Adams in 1840 was the first to describe popliteal cysts, "the enlarged bursa is normally situated beneath the inner head of the gastrocnemius and communicates with the joint by a species of valvular opening." Baker whose name has been given to the cysts suggested in 1877 that the cyst may be a distended bursa related to the semimembranosus tendon. He also suggested that the cyst was connected with the knee synovium and pointed out that the fluid could not return to the joint. He considered that rupture could occur with the formation of calf cysts and that the leak could come from the popliteus…

Arthrography revealed extended meniscus lesions in 68 of 78 patients with popliteal cysts. In the remaining cases mechanical damage or inflammatory diseases of the knee were responsible for cyst genesis. Popliteal cysts should be removed only after causal diagnosis of the underlying chronic relapsing effusion leading to increased joint pressure and cystic dilatation of the often communication submembranous bursa.

This retrospective review of 46 popliteal cysts seen in 44 patients at a pediatric orthopedic referral center clarifies the presenting symptomatology, associated conditions, and likely duration of a popliteal cyst, and the need for intervention. The patients were followed up for more than 1 year. Thirty-six of the 46 cysts with duration data showed no resolution of the popliteal cyst during the period of observation (average, 32.1 months). The 10 cysts that showed clinical resolution were present for an average of 16.9 months. The benign course of action for virtually all patients obviated the…

Baker's (Popliteal) cysts are frequently encountered on cross-sectional imaging of the knee. These consist of enlarged gastrocnemius semimembranosus bursa which typically communicate with the knee. They may be imaged with a variety of techniques including arthopgraphy, CT, ultrasound and MRI, with the latter two being more commonly used. Examples of bursa imaged with all of the above techniques are demonstrated in the essay, as well as variance of normal appearance and pathological entities including ruptured bursa, abnormally positioned bursa, osteochondromatosis, septic complications of the

Popliteal cysts are soft fluid-filled lesions of synovial origin which result from extrusion of joint fluid into the gastrocnemiosemimembranous bursa. They may occur in any age group, but 22-33 % occur in the first 15 years of life. In this age group they are rarely associated with intraarticular abnormalities and therefore rarely require treatment. This case report shows the magnetic resonance imaging (MRI) appearances of a popliteal cyst in two children.

INTRODUCTION: Pigmented villonodular synovitis is an uncommon disease characterized by hyperplastic synovium, large effusions and bone erosions. It commonly mimics other conditions. As a result, the diagnosis may remain elusive, as the classic signs of tenderness and effusion are not specific. Occasionally, PVNS presents as a popliteal cyst, which may divert attention from intra-articular pathology. In this article, we describe a case of pigmented villonodular synovitis that presented with a popliteal cyst as the chief problem.
