Reconstruction of the medial patellofemoral ligament for patellar instability using a semitendinosus autograft.

Between 1999 and 2001 thirty knees underwent a semitendinosus tendon plasty to recreate the medial patellofemoral ligament for recurrent patellar dislocation. The mean follow-up was 38 months. The mean improvement of the patellofemoral congruence angle after surgery was 14 +/- 7 degrees. All patients ended up with a full range of motion, except one patient, whose flexion was limited to 120 degrees due to superficial wound infections. Dislocation did not recur. According to the Larsen and Lauridsen outcome score the clinical results were excellent in 27 patients, good in 2 and fair in one. In c

[Tendinopathy of the patellar ligament secondary to transtendineal arthroscopy of the knee. Ultrasonographic evaluation].

During knee arthroscopy, the transtendinous approach best visualizes articular structures, even though it may cause patellar tendinitis, following the surgical procedures. Thirty patients (22 men and 8 women) who had undergone transtendinous arthroscopy of the knee were submitted to clinical and US follow-up at 3, 6, 12 months. This monitoring was aimed at correlating clinical and instrumental findings of the above iatrogenic condition. Painful symptoms were present in 30% of the cases in group I, in 10% of group II and in no patient in group III. US demonstrated thickening of the patellar ten

Three different patellar fixation techniques yield similar clinical and radiological outcomes in recurrent patellar dislocation undergoing medial patellofemoral ligament reconstruction.

PURPOSE: The purpose of this study was to evaluate and compare the clinical and radiological outcomes of three different patellar fixation techniques on medial patellofemoral ligament reconstruction (MPFLR) in the treatment of patellar dislocation (PD). METHODS: Between 2015 and 2020, 130 patients with recurrent PD who underwent surgical reconstruction were eligible for this retrospective study: 48 patients were treated with the semi-tunnel bone bridge fixation technique (Group A), 42 patients were treated with the suture anchor fixation technique (Group B) and 40 patients were treated with t

Medial Patellofemoral Ligament Repair for Recurrent Patellar Instability: Successful Outcomes Among Patients With Low Coronal Malalignment and Normal Patellar Height.

Medial patellofemoral ligament (MPFL) repair is discouraged for the treatment of chronic, recurrent patellar instability (RPI) because of high reported failure rates. However, the senior author uses MPFL repair for chronic RPI in the setting of low tibial tubercle-trochlear groove (TT-TG) distance. In this retrospective case series, we report results and evaluate outcome predictors. We used billing records to identify all patients, 14 years or older, who underwent isolated repair for chronic RPI performed by a single surgeon between September 2010 and February 2019. The TT-TG distance, patella

Longitudinal Change of Medial and Lateral Patellar Stiffness After Reconstruction of the Medial Patellofemoral Ligament for Patients with Recurrent Patellar Dislocation.

BACKGROUND: Longitudinal changes in patellar stiffness following reconstruction of the medial patellofemoral ligament for recurrent patellar dislocation at full extension are unknown. METHODS: Fifteen consecutive patients (three men and twelve women, with a mean age of twenty-two years) with seventeen knees were matched by sex and age to thirty-two reference subjects in this prospective study. The follow-up period was a minimum of twenty-four months. The medial patellofemoral ligament reconstruction used an autograft semitendinosus tendon and an interference screw system with or without later

Quantitative and Qualitative Analysis of the Medial Patellar Ligaments: An Anatomic and Radiographic Study.

BACKGROUND: The qualitative and quantitative anatomy of the medial patellar stabilizers has been reported; however, a quantitative analysis … ures to assess their locations relative to pertinent bony landmarks. RESULTS: Four separate medial patellar ligaments were identified in all specimens. The center of the MPFL attachments was 14.3 mm proximal and 2.1 mm posterior to the medial epicondyle and 8.3 mm distal and 2.7 mm anterior to the adductor tubercle on the femur and 8.9 mm distal and 19.9 mm medial …

Medial patellofemoral ligament anatomy: is it a predisposing factor for lateral patellar dislocation?

PURPOSE: Because the medial patellofemoral ligament (MPFL) is the primary restraint to lateral dislocation of the patella, we aimed, in this controlled study, to verify whether the MPFL with different measurements could be considered another predisposing factor for patellar dislocation. METHODS: A group of 100 consecutive individuals without the criteria for patellar dislocation (trochlear dysplasia, patella alta and lateral patellar tilt) was recruited as a control group and underwent magnetic resonance imaging (MRI) study and another group of 50 patients with patellar instability. Femoral c

Mechanical tensile properties of the quadriceps tendon and patellar ligament in young adults.

We analyzed mechanical tensile properties of 16 10-mm wide, full-thickness central parts of quadriceps tendons and patellar ligaments from paired knees of eight male donors (mean age, 24.9 years). Uniaxial tensile testing was performed in a servohydraulic materials testing machine at an extension rate of 1 mm/sec. Sixteen specimens were tested unconditioned and 16 specimens were tested after cyclic preconditioning (200 cycles between 50 N and 800 N at 0.5 Hz). Mean cross-sectional areas measured 64.6 +/- 8.4 mm2 for seven unconditioned and 61.9 +/- 9.0 mm2 for eight preconditioned quadriceps t…

Unusual patellar tendon injury in an adolescent runner with generalised ligamentous laxity.

