Eight Years of Expertise With the Sandwich Technique for Asymmetric Pectus Carinatum and Excavatum-Carinatum Complex.

INTRODUCTION: The Sandwich technique is a novel approach for managing pectus carinatum and the excavatum-carinatum complex. Although first described in 2016, literature and global experience are scarce. We report our eight-year experience using the Sandwich technique, with a special focus on surgical aspects. METHODS: Multicentric, retrospective study (2017-2024) including patients undergoing the Sandwich technique for pectus carinatum and excavatum-carinatum complex with a similar approach was conducted in three centers. Under thoracoscopic guidance and using sternal elevation, internal bars…

Postural Alterations in Adolescents With Pectus Excavatum and Pectus Carinatum: A Retrospective Observational Study.

BACKGROUND Pectus excavatum (PE) and pectus carinatum (PC) are the most common anterior chest wall deformities, affecting approximately 1% of the pediatric population. This study aimed to determine the prevalence of postural alterations in patients with PE and PC and to evaluate their associations with deformity type and severity. MATERIAL AND METHODS This retrospective, single-center descriptive study reviewed medical records of 268 patients aged 10-18 years who were evaluated for chest wall deformities between 2018 and 2024. Demographic variables, deformity type (PE, PC, or mixed), deformity…

Amianthoid transformation of costal cartilage matrix in children with pectus excavatum and pectus carinatum.

BACKGROUND: It is unclear if amianthoid transformation (AT) of costal cartilage extracellular matrix (ECM) has an impact on the development of pectus excavatum (PE) and pectus carinatum (PC). METHODS: AT foci were examined in intrasurgical biopsy specimens of costal cartilages of children (8-17 years old) with PE (n = 12) and PC (n = 12) and in age-matching autopsy control samples (n = 10) using histological and immunohistochemical staining, atomic force and nonlinear optical microscopy, transmission and scanning electron microscopy, morphometry and statistics. RESULTS: AT areas were identif…

The The Sandwich Technique For Minimally Invasive Repair Of Pectus Carinatum.

Minimally invasive repair of pectus carinatum (MIRPC) has been performed using the Abramson technique in which the bar that compresses the sternum is fixed with steel wires on the ribs. A 14-year-old patient underwent to a MIRPC using a sandwich technique in which two metallic bars fixed with bridges were implanted below the sternum under thoracoscopic vision, and another bar in a subcutaneous tunnel was implanted above. This technique has the potential to avoid specific problems related to the original technique like loosening of support for correction (broken wire), avoidance of induction of…

Bracing is an effective therapy for pectus carinatum: interim results.

BACKGROUND: Pectus Carinatum is a common congenital chest wall malformation. Until recently the mainstay of treatment was surgical remodeling of the deformed chest wall. Initial results suggest that non-operative bracing may be an effective therapy, but the optimal strategy for correction is not known. Herein we report the results of a self-adjustable low profile bracing system worn continuously until the defect is corrected (correction phase), then worn at night (8 h/day) until completion of axial growth (maintenance phase)-the Calgary Protocol. METHODS: Patients referred to a pediatric surg…

Clinical experience of repair of pectus excavatum and carinatum deformities.

OBJECTIVE: We present the results of surgical correction of pectus excavatum (PE) and pectus carinatum (PC) deformities in adults, and also report a new method of sternal support used in surgery for PE deformities. METHODS: We present the results of 77 patients between the ages of 10 and 29 years (mean 17) with PE (n = 46) or PC (n = 31) deformities undergoing corrective surgery from 2004 to 2011, using the Ravitch repair method. Symptoms of the patients included chest pain (15%) and tachycardia (8%). Three patients underwent repair of recurrent surgical conditions. RESULTS: All of the patie…

Pectus carinatum bracing: key factors in success and dropout.

PURPOSE: Pectus carinatum (PC) is a prevalent chest wall deformity mostly managed with orthotic bracing. Despite the effectiveness, research identifying specific predictors of treatment success remains limited. This study evaluates clinical findings from external bracing to propose a reliable indicator for predicting successful outcomes. METHODS: A comprehensive assessment was conducted on a large cohort of patients presenting to a specialized clinic during 2019 to 2024 May. Evaluations included demographics, deformity characteristics, and the “initial force of correction” measured at first ex…

Combined surgical correction of pectus carinatum and juvenile kyphosis.

