[Indirect reduction technique via Nice knot for transverse fracture of patella].

OBJECTIVE: To assess the outcomes in indirect reduction technique via Nice knot for transverse patellar fractures. METHODS: The clinical data of 25 patients with transverse patellar fractures meeting the inclusion criteria between January 2017 and December 2018 were retrospectively analyzed. The patients were divided into trial group ( n=13) and control group ( n=12) according to different intraoperative reduction methods. No significant difference was found in gender, age, affected side, cause of fracture, classification, or the time from injury to operation between the two groups ( P>0.05).

A Rare Case of Patella Fracture in Osteopetrosis: A Case Report.

CASE: Fractures in patients with osteopetrosis pose unique technical challenges to the orthopaedic surgeon. We present a case of a 43-year-old man with osteopetrosis who underwent tension band wiring of a patella fracture. The intraoperative appearance of osteopetrotic bone, technical difficulties in passing wires through dense bone, and how this was overcome are presented. CONCLUSIONS: A simple transverse fracture of the patella in a patient with osteopetrosis can be fixed successfully using a standard tension band construct.

Vertical open patella fracture, treatment, rehabilitation and the moment to fixation.

Patella fracture is relatively uncommon and the vertical trace fracture represents almost 12-17%. The open patella fracture expresses 6-30%. The association of these two uncommon conditions was the aim of this case report even as the treatment and the moment of fixation (definitive surgical treatment). A 27-year-old man after a motorcycle accident showed an open patella fracture classified as a Gustilo and Anderson type IIIA lesion. The patient was immediately treated with precocious surgery fixation with a modified tension band which consists of two parallel K-wires positioned orthogonal to t…

Case reports: pathologic fracture of the patella from a gouty tophus.

Fracture of the patella is a relatively common condition seen in patients with trauma. We report one patient with known gout who sustained relatively minor trauma that resulted in a patellar fracture. An intraoperative biopsy confirmed that much of the patella had been replaced with gouty tophus. Gout is a rare cause of patellar fracture, with few documented cases. Postoperative management of patients with patella fractures secondary to gout may be routine with the addition of medical management for the underlying pathologic process.

[Experimental study of tangential osteochondral and chondral fracture of the patella].

It has been reported that trauma is not an only responsible factor for tangential osteochondral fracture (OF) in the patellofemoral joint, but other factors predisposing to OF are also involved in many clinical cases. The present investigator manually dislocated the patella of 25 young rabbits without anesthesia (group 1). In other 25 young rabbits (group 2), dislocation of the patella followed the experimental production of the patellar subluxation which was considered as an etiology of OF. The resulting patellar cartilage changes were compared between these groups of rabbits to clarify the r

Lateral longitudinal stress fracture of the patella: report of three cases.

Stress fracture of the patella is a rare occurrence. Two types, longitudinal and transverse, are known. We report three lateral longitudinal fractures of the patella caused by running, Japanese fencing, and long distance walking. Patients returned to physical activities after 2-3 months of conservative treatment.

Recurrent non-Hodgkin's lymphoma presenting as pathological fracture of the patella.

We report a case of recurrent non-Hodgkin's lymphoma presenting as anterior knee pain due to pathological fracture of the patella. Anterior knee pains is an extremely common presenting complaint at orthopaedic clinics and the patella is an infrequent site both for primary and recurrent tumours. This case, treated by patellectomy, illustrates the difficulty of treatment of these lesions and highlights the importance of adequate radiological assessment in sites of previous tumour involvement.

Analysis of Isolated Patella Fractures: Do they Affect Walking?

The patella is an important part of the extensor mechanism of the knee. Due to its subcutaneous location, it can be easily exposed to trauma. In this study, we aimed to clarify the effect of patella fractures on walking by examining the results of gait analysis of patients with patella fractures.Patients who were treated due to a patella fracture in the last 10 years were selected from the hospital archive. A total of 18 patients and 36 healthy control subjects were included. Clinical, radiological, and functional results were collected for the fracture group retrospectively, and gait analysis…

Operative management of periprosthetic patella fractures: A case series.

