Tinnitus and Ménière's disease: characteristics and prognosis in a tinnitus clinic sample.

The aim of this study is to describe tinnitus epidemiological characteristics in Ménière's disease (MD), the prognosis and its interaction with other MD symptoms. It is a descriptive transversal study. One hundred two MD patients were referred to a tinnitus clinic. Tinnitus was evaluated according to psychoacoustical tinnitogram, visual analogue scale, and tinnitus handicap inventory (THI). All patients were diagnosed with definitive MD (following AAO-HNS' 95 criteria). Pitch was more commonly identified in low and medium frequencies. Psychoacoustical intensity was matched in 12.7 dB. The visu

[Pulmonary crackles, what does the clinician hear?].

INTRODUCTION: The overall duration of a pulmonary crackle is usually less than 20-30 ms but psychoacoustics demonstrates that an acoustical event with a duration of less than 20-40 ms cannot be estimated in terms of pitch and duration. We pose the hypothesis that the main resonant information is contained into the breath sounds following the crackle. METHODS: Eight patients with COPD, viral pneumonia, bronchiectasis, congestive heart failure, hypoproteinemia and fibrosing alveolitis were recruited for this study. Thirty-six crackles were analyzed in time and frequency domains; 12 in each cate…

[Relations between the summation potential and frequency selectivity].

In cases of inner ear diseases, e.g.m. Ménière, summation potential (SP) can be measured using the extratympanic elektrocochleographic method. This potential is a consequence of hydrops in the cochlea. In 128 patients with confirmed SP, it was also possible to measure frequency selectivity (FS) using the psychoacoustic method. Reduced FS was found to a greater or lesser degree in the basal and apical turn of the cochlea. It can be assumed that both phenomena are to be attributed to the same cause.

A Subcortical Model for Auditory Forward Masking with Efferent Control of Cochlear Gain.

Previous physiological and psychophysical studies have explored whether feedback to the cochlea from the efferent system influences forward masking. The present work proposes that the limited growth-of-masking (GOM) observed in auditory nerve (AN) fibers may have been misunderstood; namely, that this limitation may be due to the influence of anesthesia on the efferent system. Building on the premise that the unanesthetized AN may exhibit GOM similar to more central nuclei, the present computational modeling study demonstrates that feedback from the medial olivocochlear (MOC) efferents may cont

Improving Audiology Student Training by Clinical Simulation of Tinnitus: A Glimpse of the Lived Experience of Tinnitus.

PURPOSE: Student audiology training in tinnitus evaluation and management is heterogeneous and has been found to be insufficient. We designed a new clinical simulation laboratory for training students on psychoacoustic measurements of tinnitus: one student plays the role of the tinnitus patient, wearing a device producing a sound like tinnitus on one ear, while another student plays the role of the audiologist, evaluating their condition. The objective of the study was to test this new clinical simulation laboratory of tinnitus from the perspective of the students. METHOD: This study reports

A dominantly inherited progressive deafness affecting distal auditory nerve and hair cells.

We have studied 72 members belonging to a large kindred with a hearing disorder inherited in an autosomal dominant pattern. We used audiological, physiological, and psychoacoustic measures to characterize the hearing disorders. The initial phenotypic features of the hearing loss are of an auditory neuropathy (AN) with abnormal auditory nerve and brainstem responses (ABRs) and normal outer hair cell functions [otoacoustic emissions (OAEs) and cochlear microphonics (CMs)]. Psychoacoustic studies revealed profound abnormalities of auditory temporal processes (gap detection, amplitude modulation d

Otoacoustic emission as an indicator in evaluating inner ear impairments.

Stimulated otoacoustic emissions (OAEs) were recorded from subjects with various kinds of sensorineural loss. The interaural difference in the value of the OAE pseudothreshold was significantly higher in unilateral inner ear impairments, in which there was a positive correlation between the interaural difference in the values of the psychoacoustic threshold and of the OAE pseudothreshold. There were no interaural differences of OAE in the cases of unilateral functional deafness. The measurement of OAE may serve as an indicator for evaluating sensorineural hearing losses.

[Clinical value of tuning curves or frequency selectivity curves].

A simplified method, useful in clinical practice, is described in order to study the frequency selectivity in various conditions. The patient is asked to indicate the levels of pure tones which are necessary to mask completely the perception of an intermittent probe. The resulting psychoacoustic tuning curves are characterized by a more or less narrow bandwidth which reflects the ability to resolve simultaneously presented sounds. Several circumstances where these tuning curves are helpful, are presented: assessment of tinnitus, Menière's disease, unclear impairment of speech intelligibility.

