This case series introduces the medial approach for surgical management of plantar medial avulsion (PMA) fractures of the calcaneus, which frequently occur in conjunction with tongue-type fracture patterns. The technique allows for direct access to the PMA fragment, enabling precise reduction and stabilization through buttress plating. In a series of 12 patients treated at a single institution, there were no soft tissue related complications including infections, wound issues, or neurologic injuries, and only one case requiring reoperation for symptomatic lateral implants. In conclusion, the m

BACKGROUND: TMT-1 arthrodesis is an established method in hallux valgus surgery, but it is technically demanding and typically calls for a period of postoperative immobilization.
METHODS: In this cohort study, initial experience with a plantar plate is described. 58 patients (59 arthrodesis) were included.
RESULTS: The mean duration of protected full weight bearing was 7 weeks. 94.12% patients were satisfied with the results, bony union was achieved in 98.31%. The Foot Function Index improved by 33 to a mean of 8 (p<.001). The postoperative Mayo Clinic Forefoot Score was excellent in 47.04 a

To answer the question whether it is possible to differentiate, by means of a high plantar nerve block in the hind limb, flexor tendon lameness from a suspensory ligament lameness, mid-plantar tendinitis or desmitis was induced with collagenase in five Standardbred horses in two trials. Before the induction of lameness, and on the fourth (D4) and fourteenth day (D14) after the induction of lameness the horses were evaluated subjectively (clinical lameness score), objectively (ground reaction force (GRF) measurements), and ultrasonographically. Clinical evaluation and GRF measurements were also

This study aimed to (1) investigate the plantar pressure characteristics of the speed step technique and (2) evaluate the effects of curved carbon fiber plates with varying stiffness on plantar pressure distribution and performance. Twenty-six healthy competitive male jump rope athletes completed plantar pressure measurement protocols. Each participant performed at least three trials of speed step sprints under three shoe conditions: control shoe (control), shoe with moderately stiff curved carbon plates (stiff), and shoe with highly stiff curved carbon plates (stiffest). Plantar pressure data

UNLABELLED: Forefoot and lesser digital pathology continues to be a challenging area of surgical correction for foot and ankle surgeons. Many techniques for the correction of digital deformities secondary to plantar plate rupture, regardless of planal dominance, have been described including direct repair and metatarsal shortening osteotomies for repair. The authors present a new technique for multiplanar correction of deformed lesser digits without direct repair of the plantar plate rupture utilizing a specialty suture. The technique utilizes a braided synthetic polyethylene Nylon suture, whi

OBJECTIVE: The objective of our study was to assess the diagnostic performance and associations of the direct and indirect MRI features of the metatarsophalangeal (MTP) joint that are thought to be related to tears of the plantar plate (PP) using surgical findings as the reference standard.
MATERIALS AND METHODS: We retrospectively included 23 patients with symptomatic instability of lesser MTP joints who had undergone preoperative 1.5-T MRI and surgical assessment. The MRI examinations were independently assessed by two musculoskeletal radiologists. Using the surgical data as the reference s

The study was performed to investigate a new bone-ligament-bone autograft from the plantar plate of the toes. The anatomic properties of the plantar ligaments and the technical feasibility to harvest a bone-ligament-bone graft were examined to evaluate the potential use of this graft for a suitable reconstruction of the scapholunate (SL) interosseous ligament. The plantar plate of the metatarsophalangeal joints of the second to fifth toe and the proximal interphalangeal joints of the second to fourth toe were examined in 20 cadaver feet (15 fresh and 5 embalmed cadavers) and measurements such

UNLABELLED: There is uncertainty regarding the most accurate and cost-effective method for diagnosing plantar plate injuries within the foot. The purpose of this study was to examine the combined value of using clinical and radiographic findings to diagnose high grade tears (> 50% disruption) within the second metatarsophalangeal (MTP) joint. Ninety-eight consecutive patients (117 feet) who underwent corrective surgery for plantar forefoot pain at a single foot and ankle specialty clinic were included in this retrospective analysis. All patients received a structured intraoperative assessment

BACKGROUND: Derangements of the plantar plate and joint capsule are an underrecognized cause of lesser metatarsalgia. Fluoroscopic arthrography and magnetic resonance (MR) arthrography are both used for diagnosis. Currently there are no studies comparing the effectiveness of these two modalities.
METHODS: Patients suspected of having plantar plate or capsular tears underwent both fluoroscopic arthrography and MR arthrography; the imaging findings were then compared and correlated with intraoperative findings, when available, to evaluate the effectiveness of the different imaging modalities. INTRODUCTION: The modified Lapidus arthrodesis, involving the first cuneo-metatarsal joint, is a well-established surgical method and widely utilized for treating moderate to severe hallux valgus deformities with hypermobility in the first tarsometatarsal joint. The purpose of this study was to assess the rate of union following the Lapidus procedure using a plantar plate and an immediate full weight-bearing protocol. Secondary objectives included examining radiological corrections and potential associated complications.
METHODS: A retrospective study included 66 patients (80 feet) who underw

The plantar plate provides a substantial static support for the lesser metatarsophalangeal joints. Insufficiency involving tear, attenuation, or absence of this structure can result in significant sagittal plane instability and deformity. When a plantar plate tear is established and is unresponsive to conservative treatment, plantar plate repair is indicated to address symptoms and reestablish static joint stability. The authors hypothesized that combined plantar plate and hammertoe repair with flexor digitorum longus tendon transfer provides a viable surgical option to address chronic plantar

BACKGROUND: The literature for a first metatarsocuneiform joint (MCJ) arthrodesis procedure and an early weight bearing post-operative regime (<2 weeks) has been systematically reviewed [1,2]. The concern regarding an increased risk of non-union with early weight bearing has largely been anecdotal. This is a single surgeon retrospective review assessing the union rates achieved following a Lapidus (1st tarsometatarsal joint) arthrodesis in the treatment of Hallux valgus using an anatomically contoured plantar plate and applying an early weight-bearing regime.
METHODS: A retrospective study of

In this paper, we report two cases of dislocation of the hallux interphalangeal joint with interposition of the plantar plate including the sesamoid bone. These dislocations were treated with open reduction: one through lateral approach and the other through plantar approach. Plantar approach afforded repair of the plantar plate with preservation of the sesamoid bone and satisfactory result.

Metatarsophalangeal (MTP) instability is a common cause of metatarsalgia of the lesser toes. Instability often presents as pain in the plantar forefoot, progressing to coronal and transverse plane malalignment. MTP joint dislocation can develop in some cases. The plantar plate plays an integral role in MTP joint stabilization, and injury to the plantar plate is the main contributor to the instability noted at the MTP joint.

Acquired flatfoot deformity in adults is usually due to partial or complete tearing of the posterior tibial tendon, with secondary failure of other structures such as the plantar calcaneonavicular (spring) ligament (SL), which maintain the medial longitudinal arch. In flexible cases, the tibialis posterior can be replaced with the flexor digitorum longus. It is common practice to suture the SL directly in the case of a tear; however, if the tear is complete, suturing directly to the ligament alone will not be possible. Reconstruction of the ligament is needed; however, no validated methods are

Lesser metatarsophalangeal joint plantar plate degeneration and tear typically presents as an attritional pattern of capsuloligamentous deficiency in middle-aged patients or sports-related chronic injuries. Knowledge of the anatomy, pathophysiological basis, common patterns, grading and classification of these injuries, and indications for surgery will aid imaging interpretation in the preoperative setting. The acuity and extent of injury, tissue quality, and functional requirements of the patient influence clinical decision-making with respect to surgical management. This article provides an
