Propofol-induced priapism in a 25-year-old male confirmed by rechallenge is reported for its rarity and to create awareness among practitioners, because propofol is used frequently in India for the induction and maintenance of anesthesia or sedation. The probable mechanisms are highlighted. Because propofol causes low-flow priapism, early alleviation is essential to minimize and/or avert the long-term complications.

A patient taking oral risperidone while using cocaine and alcohol presented with priapism shortly after long-acting, injectable risperidone was prescribed. Another case of priapism was diagnosed in a sickle cell disease patient who was on oral risperidone. A penectomy due to necrosis was required in the first case, while the other patient regained erectile function. Priapism is persistent penile erection in the absence of sexual stimulation. The pattern of blood flow to the penis that occurs during normal erection is altered so that sustained priapism may result in edema, increased risk of abrasion, tissue drying and necrosis of the penis. Priapism of the clitoris is a rare entity. A case of painful priapism is reported in a patient who had previously suffered a radical cystectomy for bladder carcinoma pT3-GIII, followed by local recurrence in the pelvis. From a symptomatic point of view she showed a good response to conservative treatment (analgesics and anxiolytics), as she refused surgical treatment. She survived 6 months from the recurrence, and died with lung metastases. The priapism did not recur. The physiopathological mechanisms involved in the process are discussed and the literature reviewed.

The authors describe the development of conservative and surgical treatment of priapism. They analyze the aetiopathogenesis of its development. They report on their own experience with the surgical treatment in four patients, where they made three times a spongiocavernous anastomosis on one side according to Quackels' method, twice spongio cavernous anastomosis via the glands penis according to Ebbehøj-Winter. They prefer glandocavernous anastomoses because it is a simpler, less time consuming operation.

Priapism is a rare condition in childhood; the majority of reported cases are in boys with sickle cell disease, where conservative management is often appropriate. Our experience with 4 cases, with differing aetiologies, and a review of the literature highlight the need for aggressive therapy when conservative measures fail.

The association of priapism with trazodone is reviewed based on data reported to the Food and Drug Administration. The data suggest that priapism may be most likely to occur within the first 28 days of treatment and that the majority of cases occur with doses of 150 mg/day or less. All age groups appear to be vulnerable to this adverse effect. Patients should be informed of this potential side effect and instructed to discontinue the medication if any unusual erectile problems develop.
