This paper reports two cases of orbital plasma cell tumours, one initially presenting as a localized lesion, but later developing dissemination and proving fatal within 7 months; the other presenting initially as a disseminated disease and apparently cured from that particular disease within 1 year, both on equivalent treatments. The importance of investigations for general disease (for therapeutic reasons) is emphasized as well as possibility of renal involvement and the unpredictability of the clinical course.

Plasma cell leukemia (PCL) is a form of high-risk multiple myeloma comprising 1%-2% of new myeloma diagnoses, and defined by ≥ 5% circulating plasma cells. Plasma cell leukaemia is an aggressive plasma cell malignancy characterised by the presence of a high percentage of circulating clonal plasma cells on conventional peripheral blood smear examination. A 10 years old girl presented with fever, cough, haemoptysis, dyspnoea on exertion of two months duration. Chest skiagram and CT-scan of thorax revealed well defined mass lesion of varying density occupying right upper and middle lobes with areas of calcification. On thoracotomy, a huge mass in the right upper lobe was resected and hispathological examination proved it to be a case of plasma cell granuloma.

We describe a case of plasma cell leukemia associated with myelofibrosis. A 60-year-old woman was admitted due to lumbago and monoclonal hypergammaglobulinemia. Peripheral blood showed about 40% of plasma-cell-like cells. A bone marrow aspiration was dry tap. The patient was diagnosed as having plasma cell leukemia with myelofibrosis by bone marrow biopsy. Twenty-one patients with plasma cell tumors received vindesine (VDS) at the dose of 3 mg/m2 i.v. on day 1 plus prednisone at the dose of 100 mg p.o. from day 1 to 5, recycling every 8 days 3 times and then every 10-12 days. In 3 patients with gastric or duodenal ulcer prednisone was not administered. All but one patient were heavily pretreated and resistant to M-2 regimen. Diagnosis of myeloma depends on identification of malignant plasma cells and the product of these cells, a monoclonal immunoprotein. Of the clinical manifestations of plasma cell myeloma, skeletal pain and anemia are two of the more common. Unexplained anemia and osteoporosis noted in the elderly should suggest the possibility of myeloma; this combination of symptoms certainly warrants obtaining a protein electrophoresis. Hypercalcemia and renal insufficiency are frequent sequelae of myeloma.
