Prurigo nodularis is a chronic skin condition characterized by severely pruritic nodules that cause a profound negative impact on quality of life. The second article in this 2-part continuing medical education series focuses on reviewing the pathogenesis of prurigo nodularis and exploring management algorithms for this condition. In addition, we discuss some emerging and novel therapies for treating prurigo nodularis. The first article in this 2-part series describes the broader epidemiology, patient demographics, physical examination findings, and symptoms to aid in the timely recognition and…

BACKGROUND: There are no studies on contact allergy in patients with prurigo. With itch being important in the pathophysiology of prurigo diseases and being a symptom of allergic contact dermatitis, we aimed to investigate contact allergy in patients suffering from prurigo.
OBJECTIVES: Exploratory analysis of patch test results in prurigo patients.
MATERIALS AND METHODS: A retrospective analysis of data of the Information Network of Departments of Dermatology, 2005-2014, was performed.
RESULTS: Of 116 744 patch tested patients, 639 (0.55%) were diagnosed with prurigo. The median age was 61 …

BACKGROUND/OBJECTIVES: This article gives an overview of the theoretical background and the empirical research concerning psychosomatic aspects of prurigo nodularis (PN).
MATERIALS AND METHODS: Literature research in PubMed with the search terms "prurigo nodularis", "nodular prurigo", "psych*"; clinical experience and research of the authors.
RESULTS: Although reviews on PN frequently mention psychosomatic aspects, there is little empirical research concerning the subject. Psychosomatic aspects of PN may play a role in (1) the etiology of the chronic pruritus; (2) scratching, which leads to …

BACKGROUND: Paraneoplastic cutaneous manifestations associated with hepatocellular carcinoma may allow early diagnosis and curative treatment. Chronic prurigo alone may reveal hepatocellular carcinoma hepatocellular carcinoma in the absence of any other abnormal laboratory results potentially indicative of hepatic disease.
CASE REPORT: A 79-year-old woman was referred to our unit for prurigo beginning 3 months earlier, occurring night and day and causing insomnia. The initial results were completely normal. In the absence of improvement under treatment with dermal corticosteroids, additional …

BACKGROUND/AIMS: Numerous data have suggested that there may be a relationship between Helicobacter pylori (H. pylori) infection and extragastric diseases, including dermatological pathologies. We studied some cases of Hide's Prurigo Nodularis (NP), a very itchy skin disease of unknown origin, in order to asses whether there is a pathogenic correlation between PN and H. pylori infection.
METHODOLOGY: Prurigo Nodularis had been diagnosed clinically and histologically in 42 outpatients (27 females and 15 males with mean age of 62 +/- 5 years). All patients were screened for H. pylori infection …

BACKGROUND: Increased numbers of dermal nerves have been demonstrated in prurigo nodularis and have been theoretically linked to the intense pruritus. We hypothesized that the neuronal proliferation in prurigo nodularis might be associated with an increased density of Merkel cells because they are also a component of the neurocutaneous system.
METHODS: We examined skin biopsy specimens from 20 cases of prurigo nodularis for Merkel cells with the use of a standard immunohistochemical assay (avidin-biotin-peroxidase complex system) with an antibody to cytokeratin 8 (CAM 5.2). Six cases of liche…

Twenty-two patients suffering from prurigo nodularis were treated with thalidomide 50-300 mg daily for an average of 12 months (two weeks to five years), following a special arrangement between the Danish Government and the producer Grünenthal GMBH, by which the Government took full economical responsibility for any claims related to the treatment. All patients received prior to treatment a detailed written information about all potential side-effects from the treatment, and had by their signature acknowledged, that they wished to be treated. All but two of the patients had an immediate pronou…

Thirty-two actinic prurigo patients of Cree ancestry underwent human lymphocyte antigen (HLA) typing and were compared with 32 control subjects of Cree ancestry. We found a significantly increased frequency of HLA-A24 and Cw4 antigens and a significant decrease in the frequency of the A3 antigen in actinic prurigo patients. These HLA associations may be helpful in determining whether actinic prurigo is a distinct disease or a variant of polymorphous light eruption.

Thirty-eight biopsies of prurigo lesions (22 large, mature nodules and 16 smaller, intermediately developed nodules) were obtained from 17 patients with idiopathic prurigo nodularis Hyde who had not been bitten by insects, nor had other underlying diseases. Each biopsy was serially sectioned to reveal its histological structure. In 88% (14/16) of the smaller nodules and in 55% (12/22) of the large ones, a hair follicle was observed in the center of the prurigo lesion. The follicular epidermis and the adjacent surface epidermis showed various degrees of acanthosis and spongiosis. The dermal inf…

Actinic prurigo can be defined as an idiopathic photodermatosis which usually starts in childhood, particularly affects the female, with some evidence for an atopic and familial photosensitivity background. It affects both exposed and covered skin but mainly the former and although it may be present all the year round is usually maximal on the exposed skin during months of sunshine. The changes are those of a chronic prurigo along with more acute episodes following sunshine exposure in which there is a background of edematous erythema. It usually lasts throughout childhood and adolescence, but…

Four patients with classic recalcitrant prurigo nodularis had symptomatic and physical responses to thalidomide with remissions. Three of the four patients had increased IgE levels that decreased during therapy. In two patients, short-term treatment (2 to 3 months) was not sufficient to produce remission, but retreatment was effective. Two patients had long-term remission with more than 6 months of treatment. No significant side effects occurred.

The case of a 72-year-old female with a syndrome of malabsorption associated with prurigo nodularis is reported. The patient had been suffering from these disorders for 16 years. The pathological alteration of her malabsorption syndrome was an idiopathic sprue. 5 months after treatment with gluten-free diet supplemented with vitamins and iron, the disappearance of the clinical and analytical alterations was complete.