Pectus carinatum may rarely be associated with kyphosis. However, the correlation between both conditions is not well reported. Therefore, there are no reports for combined correction of both deformities in the same patient. Moreover, studies estimating the kyphosis prevalence in patients with pectus carinatum are lacking. To our knowledge, this is the first paper to present such a case. We report an 18-year-old boy with both pectus carinatum and kyphosis that were surgically corrected in a combined procedure. The indication of surgery is cosmetic, and the postoperative recovery included pneum…

Changes in self-image after pectus carinatum brace treatment.

BACKGROUND/PURPOSE: Pectus carinatum (PC) is a chest wall deformity resulting in anterior protrusion of the chest. PC does not typically result in significant physical or cardiopulmonary symptoms, but patients with this condition can experience a disturbed body image, lower self-esteem and reduced quality of life.  The purpose of this study was to investigate the relationship between self-image and non-surgical correction of PC using a brace. METHODS: This study was a descriptive, pre-post survey design. The sample included children ages 11 to <18 years undergoing PC treatment with the dynami…

Measured dynamic compression for pectus carinatum: A systematic review.

BACKGROUND: Patients with pectus carinatum have been treated with orthotic braces since the 1970s. By applying external pressure to the anterior chest wall, a normal chest shape can be restored. However, high patient treatment dropout rates were reported because of the subjectively high pressures applied to the patient´s skin. Measured dynamic compression allows measurement and adjustments of the brace's pressure on the thoracic wall, leading to a controlled correction. METHODS: We performed an electronic database search (Pubmed and Cochrane) of the medical literature on measured dynamic comp…

Minimal access treatment of pectus carinatum: a preliminary report.

BACKGROUND: Unlike the minimal access repair of pectus excavatum (PE), a minimal access repair of pectus carinatum (PC) has not been established. This initial experience reports the correction of PC using a minimal access technique in five patients. METHODS: The procedure was performed by insertion of a pectus bar through either bilateral or only left midaxillary incision. The pectus bar was placed so that the elevated sternum could be depressed. Thoracoscopy was not employed during the procedures and no stabiliser plates were used. On completion of the procedure, a chest tube was inserted in…

Initial results with minimally invasive repair of pectus carinatum.

OBJECTIVE: Pectus carinatum is traditionally repaired by using some modification of the open Ravitch procedure, with its possible long-term sequelae, such as poor postoperative compliance of the chest. In this study we assessed our results with a new minimally invasive repair of pectus carinatum that requires neither cartilage incision nor sternotomy. METHODS: From June 2005, we have corrected pectus carinatum using a method analogous to the Nuss procedure for pectus excavatum repair. Thus far, we performed this intervention on 14 patients (mean age, 15 +/- 1.5 years). A steel bar has been in…

Pectus carinatum successfully treated with bracing. A case report.

We report the case of a 14 year old girl who desired treatment for a cosmetically objectionable pectus carinatum deformity. She was initially managed with a corrective underarm body cast for six weeks followed by full-time bracing for seven months. Subsequently, she was braced only at night for another eight months. Seven years after the onset of treatment she had an excellent result without recurrence of deformity. In a well-motivated, skeletally immature individual bracing can be an effective treatment for cosmetically displeasing pectus carinatum.

Ten Years of Abramson Experience in Patients With Pectus Carinatum.

OBJECTIVES: Pectus carinatum is the second most common congenital chest wall deformity. While open thoracic reconstruction surgeries like the Ravitch procedure remain a treatment option, minimally invasive techniques such as the Abramson procedure are increasingly preferred. This study presents our clinical experience with the Abramson procedure for pectus carinatum. MATERIALS AND METHODS: A retrospective review was conducted on 86 patients who underwent the Abramson procedure in the Department of Thoracic Surgery at Dokuz Eylul University between 2011 and 2021. Data were collected on age, ge…

Cardiopulmonary function in adolescent patients with pectus excavatum or carinatum.

BACKGROUND: Pectus excavatum (PE) and pectus carinatum (PC) have generally been considered an aesthetic issue, although there is growing evidence of associated cardiopulmonary function (CPF) impairment, especially in PE patients. The study goal was to determine any correlation between pectus malformations and cardiopulmonary symptoms and function based on systematic assessment of CPF and thoracic measurements, such as Haller Index (HI) and sternal torsion angle (STA). METHODS: Data from 76 adolescent patients with PE (n=30) or PC (n=46) were retrospectively collected referred between January …