Information in speech: observations on the perception of [s]-stop clusters.

A series of experiments is reported that investigated the pattern of acoustic information specifying place and manner of stop consonants in medial position after [s]. In both production and perception, information for stop place includes the spectrum of the fricative at offset, the duration of the silent closure interval, the spectral relationship between the frequency of the stop release burst and the following periodically excited formants, and the spectral and temporal characteristics of the first formant transition. Similarly, the information for stop manner includes the duration of silent

Gore-Tex Medialization Thyroplasty: objective and subjective evaluation.

Management of vocal fold paralysis and paresis continues to give rise to controversy and the use of numerous surgical techniques. The senior authors' (RTS, JRS) initial experience with fourteen patients using Gore-Tex Medialization Thyroplasty suggests that this technique has value. Evaluation included pre- and postoperative videostroboscopic assessment, aerodynamic measures, acoustic analysis, phonatory analysis, psychoacoustic evaluation, and patient self-evaluation. Stroboscopic and perceptual assessments were blinded. Statistical analysis supports the efficacy of this technique.

Minimal Clinically Important Difference of Tinnitus Outcome Measurement Instruments-A Scoping Review.

OBJECTIVE: Tinnitus assessment and outcome measurement are complex, as tinnitus is a purely subjective phenomenon. Instruments used for the outcome measurement of tinnitus in the context of clinical trials include self-report questionnaires, visual analogue or numeric rating scales and psychoacoustic measurements of tinnitus loudness. For the evaluation of therapeutic interventions, it is critical to know which changes in outcome measurement instruments can be considered as clinically relevant. For this purpose, the concept of the minimal clinically important difference (MCID) has been introdu

Clinical Validation of a New Tinnitus Assessment Technology.

Current clinical assessment of tinnitus relies mainly on self-report. Psychoacoustic assessment of tinnitus pitch and loudness are recommended but methods yield variable results. Herein, we investigated the proposition that a previously validated fixed laboratory-based method (Touchscreen) and a newly developed clinically relevant portable prototype (Stand-alone) yield comparable results in the assessment of psychoacoustic tinnitus pitch and loudness. Participants with tinnitus [N = 15, 7 with normal hearing and 8 with hearing loss (HL)] and participants simulating tinnitus (simulators, N = 15

Evaluation of acoustic characteristics of snoring sounds obtained during drug-induced sleep endoscopy.

INTRODUCTION: Snoring sounds are discussed to contain acoustic information about their geneses. Nocturnal snoring can easily be recorded acoustically but it is difficult to visually verify its genesis. Contrary, snoring patterns induced by drug-induced sleep endoscopy (DISE) can be visually differentiated. The aim of the study was to classify patterns of obstructions and vibration during DISE and to evaluate acoustic characteristics between these different patterns of snoring. METHODS: DISE was performed in 41 male patients with sleep-disordered breathing. The recorded video sequences (n = 10

[Auditory fatigue].

INTRODUCTION AND OBJECTIVES: Given the relevance of possible hearing losses due to sound overloads and the short list of references of objective procedures for their study, we provide a technique that gives precise data about the audiometric profile and recruitment factor. Our objectives were to determine peripheral fatigue, through the cochlear microphonic response to sound pressure overload stimuli, as well as to measure recovery time, establishing parameters for differentiation with regard to current psychoacoustic and clinical studies. MATERIAL AND METHOD: We used specific instruments for

Effects of continuous masking noise on tone-evoked magnetic fields in humans.

Two different types of steep loudness growth have been reported in detail in psychoacoustical studies but have rarely been evaluated by objective methods in humans. One occurs in inner-ear hearing-impaired patients and is known as loudness recruitment. Another similar phenomenon is observed in healthy subjects with concurrent presence of background noise. Concerning the first type, our previous study using magnetoencephalography (MEG) showed that enhancement of the dipole moment of N100m with increase in stimulus intensity was greater in patients than in normal individuals. However, it is uncl

Registers in Infant Phonation.

The primary vocal registers of modal, falsetto, and fry have been studied in adults but not per se in infancy. The vocal ligament is thought to play a critical role in the modal-falsetto contrast but is still developing during infancy (Tateya and Tateya, 2015).41 Cover tissues are also implicated in the modal-fry contrast, but the low fundamental frequency (fo) cutoff of 70 Hz, shared between genders, suggests a psychoacoustic basis for the contrast. Buder, Chorna, Oller, and Robinson (2008)6 used the labels of "loft," "modal," and "pulse" for distinct vibratory regimes that appear to be ident
