosyphilis such as tabes dorsalis and general paresis of insane are read in textbooks only and rarely encountered in clinical practice in the 21(st) century.

A case of cyclic superior oblique paresis.

BACKGROUND: Cyclic strabismus, which generally has a 24 hours on-off cycle, is an unusual but interesting form of ocular motor disorder. Cyc … ave been described. In this report, we present a case with a 24 hours on-off cycle superior oblique paresis that resolved with surgery performed during the alternate day rhythm period. CASE REPORT: A 43 year old woman developed an alternate day hypertropia with bothersome diplopia. During the 24 hour "on" cycles she showed a unilateral left superior obli …

[A case of partial fascicular oculomotor paresis].

We report a case of partial fascicular oculomotor paresis in a 22-year-old woman. The patient noticed sudden onset diplopia which had transiently occurred twice in the past two months for several hours. She also had left blepharoptosis. Neurological examination revealed left blepharoptosis and diplopia caused by paresis of the left inferior oblique and superior rectus muscles. No other focal neurological signs were detected. Laboratory data, CSF examinations, ABR, SEP and VEP studies were all normal. Brain MRI revealed a high-signal-intensity lesion of about 7 mm in diameter in the left parame…

Traumatic abducens nerve paresis in a child.

Trauma is a frequent cause for abducens (sixth) nerve paresis in a child, usually attributed to injury along the nerve's course. An unusual focal lesion of the sixth nerve nucleus is described.

Recurrent transient paresis in a turkey flock.

Recurring episodes of extreme leg weakness and associated mortality were documented in a turkey flock at 8 to 15 weeks of age. Flock mortality attributed to posterior paresis was approximately 12%, or 4800 of 40,000 turkeys. Four of six open-confinement units were affected. Gross and histological examinations revealed no significant lesions. Immunology and virology were uninformative. There were no significant differences in serum chemistry between clinically affected and normal turkeys. Testing of feed, water, soil, and tissues revealed no common toxicants. Isolation and supportive care for a…

Intermittent bladder paresis in the pseudoclaudication syndrome.

The pseudoclaudication syndrome, now a well known neurological entity, can simulate intermittent claudication of arterial origin in its clinical appearance. Walking and sometimes simply standing upright for a while can cause transitory pain, numbness or weakness in the legs. The symptoms are caused by entrapment of the cauda equina fibers. Intermittent bladder paresis with total inability of micturition despite urgency to void may sometimes be a prominent feature of the syndrome, a fact that seems to be less well known as seen from a review of the literature.

Coefficients of impairment in deforming spastic paresis.

This position paper introduces an assessment method using staged calculation of coefficients of impairment in spastic paresis, with its rationale and proposed use. The syndrome of deforming spastic paresis superimposes two disorders around each joint: a neural disorder comprising stretch-sensitive paresis in agonists and antagonist muscle overactivity, and a muscle disorder ("spastic myopathy") combining shortening and loss of extensibility in antagonists. Antagonist muscle overactivity includes spastic cocontraction (misdirected descending command), spastic dystonia (tonic involuntary muscle …

Parturient paresis and hypocalcemia in ruminant livestock.

Parturient paresis (hypocalcemia) is most likely to affect dairy cattle around the time of parturition. It causes progressive neuromuscular dysfunction and flaccid paralysis. Older dairy cows, cows with a history of parturient paresis during a previous lactation, high-producing cows, and cows from the Jersey and Guernsey breeds are at highest risk for developing parturient paresis. Nonparturient hypocalcemia may also occur and is related to events other than parturition, such as severe stress, that temporarily overwhelm the mechanisms of calcium homeostasis. Beef cattle, sheep, and goats are a…

Superior division paresis of the oculomotor nerve.

Five patients with isolated ptosis and a paresis of ocular elevation in abduction consistent with an isolated superior division III nerve palsy are reported. In all instances the III nerve appeared to be involved before its reported anatomic bifurcation into a superior and an inferior division, in the anterior cavernous sinus.
