Guidelines for managing pressure ulcers with negative pressure wound therapy.

Pressure ulcers are a serious health issue, leading to clinical, financial, and emotional challenges. Numerous treatment modalities are available to promote wound healing, yet clinicians may be unsure how to incorporate these treatment options into an overall plan of care for the patient with a pressure ulcer. A consensus panel of experienced wound care clinicians convened in July 2004 to review the mechanisms of action and research basis for one such treatment modality: negative pressure wound therapy. After answering key questions about this modality, they developed an algorithm to assist th…

Urethral closure pressure and leak-point pressure in incontinent women.

OBJECTIVE: To examine the association between maximum urethral closure pressure and Valsalva leak-point pressure in women with genuine stress incontinence (GSI). METHODS: Fifty-six women with GSI were evaluated in a urodynamics laboratory. Maximum urethral closure pressure and vesical leak-point pressure were measured with multichannel urodynamic testing. The correlation coefficient and regression line were calculated. RESULTS: Forty subjects demonstrated a leak on Valsalva maneuver and, in these women, maximum urethral closure pressure and leak-point pressures were related significantly (r …

Estimation of pulmonary artery pressure from pulmonary vein wedge pressure.

Correlations between pulmonary artery and pulmonary vein wedge pressures were investigated in 13 patients with atrial septal defect and 1 patient with Tetralogy of Fallot. Pulmonary vein wedge pressure wave form resembled that of pulmonary artery pressure, and the former lagged behind the latter by 70 to 110 msec (mean 88 +/- 14) as observed by the fluid-filled catheter system. Diastolic pulmonary artery and diastolic pulmonary vein wedge pressures were nearly identical. Although systolic and mean pulmonary artery pressures correlated well with respective pulmonary vein wedge pressures, there …

Independence of pressure reactivity from pressure levels in Tecumseh, Michigan.

The relation between blood pressure level and reactivity to mental arithmetic and isometric exercise was investigated in 169 men and 120 women (average age, 32.3 years) from the village of Tecumseh, Mich. In the entire population, the correlation between baseline blood pressure and blood pressure response to both stressors was not significant. Blood pressure reactivity to both stressors was not increased in participants with borderline hypertension (one clinic reading of more than 140 mm Hg systolic and/or 90 mm Hg diastolic). When subjects were classified according to blood pressure response …

Effect of positive-pressure ventilatory frequency on regional pleural pressure.

Regional lung ventilation is modulated by the spatiotemporal distribution of alveolar distending forces. During positive-pressure ventilation, regional transmission of airway pressure (Paw) to the pleural surface may vary with ventilatory frequency (f), thus changing interregional airflow distribution. Pendelluft phenomena may result owing to selective regional hyperventilation or phase differences in alveolar distension. To define the effects of f on regional alveolar distension during positive-pressure ventilation, we compared regional pleural pressure (Ppl) swings from expiration to inspira…

Comparing Central Aortic Pressures Obtained Using a SphygmoCor Device to Pressures Obtained Using a Pressure Catheter.

BACKGROUND: This study compared aortic pressures estimated using a SphygmoCor XCEL PWA device (ATCOR, Naperville, IL) noninvasively with aortic pressures obtained using pressure catheters during catheterization procedures and analyzed the impact of a linear-fit function on the estimated pressure values. METHODS: One hundred and thirty-six patients scheduled for cardiac catheterization procedure were enrolled in IRB approved studies. Catheterization procedures were performed according to standard-of-care to acquire aortic pressure measurements. Immediately after the catheterization procedure w…

Influence of positive end-expiratory pressure on intracranial pressure and cerebral perfusion pressure in patients with acute stroke.

BACKGROUND AND PURPOSE: We undertook this study to evaluate the influence of positive end-expiratory pressure (PEEP) on intracranial pressure (ICP) and cerebral perfusion pressure (CPP) in patients with acute stroke. METHODS: A total of 20 ventilated patients of a neurological intensive care unit were examined under a protocol entailing variation of PEEP to 4, 8, 12, and 4 mm Hg; mean arterial blood pressure (MAP), ICP, heart rate, and mean velocity of the middle cerebral arteries (V(m) MCA) were recorded. RESULTS: CPP significantly changed depending on the various PEEP levels. No significan…

Variable pressure support ventilation vs. biphasic positive airway pressure/airway pressure release ventilation in experimental acute respiratory failure.

BACKGROUND: Spontaneous breathing during mechanical ventilation (MV) can improve cardiorespiratory function and lung protection. Biphasic Positive Airway Pressure (BIPAP) and Airway Pressure Release Ventilation (APRV) are common MV modes that combine spontaneous breathing with controlled ventilation. Variable pressure support ventilation (PSV), which varies breath-by-breath pressure support randomly, has been shown to improve gas exchange and reduce lung injury compared to conventional MV modes. This study aimed to compare the short-term effects of variable PSV with BIPAP in a model of acute r…

Skin perfusion pressure, distal blood pressure vs. trans cutaneous oxygen pressure for predicting wound healing after major amputation.

OBJECTIVE: To compare measurements of skin perfusion pressure (SPP) and transcutaneous oxygen pressure (TcPO2) to predict postamputation wound healing according to amputation level. METHODS: This study was conducted as a prospective two-centre, head-to-head study. RESULTS: Fifty-two patients had SPP, TcPO2 measured (below and above the knee), and toe and ankle blood pressure measurements taken before major amputation. Paired measurements of SPP and TcPO2 were used to compare the methods. We found overall poor agreement between SPP and TcPO2 measurements, with crude agreement below the knee i…

Intraocular Pressure Matters.

To date, evidence from multiple randomized controlled trials has shown that effective intraocular pressure (IOP)-lowering therapy significantly reduces the risk of glaucomatous progression across all stages of the disease. Changes in IOP have a substantial impact on the load-bearing connective tissues of the optic nerve head (ONH), as well as the overlying neurovascular tissues of the ONH and retina. An initial treatment goal of reducing IOP by 25% to 35%-and possibly more in advanced cases-can help prevent progression. Additionally, maintaining stable IOP by minimizing both diurnal and long-t…

Normal Pressure Hydrocephalus.

PURPOSE OF REVIEW: Since it was first described in 1965, normal pressure hydrocephalus (NPH) has been a controversial subject. New studies have shed light on its epidemiology and pathogenesis and provided objective ways to measure outcome in patients with NPH. Neuroimaging has improved and allows better recognition of both NPH and the presence of overlapping diseases RECENT FINDINGS: Several recent epidemiologic studies confirm that NPH is a rare disease, but the presence of large ventricles is a common finding with aging. NPH may be multifactorial, including congenital causes, vascular diseas…

[High pressure trap].

Pheochromocytoma is a rare tumor potentially life-threatening and associated with non specific and diverse symptomatology. Cardiac symptoms may mislead diagnosis; they could manifest as myocardial sideration concomitant to a hypertensive peak or supraventricular arythmia. We report a case of pheochromocytoma associated with hypokaliemia revealed by a myocardial ischemia with acute cardiac failure and severe left ventricular depression and complete reversal after surgery.
