PURPOSE OF REVIEW: The causes of exudative pleural effusions are diverse and frequently remain unclear despite exhaustive examinations. Recently recognized IgG4-related disease (IgG4-RD) is a fibroinflammatory disorder that can affect nearly any organ including the lungs. This review will focus on the involvement of IgG4 in exudative pleural effusion of unknown cause.

Malignant pleural effusion (MPE) is a very disabling condition that often affects patients with advanced neoplasm. Conservative approach, repeated thoracentesis, pleurodesis and use of indwelling pleural catheters (IPC) are the main methods to deal with this condition. The ideal treatment must focus on symptom relief and has to take into account patient underlying diseases, performance status and necessity of adequate tissue sample for diagnosis. In a video we show techniques to perform video assisted thoracic surgery (VATS) pleural biopsy and talc poudrage pleurodesis.

The occurrence of microfilaria in pleural fluid is rare. Filarial lung involvement occurs in the form of Tropical Pulmonary Eosinophilia with pulmonary infiltrates and peripheral eosinophilia. A 16-year-old Caucasian male presented with non-specific acute respiratory symptoms, on a background of irritable bowel syndrome and type 2 diabetes mellitus. Clinical examination and initial imaging were highly suggestive of a pleural effusion. Routine confirmatory ultrasound imaging prompted subsequent computed tomography scanning, revealing striking images of a Bochdalek hernia, therefore enabling appropriate subsequent management and avoiding patient harm.

Based on literature data the authors demonstrate the complexity of differential diagnosis of pleural effusion syndrome (PES). At the same time they emphasize the quantitative increase in patients with splenic abscess (SA). The authors give the observation of SA, which is complicated by purulent pleurisy and pleural effusion syndrome. They accentuate erroneous diagnostic actions.

A 44-year-old man visited our hospital because of right chest pain. Pleural effusion in the right lung was detected on a chest radiograph. A chest CT scan demonstrated no abnormal lesions in either lung field, but passive atelectasis due to the pleural effusion was present. Sjögren's syndrome (Sjs) can cause many organic changes, but is rarely accompanied by pleuritis. We report here a 62-year-old patient with subclinical Sjs who developed unilateral pleuritis with moderate effusion. We have described a 10-year-old child in whom a pleural effusion complicated an otherwise typical case of IM. Although rare, pleural effusions are a possible complication of infectious mononucleosis, and IM should be considered in the differential diagnosis of pleural effusions in children.
