Integrated Prenatal Care for Women Living With HIV: Primary Care Outcomes in Saskatoon, Saskatchewan.

Saskatchewan has the highest rate of new human immunodeficiency virus (HIV) infections in Canada. Of those newly diagnosed, 56% identify as female, 76% identify as Indigenous, and 71% report a history of intravenous drug use. These statistics are strikingly different compared with Canadian data. This brief communication describes prenatal care provided to women living with HIV at an interdisciplinary primary care clinic in Saskatchewan, demonstrating that, despite facing great barriers such as housing insecurity, substance use, and institutionalized racism, women living with HIV can have posit…

Evidence-based prenatal care: part II. Third-trimester care and prevention of infectious diseases.

All pregnant women should be offered screening for asymptomatic bacteriuria, syphilis, rubella, and hepatitis B and human immunodeficiency virus infection early in pregnancy. Women at increased risk should be tested for hepatitis C infection, gonorrhea, and chlamydia. All women should be questioned about their history of chickenpox and genital or orolabial herpes. Routine screening for bacterial vaginosis is not recommended. Influenza vaccination is recommended in women who will be in their second or third trimester of pregnancy during flu season. Women should be offered vaginorectal culture s

Effects of prenatal care on child health at age 5.

The broad goal of contemporary prenatal care is to promote the health of the mother, child, and family through the pregnancy, delivery, and the child's development. Although the vast majority of mothers giving birth in developed countries receive prenatal care, past research has not found compelling evidence that early or adequate prenatal care has favorable effects on birth outcomes. It is possible that prenatal care confers health benefits to the child that do not become apparent until after the perinatal period. Using data from a national urban birth cohort study in the US, we estimate the …

Pregnancy and alcohol use: evidence and recommendations for prenatal care.

Pregnancy alcohol consumption has been linked to poor birth outcomes and long-term developmental problems. Despite this, a significant number of women drink during pregnancy. Although most prenatal care providers are asking women about alcohol use, validated screening tools are infrequently employed. Research has demonstrated that currently available screening methods and intervention techniques are effective in identifying and reducing pregnancy drinking. Implementing universal screening and appropriate intervention for pregnancy alcohol use should be a priority for prenatal care providers, a…

Disparity in prenatal care among women of colour in the USA.

OBJECTIVE: To describe the disparity in prenatal care among women of colour in timing of initiation of prenatal care and total number of prenatal visits. DESIGN: A retrospective, descriptive design. SETTING: A large, urban university midwifery faculty practice. PARTICIPANTS: 439 healthy women at term (37-42 weeks gestation) with a vertex singleton pregnancy, and an essentially uncomplicated prenatal course. One clinic, the university facility, provided full-scope services. The other four community clinics, all outside the university in the larger metropolitan area, were designed to provide …

Lack of prenatal care in a traditional society. Is it an obstetric hazard?

OBJECTIVE: To examine the association between lack of prenatal care (fewer than three visits at any prenatal care facility) and perinatal complications in the Bedouin parturient population where lack of prenatal care is not associated with absence of prenatal services, substance abuse or marital status. STUDY DESIGN: The study population consisted of all Bedouin women lacking prenatal care (n = 7,601) who gave birth between the years 1990-1997 in the Soroka University Medical Center. The analysis consisted of a comparison of labor and delivery outcomes in women without prenatal care to outcom…

[Current prenatal care with reference to state of the art knowledge].

Every physician taking care of pregnant women believes to be well acquainted with the state-of-the-art requirements. However, changes imposed by the new Swiss health insurance legislation (KVG) and by the pressure of Evidence Based Medicine brought these into a new scope. In Switzerland there is no general agreement on optimal pregnancy-care. We propose a care-standard, based on rational and efficient clinical, laboratory and sonographic checks, which allow a timely diagnosis of pregnancy-at-risk. Targeted laboratory investigations as well as the anti-D-immunoprophylaxis, and screening for dia

[Is diagnosis of toxoplasmosis within the scope of prenatal care meaningful?].

In the course of prenatal care, the sera from 5670 pregnant women were investigated. The average infection rate in the population was 39.22%; 60.39% were seronegative. To minimise the risk involved in toxoplasmosis infection, toxoplasma-antibodies must be definitely determined during prenatal care. This should be carried out simultaneously with the determination of antibodies to the rubella virus.

Prenatal care utilization for fetuses with spina bifida in California.

BACKGROUND: To (1) compare prenatal care (PNC) utilization of women carrying fetuses with and without spina bifida (SB), (2) identify factors associated with less than adequate PNC in the SB group, and (3) correlate neonatal complications with PNC utilization in the SB group. METHODS: Retrospective cohort study using data from the California Department of Health Care Access and Information Database for all liveborn infants in 2005-2012. We compared women carrying fetuses with SB to women carrying fetuses without SB. The primary outcome was PNC utilization, assessed using the Adequacy of Prena…

Syphilis in pregnancy: A practical guide for prenatal care providers.

Syphilis during pregnancy remains a persistent global public health challenge. Untreated or inadequately treated syphilis infection during p … ates for universal early screening, strengthened health systems to ensure access to free and timely prenatal care, and the integration of effective follow-up strategies. As early diagnosis and treatment are highly effective in reducing transmission, syphilis screening should begin at the first prenatal visit. While regional protocols vary, at a minimum, screeni …

The impact of prenatal care on pregnancy outcomes in women with depression.

OBJECTIVE: Depression occurs in 7-13% of pregnant women and is associated with increased risk during pregnancy including increased rates of cesarean delivery, preterm birth, and preeclampsia. Prenatal care is thought to decrease adverse outcomes in pregnancy. This study aimed to examine how delayed access to prenatal care affects maternal and neonatal outcomes in a cohort of women suffering from depression. STUDY DESIGN: This is a retrospective cohort study of linked vital statistics and hospital discharge data among singleton, non-anomalous births in California between 2005 and 2008 comparin…

Prenatal counseling: Guidelines for the care of people with spina bifida.

As the diagnosis of Spina Bifida (SB) is often made prenatally, SB-specific prenatal counseling is needed. It is essential to provide information about medical care and lifelong impact of this diagnosis, treatment options available to women carrying fetuses affected, and resources that will assist in the care of individuals with SB. This article outlines the SB Prenatal Counseling Guidelines from the 2018 Spina Bifida Association's Fourth Edition of the Guidelines for the Care of People with Spina Bifida and acknowledges that further research in SB prenatal counseling is warranted.

Improved Value of Individual Prenatal Care for the Interdisciplinary Team.

OBJECTIVE: Innovative models of prenatal care are needed to improve pregnancy outcomes and lower the cost of care. We sought to increase the value of traditional prenatal care by using a new model (PodCare) featuring a standardized visit schedule and coordination of care within small interdisciplinary teams in an academic setting. METHODS: Prenatal providers and clinic staff were divided into four "Pods". Testing and counseling topics were assigned to visits based on gestational age. Interdisciplinary weekly Pod meetings provided coordination of care. A retrospective chart review was performe…