A case is reported of an acute traction injury of the patellar tendon in a boy of 14 with joint hypermobility. Such injuries are unusual but early diagnosis and surgical repair lead to a good long term outcome. In adolescents participating in sports, the awareness of the possibility of a rare knee extensor mechanism injury is essential for a successful outcome.

Effect of anterior cruciate ligament reconstruction with patellar tendon or prosthetic ligament on the morphology of the other ligaments of the knee joint. An experimental study in dogs.

The effect of anterior cruciate ligament replacement on the other ligaments of the knee, including the medial or lateral collateral and posterior cruciate ligaments, was studied histologically in the canine knee. The anterior cruciate ligament was replaced either with the medial third of the patellar tendon or with a prosthetic ligament. Histologic analysis was performed 3 months after prosthetic ligament implantation and 1 year after patellar ligament reconstruction. In all ligaments, numerous pathologic alterations were found. Degenerative changes such as glycosaminoglycan accumulation, meta…

Concomitant Patellar Tendon Reconstruction With Hamstring Autografts and Medial Patellofemoral Ligament Reconstruction With Quadriceps Autograft in the Setting of Severe Patellar Tendinopathy With Recurrent Lateral Patellar Instability.

Chronic injuries to the extensor mechanism present unique challenges to physicians because of their complex nature and the need for a multifactorial treatment approach. When conservative treatment is unsuccessful in the setting of patellar tendinopathy, surgical intervention may be required to improve symptoms. For patients with concomitant lateral patellar instability, surgical treatment is also indicated to prevent future dislocations and worsening patellofemoral symptoms. Current literature presents numerous patellar tendon and medial patellofemoral ligament arthroscopic and open reconstruc

Ultrasonographic examination of the patellar ligament after capsular and fascial imbrication for the treatment of cranial cruciate ligament rupture in dogs.

INTRODUCTION: Cranial cruciate ligament (CCL) rupture is the most common orthopedic condition in dogs, with many surgical options available for its treatment. Thickening of the patellar ligament after capsular and fascial imbrication (CFI) is a frequently reported complication, but its clinical significance remains unclear. METHOD: This prospective study evaluated patellar ligament structural and thickness changes after CCL repair using CFI. Forty-six dogs with CCL ruptures treated at the Small Animal Clinic of Freie Universität Berlin between July 2013 and April 2015 were included. Clinical,…

Allograft Medial Patellofemoral Ligament Reconstruction Using Patellar Suture Anchors and Stepwise Medial Imbrication.

Patellar instability commonly affects adolescents and young athletes and is associated with injury to the medial patellofemoral ligament (MPFL). MPFL reconstruction is a well-established treatment for recurrent instability; however, complications related to graft tensioning, fixation, and incomplete restoration of medial soft-tissue restraints persist. We describe a surgical technique for allograft MPFL reconstruction that emphasizes stepwise imbrication of the medial knee layers, anatomic femoral tunnel placement, and all-suture fixation to the patella. This approach restores both the static

Bone-Patellar Tendon-Bone Autograft in Anterior Cruciate Ligament Reconstruction: A Comprehensive Approach.

Anterior cruciate ligament reconstruction using a bone-patellar tendon-bone autograft is widely regarded as the graft of choice for many sports medicine surgeons for young, active patients with anterior cruciate ligament injuries. It is favored for use in athletes because of its high return-to-play rates and low re-rupture rates. Despite its benefits, there are many pitfalls in the surgical technique, and the use of a careful surgical technique can avoid them. This article describes our preferred technique.

Simple Cost-Effective Reinsertion of Avulsed Medial Patellofemoral Ligament in Acute Patellar Dislocation.

The medial patellofemoral ligament (MPFL) is the main restraining force against lateral patellar displacement in the first 20° of flexion and is disrupted after patellar subluxation or dislocation. Management of acute patellar dislocations is controversial, and many clinicians opt for conservative treatment in the acute phase. However, a traumatic rupture of the MPFL warrants surgical attention. Several considerations must be made by surgeons attempting reinsertion of the MPFL, including the choice of implant and timing of surgery, to restore the anatomy and biomechanics of the patellofemoral

[Simultaneous Traumatic Rupture of Patellar Ligament and Contralateral Rupture of Quadriceps Femoris Muscle].

Our paper presents a unique case of a 64-year-old patient after a fall, treated with oral antidiabetic drugs for type II diabetes mellitus. Following a series of examinations, a bilateral injury was diagnosed - patellar ligament tear on the right side and rupture of quadriceps femoris muscle on the left side. It is a rare injury, complicated by simultaneous involvement of both knee joints. The used therapy consisted of a bilateral surgery followed by gradual verticalisation, first with the support of a walking frame and later with the use of forearm crutches. During the final examination, the …

Patellar subluxation at terminal knee extension: isolated deficiency of the medial patellomeniscal ligament.

BACKGROUND: The medial patellomeniscal ligament has been determined in vitro to be the only substantial distal-medial restraint to lateral patellar displacement. The purpose of this study of prospectively collected data was to determine if isolated deficiency of the medial patellomeniscal ligament, resulting in excessive lateral patellar translation and associated symptoms, could be identified. We hypothesized that, if symptoms were associated with isolated deficiency of that ligament, the laxity and symptoms should resolve after plication of that ligament alone. METHODS: Testing for lateral
