OBJECTIVE: The diagnosis of common peroneal nerve entrapment neuropathy (CPNEN) is based on clinical symptoms and nerve conduction studies. However, nerve conduction studies may not detect abnormalities. Under the hypothesis that repetitive plantar flexion that loads the peroneal nerve (PN) at the entrapment point without lumbar loading would be a useful CPNEN provocation test, we evaluated the repetitive plantar flexion (RPF) test as an adjunct diagnostic tool for CPNEN. The study design was a retrospective analysis of prospectively collected data.
METHODS: Our study population consisted of

We present a unique case of a man in his 60s with a knee intra-articular loose body causing a compression neuropathy on his common peroneal (fibular) nerve (CPN). This neuropathy caused a reduction in function of his leg's motor function causing a foot drop. Through a strong clinical interview and focused clinical examination, the neuropathy was localised, and the diagnosis was reinforced through nerve conduction studies and imaging. The intra-articular loose body was surgically excised after the careful isolation and decompression of the peroneal nerve. This resulted in a marked improvement i

INTRODUCTION: Popliteal cysts (Baker cysts) causing compressive neuropathy of combined tibial and common peroneal nerves is an exceptional entity. Especially, an isolated multi-septate unruptured cyst (usually posteromedially) dissecting posterolaterally, thereby causing compression of multiple components of the popliteal neurovascular bundle as seen in this case report is unique in clinical field and in the literature. Awareness and early diagnosis of such cases along with a prudent technique will preclude permanent impairment.

PURPOSE: Identification of modifiable risk factors for falling is paramount in reducing the incidence and morbidity of falling. Peroneal neuropathy with an overt foot drop is a known risk factor for falling, but research into subclinical peroneal neuropathy (SCPN) resulting from compression at the fibular head is lacking. The purpose of our study was to determine the prevalence of SCPN in hospitalized patients and establish whether it is associated with a recent history of falling.
METHODS: We conducted a cross-sectional study of 100 medical inpatients at a large academic tertiary care hospit

Osteochondromas are the most common benign bone tumors, comprising 9% of all bone tumors and 35% of benign bone tumors. They are frequently diagnosed incidentally. Osteochondromas are mostly asymptomatic, but may cause mechanical findings depending on the location and size of the tumor. Rarely, they may originate from the interosseous surface of the tibia and affect the fibula. We report here a case of a rare osteochondroma originating from the distal tibial metaphysis and causing deep peroneal nerve entrapment syndrome with clinical and radiological findings. To our knowledge, this is the fir

A detailed clinical and electrodiagnostic study has been undertaken of demyelinating polyneuropathy in 14 children (9 male, 5 female) from 11 sibships and their parents. The onset of symptoms was before the age of 2 years in 12 of the 14 children, and the condition in all cases was nonprogressive or very slowly progressive. In each case one of the parents had a slow motor nerve conduction velocity. Five of the 11 affected parents were completely asymptomatic. Electrodiagnostic studies in both parents of all children with demyelinating peripheral neuropathy are thus important to identify the do

BACKGROUND: Baker's cysts are usually located in the posteromedial side of the knee and seldom cause neuropathy.
CASE: We describe the rare case of a 57-year-old woman with a popliteal cyst who presented with limping gait and pain in her lower leg. She was electronically diagnosed with common peroneal neuropathy and transferred to our pain clinic. On ultrasound examination, about 2.0 × 1.2 cm sized popliteal cyst was found to extend to the fibular head, compressing the common peroneal nerve. Therefore, ultrasound-guided aspiration of the cyst and a common peroneal nerve block were performed.

OBJECTIVE: The objectives of our study were to describe the previously unreported normal MR anatomy of the distal biceps femoris muscle and its relationship with the common peroneal nerve and to present a case in which previously unreported MR evidence of an anatomic variation in the distal biceps femoris muscle was associated with common peroneal entrapment neuropathy.
MATERIALS AND METHODS: One hundred consecutive 1.5-T knee MR studies of 97 asymptomatic patients were retrospectively reviewed by two observers in consensus for, first, normal anatomy of the distal biceps femoris muscle; secon