BACKGROUND: Periprosthetic patella fractures (PPPFs) following total knee arthroplasty (TKA) are rare but challenging complications associated with poor outcomes and high failure rates. Optimal surgical fixation strategies remain unclear, particularly in the setting of compromised bone stock and extensor mechanism disruption. QUESTIONS/PURPOSES: We aimed to (1) evaluate clinical and radiographic outcomes following operative fixation of PPPFs, (2) compare results across fixation methods, and (3) assess functional recovery using extension lag, ambulation status, and PROMIS Physical Function sco…

Developments in the epidemiology and surgical management of patella fractures in Germany.

BACKGROUND: Patella fractures account for approximately 1% of all skeletal injuries. Treatment options are vast and no definitive conclusion on what option is the most beneficial could be made so far. Plate osteosynthesis appears to gain in importance. We aim to give insight into the more recent trends and developments as well as establish the epidemiology of patella fractures in Germany by analysing treatment and epidemiological data from a national database. METHODS: Anonymised data was retrieved form a national database. In the period of 2006 to 2020, all patients with patella fractures as…

Crossing Suture Technique for the Osteochondral Fractures Repair of Patella.

Osteochondral fracture of the patella is a common concomitant injury of the knee, especially in lateral patellar instability, and the importance of early stable fixation with minimal complication and early mobilization should be emphasized. Screws and Kirschner wires both absorbable and nonabsorbable have been the common mode of fixation of these fractures. Nevertheless, these fixation techniques require larger osteochondral fragments and are associated with cartilage abrasion, hardware prominence, synovitis, and foreign body reaction. In contrast, suture fixation can adequately stabilize smal

Sleeve fractures of the patella in children: a report of three cases.

The clinical characteristics of sleeve fractures of the patella are described and the methods of treatment are reviewed. Attention is drawn to the fact that the diagnosis may be missed because the distal bony fragment may be so small as not to be detectable by radiography. This is important because a large fragment of articular cartilage also separates. The best result was obtained from reconstitution of the extensor apparatus by internal fixation of the patellar fracture with repair of the quadriceps expansion.

A new technique for the treatment of comminuted, transverse fractures of the patella.

In an attempt to re-establish continuity of the quadriceps mechanism following partial patellectomy more securely in patients with comminuted, transverse fractures of the patella, a technique whereby the patellar ligament is inserted into a transverse groove created in the proximal patellar fragment and this anastomosis is then protected by a tension band wiring, is described and depicted. Using this method, it is possible to provide a larger surface area fro more satisfactory tendon to bone healing and to initiate relatively early motion of the knee, without compromising the site of reconstit

The fractured patella: a long-term follow-up study.

One hundred and three patients with 104 patellar fractures were examined over a follow-up period ranging from three to 11 years. Patients were assessed subjectively and objectively. The fractures were classified into three groups, transverse, comminuted and polar. Four methods of treatment were used, conservative, internal fixation, partial excision and total excision. In general, the results in the majority of patients were satisfactory, those patients who underwent total patellectomy faring at least as well as any others requiring surgical intervention.

Patella fractures are associated with bone fragility - a retrospective study.

Patella fractures are not typically considered osteoporotic fractures. We compared bone mineral density (BMD) and microstructure in elderly women from a multiethnic population-based study in New York City with any history of a patella fracture (n = 27) to those without historical fracture (n = 384) and those with an adult fragility forearm fracture (n = 28) using dual energy x-ray absorptiometry (DXA) and high resolution peripheral quantitative computed tomography (HR-pQCT). Compared to those without fracture, women with patella fracture had 6.5% lower areal BMD (aBMD) by DXA only at the total…

Chondroblastoma of the patella with pathological fracture in an adolescent: a case report.

BACKGROUND: Chondroblastoma is a rare primary bone tumor of young people that generally occurs in the epiphyseal plate of long bones. To date, only 13 cases of patella with pathological fracture in chondroblastoma have been previously published. CASE PRESENTATION: A 15-year-old male patient presented with acute pain in the left knee after an injury occurred while playing basketball. Plain radiographs and computed tomography showed a pathological fracture of the left patella with an osteolytic lesion (1.5 × 2 × 3 cm). Magnetic resonance imaging revealed an expansile lesion within the patella w
