[Cardiopulmonary exercise testing: Key practical aspects].

Functional exercise testing (FET) assesses an individual's capacity to adapt to effort and identifies limiting factors, particularly dyspnea. It orients therapeutic choices, predicts the progression of chronic pathologies, and estimates preoperative risks, at times contraindicating surgery. The aim of this article is to provide a summary of the specific indications for functional exercise testing, test protocol selection, test equipment, appropriate personnel, and patient and test safety. This article is intended for healthcare professionals conducting or considering functional exercise testin

Ambulance dispatching during a pandemic: Tradeoffs of categorizing patients and allocating ambulances.

Amidst a pandemic, operators of emergency medical service (EMS) systems aim at upholding service at sufficiently low response times while reducing the infection probability of their personnel. Designating ambulances to serve only infected patients and suspected cases may reduce the outage probabilities of ambulances and consequently the response times of the EMS. We investigate the benefits that EMS personnel and patients can gain from such a split. As a solution method to quantify these benefits, we apply a two-stage approach. First, we run a first-stage optimization model to pre-select ambul

Risky driving among regular armed forces personnel from the United Kingdom.

BACKGROUND: Road traffic accidents are the leading cause of death for service personnel from the United Kingdom (UK). Little is known about the pattern of risky driving by these service personnel. METHODS: Cross-sectional data (collected postdeployment, between June 2004 and March 2006) were analyzed from a large, randomly selected cohort of military personnel from the UK. These analyses were limited to regular-service personnel who were drivers (n=8,127; 7,443 men and 684 women). "Risky driving" (not wearing a seatbelt, speeding, or both) was examined. Analyses were then repeated but restric

[Intersensory interactions as the basis of methods in enhancing resistance to motion sickness].

The paper considers how to improve prophylactic measures for air motion sickness. It outlines the results of experimental investigations to substantiate vestibular selection and training methods based on the use of polysensory vestibular and visual stimulations, somatosensory afferent effects and mental activity in the flight personnel during exercise tests. The developed methods are shown to have a high efficiency in providing good tolerance of vestibular stimulations during flight under the conditions of higher flying educational establishments at the stage of primary flying training.

Workplace violence and influencing factors among paramedic pre hospital paramedic personnel (city and road) in Iran: a quality content analysis.

BACKGROUND: The goal of every emergency department is to provide the highest quality services in the shortest time using limited resources. However, occupational violence is so prevalent among pre-hospital paramedic personnel that some experts claim that it is impossible to find pre-hospital personnel without an experience of violence in the workplace. Therefore, it seems necessary to investigate the causes of violence among this population group and find ways to control it. AIM: The present study aimed to investigate the Violence and influencing factors among paramedic pre-hospital personnel

Arms Race Control Score Standardizes Residency Applicant Publication Assessment.

BACKGROUND AND OBJECTIVES: The publication "arms race" refers to the recent, exponential increase in publications among residency "Match" applicants. Total number of residency applicant publications is the strongest predictor of "Match" success in neurosurgery beyond licensing examination scores. This study sought to introduce an objective metric by which publications may be scored to assess residency applicant authorship effort. METHODS: A retrospective, quantitative assessment of 537 publications by 57 interviewed applicants at a neurosurgery residency program was conducted from the 2022 to

The Current Status of Women in Surgery: How to Affect the Future.

IMPORTANCE: Despite women composing half of current medical school classes, surgical specialties still struggle to attract and retain women. Even after successful recruitment into training, women surgeons continue to face gender bias and various obstacles to career advancement, including lower rates of surgical residency completion, board certification, and professional advancement. Gender inequality in medicine has drawn attention; particular consideration regarding the status of women in surgery is warranted. We review research delineating disparities and investigating the causes underlying

Recruiting physicians without inviting trouble.

Many hospitals use physician recruitment strategies--generally assistance or employment strategies--to ensure medical staff loyalty. Although these strategies appeal to both hospitals and physicians, they are becoming increasingly problematic. Over the past three years, the government has issued pronouncements that question their legality. Thus any hospital considering physician recruitment strategies would be wise to evaluate them in light of various legal issues. such as reimbursement, nonprofit taxation, corporate practice of medicine, and certificate-of-need statutes. The consequences of f

Recruitment Strategies To Improve Gender Equity in Clinical and Translational Research Training in Ethiopia.

Women from sub-Saharan Africa, including Ethiopia, are underrepresented in biomedical research due in part to limited access to high-quality research training and mentorship. Tuberculosis (TB) is a major public health problem in Ethiopia, with a limited number of female Ethiopian scientists engaged in TB-related research. To improve access to TB-related research training among junior women scientists, our NIH Fogarty International Center-funded D43 program released an all-women request for applications (RFA), which substantially increased the number of women applying for research training and

Chairs and chiefs of plastic surgery: is it an insider job?

PURPOSE: There is no more important decision an academic Plastic Surgery Department or Division can make than naming a chair or chief. Externally recruited leadership brings fresh perspectives and connections. Critics, however, argue that they lack the in-depth knowledge of the institution's culture and history that may be needed to succeed. The ability and skill of an internal candidate is already known and can increase the odds of that person's success in the leadership position. Finally, external recruitment can be a more costly process. Ultimately, the decision is really a litmus test for

Incarceration history and ethnic bias in hiring perceptions: An experimental test of intersectional bias & psychological mechanisms.

This study seeks to better understand mechanisms of bias against formerly incarcerated and ethnically minoritized job applicants as well as the interactive effects of those two identities. In a sample of 358 hiring managers in the United States, the 2 (incarceration history) x 4 (ethnicity) experiment will manipulate incarceration history and ethnicity through job application materials, and measure hireability, and perception of job applicants along dimensions of sociability/warmth, competence, and morality. We will use a moderated mediation model to test hypotheses regarding a main effect of

The Otolaryngology Residency Program Preference Signaling Experience.

PROBLEM: In the 2021 residency application cycle, the average otolaryngology applicant applied to more than half of programs. Increasing application numbers make it difficult for applicants to stand out to programs of interest and for programs to identify applicants with sincere interest. APPROACH: As part of the 2021 Match, otolaryngology applicants could participate in a preference signaling process, signaling up to 5 programs of particular interest at the time of application submission. Programs received a list of applicants who submitted signals to consider during interview offer delibera

The future of neurosurgery: a white paper on the recruitment and retention of women in neurosurgery.

PREFACE: The leadership of Women in Neurosurgery (WINS) has been asked by the Board of Directors of the American Association of Neurological Surgeons (AANS) to compose a white paper on the recruitment and retention of female neurosurgical residents and practitioners. INTRODUCTION: Neurosurgery must attract the best and the brightest. Women now constitute a larger percentage of medical school classes than men, representing approximately 60% of each graduating medical school class. Neurosurgery is facing a potential crisis in the US workforce pipeline, with the number of neurosurgeons in the US

[Atrial fibrillation--incidence on flying safety--practical implication on military and civil aircrew].

Recent epidemiological studies and new approaches in the management of atrial fibrillation (AF) incite to reconsider its incidence on medical aircrew certification. Paroxysmal as well as chronic AF are common diseases and are found among pilots particularly when advanced in years. It is usually associated with heart disease, especially with rheumatic valvulopathies. However idiopathic or "lone" AF is observed in subjects apparently-free of any heart disease. Mortality is twice higher in patient suffering from AF. It is responsible for miscellaneous symptoms among which palpitations or even syn