Ganglion cysts are very rare in the lower limb and when present, ganglion cysts rarely cause compression neuropathy at any site. Peripheral nerve sheath tumors as a whole, are also very rare and mostly presented as a painful lump along the nerve path. Ganglion cysts are non-neoplastic gelatinous cysts, which lack true synovial lining. They can be divided into intraneural cysts which can be found within the epineurium of a peripheral nerve and lead to signs and symptoms of peripheral neuropathy or extraneural cysts which can develop from surrounding joints or tendon sheaths causing gradual nerv

BackgroundThe causes of peroneal neuropathy are various, but are rarely due to weight loss. Bilateral peroneal neuropathy caused by weight loss after surgery has been reported only after bariatric surgery and there were no reports associated with other abdominal surgery. In this report, we describe a case of the bilateral peroneal neuropathy that occurred due to marked weight loss after biliary surgery.Case summaryA 58-year-old male did not receive adequate nutritional support after biliary surgery, and showed a massive weight loss over a short period of time (body mass index; 24.1 kg/m2 to 20

The assessment of neuromuscular recovery after peripheral nerve surgery has typically been a subjective physical examination. The purpose of this report was to assess the value of gait analysis in documenting recovery quantitatively. A professional football player underwent gait analysis before and after surgery for a peroneal intraneural ganglion cyst causing a left-sided foot drop. Surface electromyography (SEMG) recording from surface electrodes and motion parameter acquisition from a computerized motion capture system consisting of 10 infrared cameras were performed simultaneously. The common peroneal nerve is a major source of innervation to the lower limb, but it is sometimes compressed or entrapped at the fibular head. The authors present what they believe is the first reported case where peroneal nerve impingement caused by ganglion cyst compression of the nerve at the fibular head was diagnosed using a combination of ultrasound imaging and electrodiagnostic studies. The authors described the history, physical examination, electrodiagnostic findings, and musculoskeletal ultrasound findings of a patient with a left foot drop caused by a ganglion cyst compressing the c

INTRODUCTION: There have been intermittent reports of peroneal neuropathy (PN) occurring after liver transplantation. Although PN may not be viewed as a serious complication by liver transplant (LT) recipients who require the transplant for survival, PN can significantly reduce quality of life. The incidence of PN appears to have increased after the use of gel pads was introduced. These gel pads, which are placed under patients' knees during surgery, are used to reduce lower back strain and prevent contact between the peroneal nerve at the fibular head and the hard surface of the operating tab

Mononeuropathies are unusual at birth, and electromyographic (EMG) definition the first day of life has not been reported previously. Although neonatal mononeuropathies may be related to obstetric complications, prenatal mechanisms also merit consideration. We report an infant, born with a peroneal neuropathy, whose EMG was performed 18 h after birth. An isolated peroneal nerve lesion with lack of compound muscle action potential and the presence of fibrillation potentials, confined to the tibialis anterior muscle, suggested a primary intrauterine mechanism for this mononeuropathy. Because of

An accessory head of the gastrocnemius muscle, known as the gastrocnemius tertius (GT), is the most common anatomical variation of the gastrocnemius muscle. While often asymptomatic, this variant can occasionally compress adjacent structures. We report the case of a 61-year-old man with chronic lower limb pain, found to have a unilateral GT muscle with a concurrent anatomical variant course of the common peroneal nerve. MRI revealed fascicular thickening and increased T2 signal intensity within the nerve, suggestive of neuritis. This is, to our knowledge, the first reported case of symptomatic

Peroneal nerve entrapment, typically associated with behaviors like cross-legged sitting or squatting, can also occur from extended periods of lying down where the lower limbs usually assume a position of hip external rotation and knee flexion. In such positions, the fibular head's prominence can exert sustained pressure on the peroneal nerve. We report three cases of unilateral peroneal neuropathy in intensive care unit (ICU) patients during the coronavirus disease (COVID-19) pandemic, highlighting the possible role of prolonged supine or lateral decubitus positions in the development of this

Lateral knee cysts are relatively common synovial swelling. They are commonly benign swelling due to synovial fluid herniation from the joint. They are usually asymptomatic and in most cases are so small that only magnetic resonance imaging (MRI) can confirm the diagnosis. A synovial cyst of size more than 4 cm with nerve compression leading to peroneal neuropathy has grave clinical implications. We report an unusual case of a large, neglected, symptomatic, chronic, lateral synovial cyst with foot drop in a 67-year-old male. He presented after almost two months of onset of weakness in the righ
